Team member was closing a door on the finish mill and caught fingertip between a wire sling causing a laceration.

While electrician was traveling to shipping, gusty wind conditions caused a dust particle to get into eye. Flushed but did not relieve irritation so went to emergency care and prescribed a prescription ointment.

Strained elbow working on kiln baghouse dust collector

During kiln outage 3 contractors in PH tower passenger elevator when it stopped moving between ground and 1st floor. When evident it would be >30 min, MSHA called between 1115 and 1120 pm per supervisor making call. Constant communication maintained until elevator repair person arrived on site and extracted employees at 1245 am

Employee was using a 3 lb hammer to form metal after welding it. Employee missed target with hammer and felt strain on elbow. Placed on work restriction, no lifting >5 # for next 7 days.

Maintenance mechanic was working at VRM roll 1 when a disruption in air flow caused the VRM to become positive, and dust escaped from an inspection door. The mechanic was exposed to hot dust, and was removed from the area. Upon evaluation, left wrist received a minor burn and mechanic was placed on restricted work duty.

While clearing a plug on the Stage 1-West vessel material flushed out of the vessel opening onto employees foot, causing a burn to EE's right foot.

Based on initial investigation, appears unknown hot material or object was inside alt fuel storage trailer. As material unloaded from trailer material/object exposed to oxygen causing spontaneous combustion. Dry chem initially extinguished the fire, but quickly reignited and spread. Local FD came on site to fully extinguish.

While lowering a steel cover from a roller, mechanic pinch EE's right middle finger between steel plates.

Mechanic was assisting welding when a piece of slag got into their eye.

Technician was tightening a screw in the electrical compartment of an hvac unit when technician's hand slipped and was cut on the housing of the cabinet.

Mechanic was cutting a conveyor belt using a razor knife when the blade slipped and cut left wrist. Bleeding was severe and mechanic was transported to emergency department where they received three sutures and a tetanus shot. Employee placed on light duty.

Contractor was preparing glassware for air sampling when it broke cutting right hand through gloves. Contractor received 15 stitches but there is no impairment.

Mechanic had to enter a conveyor chute to cut a section of chain from a sprocket. While exiting the chute, mechanic dislocated right shoulder and had to be transported to hospital to relocate.

Mechanic stuck in preheat tower passenger elevator for over 30 minutes when elevator stopped moving between 1st and 2nd floor. Elevator repair contractor brought on site to move elevator to floor where employee could safely exit. Maintained constant communication with employee during event.

2 employees were stuck in passenger elevator between 4th and 5th floor. Neither employee was in a life threatening situation. MSHA notified after 30 minutes. Employees able to safely exit elevator at approximately 7 a.m. Elevator contractor able to service and repair elevator to full function on 03/06/2021.

Mechanic replacing chain on primary crusher and while swinging sledge to join connecting link plate hammer hit pad eye causing hammer handle to impact hand caused bruising. Mechanic did not report injury until 01/19/21. After self-care did not help, placed on restrictions 01/25/21 resulting in medical incident.

Mechanic using pipe wrench to loosen blowpipe on dust collector and overextended EE's elbow. Incident was not reported until 01/08/2021. Employee instructed to have medical evaluation on 01/08/2021 and medical provider placed employee of work restrictions.

Employee was making a jig to hold drill bits, drilling holes into a board and had hand placed under the board, resulting in the tip of the drill bit puncturing EE's left hand and index finger.

Employee was walking through plant and felt sudden pain in lower back. Employee stated EE did not have any pain at start of shift and that EE did not slip or trip on anything to cause the sudden pain.

Clinker belt 5BC660 transporting clinker into Clinker Silo 42 caught fire required local fire department assistance to extinguish. FD on site at 09:40, MSHA notified 09:58, fire extinguished 10:16.

Employee tripped over metal plate, and then slipped in a mud puddle, causing EE to fall directly onto EE's left knee.

Employee clearing a blockage of coal at coal mill rotary feeder. 1 bolt was left in the door and swung the door to the side to start unplugging blockage. A surge of coal hit the table causing the mill to vibrate and the door to swing closed. Employee's hand was in the doorway and in return smashed three fingers, cutting the ring finger below the finger nail.

Mechanic removing pan from D8 dozer. When Mech removed last bolt holding pan, the pan slipped out of EE's hand pinching left hand between pan and ground, causing cut to hand. 1st Aid provided and sent to clinic for exam. No broken bones or sutures required, but placed on restricted duty for 3 days (no use of left hand).

Employee operating bobcat clearing material from vertical roller mill reject bin when blocked material flushed and hot powdery material entered cab of bobcat. Operator received 2nd degree burn to 2 fingers and thumb on right hand.

Mechanic working cleaning surface of expansion joint to being work and wind blew dust into left eye. Attempted to flush with water but could not remove particle, so went to medical clinic where the particle was removed. Prescribed antibiotic eye drops. Employee released back to full duty with no restrictions.

Mechanic working in vertical raw mill and had to climb up and down to access separator. Mechanic believes EE strained right arm during climbing. Afterwards EE thought it was just sore and did not report until 3/27/19. Mechanic referred to clinic for eval and was placed on work restrictions, no lifting >5 lbs.

2 employees carrying metal box down stairs. When 1st employee was setting EE's end of box down to ground it dropped approx. 3 inches from the ground while the 2nd employee was still holding. The employee still holding the box was jarred due to impact on ground and injured EE's elbow. On 3/11/19 employee was placed on work restrictions per medical assessment.

Empl did not report injury to Supv until 2/26/19; following med exam was placed on work restrictions 2/27/19. Empl using air lance to clear chute of material buildup and states after performing task over the course of several days began to experience persistent right shoulder pain. Employee is currently receiving physical therapy and is limited to push/pull/lift 5 lbs.

Cement transport driver walking down a flight of stairs after tank wagon was loaded and claims to have slipped on the 3rd step from the bottom of the stairs causing driver to fall. Driver indicated left knee was in pain & outside medical services were contacted. Driver was transported to Trident Hospital. After this, very little information provided to mine operator until 08-20-18.

Employee was working in Finish Mill building when EE started feeling dizzy. EE walked outside to get some fresh air. EE sat down to rest and tried to stand up EE realized EE was too dizzy, nauseated, and could not see very well. EE then laid down on the walk way until help could arrive and went to ER. Note: Employee did state at beginning of shift that EE was not feeling well

Employee was laying down on the floor cutting a belt guard off. Stood up from a kneeling position and half way they felt a sharp cramp like pain in the lower back. Employee notified Supervisor at lunch time and decided to sleep on the injury to make sure if it went away overnight. Employee then notified the supervisor the next day that it had got worse and called Health Bridge.

Mechanic trying to remove coupling hub from head shaft of limestone feeder. Using a strong back with a port-a-power ram and 1/2" all-thread rods to rig coupling for removal. While applying pressure to the ram, a rod broke & strong back swung around & hit mechanic on left hand. Treated as first aid but did not improve, instructed to go to doctor & x-ray found broken bone.

The mechanic was removing a pipe nipple with a 36" pipe wrench from the down comer duct and reported severe pain in abdomen. EE was sent to the doctors and put on light duty until further evaluation could be completed.

PA cut the air off of one of the airlines used to aerate the Fringe silo. Cement had backed up in the air hose and blew out of the hose going under their glasses. PA advised that they had been rinsing their eyes and had the cement out of the right eye but still had cement in the left eye. Sent to ER, given prescription and follow up with Ophthalmologist.

Mechanic was assisting with the installation of kiln main burner pipe. While loosening bolts on a rod, bolt broke loose and clinker dust dispersed into the air. The dust got under safety glasses and into left eye. Eye was flushed and mechanic went to doctor to have foreign debris removed. Had to return on 9/16 due to eye still bothering and was given prescriptions.

Weather do to effects of hurricane Irma caused short power outage, E-compressor needed to be restarted. While restarting compressor a chunk of material fell from above striking pcs on left shoulder. Called Heathbridge and received self-care instructions. Symptoms worsened overnight and was sent to doctor and put on light duty

Employee identified an uncontrolled conveyor belt fire at the weathered clinker conveying system. 911 called at 11:50 and MSHA called at 12:05. The fire was extinguished at approximately 12:20 however fire department staff remained on site to insure the fire did not restart. No one was injured as a result of this fire

Employee was working on Finish Mill 3 300 Dust Collector and dust fell into the right eye while changing out the bags. The mechanic left the scene with the help from partner and went to the eye wash station. EE flushed eye for ten minutes. EE still had some irritation to eye and ee was taken to see a doctor.

Two employees were lowering metal screening roll down from rack in storeroom. EE stated the material was awkward and heavy. EE turned wrist while lowering the material. EE told supervisor of the incident, but did not feel it needed further action. The EE administered home first aid. On 5/18/17 the EE told supervisor symptoms were worse so EE was sent for medical treatment

Employee made 3 pt turn after dumping material, hit a pothole that jarred and twisted their back. After getting a few buckets of marl, noticed back was starting to hurt but did not report incident at the time. After going home and sleeping, back was still hurting. Employee worked next shift on 3/19. During 3/20 shift pain had worsened and health bridge was called.

Employee went out on Short Term Disability thought to be kidney stone related in March 2017. At some point, treatment changed to back. Company was notified 5/10/17 that the employee believed injury was from operating excavator on 3/2/17, although no incident was reported by employee. As a precautionary measure we are filing this report.

Preheater elevator stuck between the 1st and 2nd floors with contractor inside for approx. 30 mins when MSHA was contacted. Elevator emergency brake faulted when someone attempted to open the door while elevator was in operation. Elevator brake was reset and car was brought to ground. No injury resulted.

Employee was removing putlog pipe on 3rd floor. Employee was pulling on one side. When beam on opposite side of vessel came off refractory on port, the pipe came up and struck employee in right front tooth and chipped it.

Two contract mechanics were assigned to trim the teeth of the head sprocket to the ASF bucket elevator. The ASF system was offline at the time of incident. Sprocket bolt was cut with a torch, which fell down the ASF elevator shaft, igniting ASF material approximately half way down the shaft. The local fire department was called and fire was extinguished completely by approx. 13:00.

While sitting on a scaffold board, ee began removing an elevator bucket to gain access to the head segments. The hot slag fell onto left pants leg at knee. Once ee saw the slag on pant leg, ee tried to knock it off. It fell through pant leg into boot, causing the hot slag to burn the top of foot.

While replacing inboard bearing assembly on VRM KF pump, mechanic used 1/2 in pull handle to try to remove 3 bolts. Mechanic felt discomfort in rt shoulder, but finished EE's shift without reporting any concerns. Overnight EE felt increased pain in shoulder and reported incident to EE's supervisor the next day. On 11/22 EE was placed on restricted duty after a follow-up exam

Supervisor observed hot spot at precal pulverized fuel bin and flames near insulation. Instrumentation indicated possible smoldering inside the bin. Coal mill equipment was shut down, CO2 fire suppression system activated and system extraction was maximized to remove hazard. Fire dept called at 03:00 and fire extinguished by 03:05. No one was injured as a result of the fire.

While was working at the ASF elevator employee was lifting a tool bag weighing approx. 60 lbs., felt pain in upper back between shoulder blades. Called Healthbridge and was told to administer 1st aid but condition did not improve. Healthbridge sent employee to urgent care. Employee has trapezius muscle strain and was placed on modified duty.

Employee had been lifting heavy items including conveyor screw and chain fall. Items felt heavy but did not have any pain on 6/21. Went home and woke up 6/22 with tingling in the left arm and leg along with pain in upper and lower back. Called HealthBridge and went to the doctor. Doctor gave employee medication, requested additional testing and was put on restricted duty.

employee was bolting up a box. While bolting the box another employee began to unstrap the load. When this happened the duct the box was attached to rolled over onto EE's left thigh cutting upper left thigh.

EE used a forklift to move pallet of motor parts (each < 30 lbs) to metal recycling. EE tossed each part in container. EE turned to walk back to forklift and felt sharp pain in back. Next day pain had worsened. Called Healthbridge and told to self-care with OTC pain medicine. On 5/23/16 EE still had pain, went to Dr. and placed on light duty and prescribed medicine.

Employee was working in the VRM on 12/21/15. While walking underneath scaffolding he struck his hard hat on a support and felt his neck jam from the impact. Thought pain would go away and didn't report. Neck pain became worse and started having pain radiating to shoulder. Reported to supervisor on 1/14/16 and was sent to doctor. Follow up visit on 1/25/16 and given modified duty.

Cement silo elevator stuck on way to top of silos with 3 employees inside for approx. 5 hrs, 15mins. MSHA contacted at 25 mins. No injury resulted. Elev tech replaced faulty circuit board for door lock safety. Board failure activated car safety device.

Employee & two co-workers were clearing out south VRM reject chute. Employee and one co-worker were below the chute and the other co-worker was behind the chute. Large amount of powder material flushed out. Material entered top of employee's boot, burning left foot. Administered 1st aid and was transported to ER for burn treatment

While moving boom lift on wet road lift dropped into a pot hole bouncing employee up causing him to twist his foot on accelerator cover plate.

4 Prod Asst were installing kiln hood beams. IE was operating the grip hoist on the west side of kiln hood. EE turned from facing kiln to find IE on his knees with head on ground. His ear was cut and he was confused. IE doesn't remember what happened. It is likely that the grip hoist handle struck IE when the cable twisted.

Slippery surface.

EE was welding a double plate to inside of wheel loaders bucket. Employee was welding for appox. 3 hrs. in slightly bent position at the waist. When repositioning his body he felt sharp pain in his lower back. He reported incident to supervisor before the end of his shift. The next morning came in and he requested to see a Dr. as his condition was worse.

EE installing buckets on elevator chain on clk S elevator. Wind shifted and dust went toward EE face and eyes from N elevator (via S elevator casing). EE washed eyes out, got goggles and finished with no issue. Tues EE worked shift with no issue. Started to have eye discomfort Tues night. Went to doctor Wed; diagnosed with abrasion in right eye

Employee was welding beams on a structure when he lost his footing and grabbed a near by beam for support. The sudden movement caused a muscle contusion.

Employee was removing 1st row of mill shell liners from compartment 1 on finish mill 3. Bull-pin & hammer was used to remove wedged mill balls found between liners & head liners. When striking bull-pin with the hammer the lodged mill ball came loose causing the bull-pin to ricochet hitting him in the left knee. Completed shift & once home had pain in knee. Went to doctor 11/25.

Reportable Hearing Loss to Lab Employee

While changing a flat tire, the jack failed trapping the workman's arm between the tire and the wheel well. Laceration to forearm/wrist.

Preheater elevator stuck between the 4th and 5th floors with employee inside for approx. 45 mins. MSHA contacted at 30 mins. No injury resulted. Elevator closed door contact was misaligned due to door slamming and elevator interlock stopped operation. Contact has been realigned.

Contractor was removing strap from bars being hoisted up onto the cooler deck. Bar slipped and cut his RT hand, 2nd finger. He realized he was cut(he had work gloves on) he went to 1st aid room and applied bandages. When he changed bandage later at home it was still bleeding. He sought medical help and received a tetanus shot, was re-bandaged and RTW the next day on modified duty.

Employee was helping lift a piece of handrail when he pulled a muscle in his back.

Employee was removing a diagonal brace on the recycle tower to make room to set a conveyor head section. The bolt started to spin. The employee placed a spud wrench on the nut and used a battery powered ratchet to loosen the bolt. When the nut came off the bolt the brace dropped mashing the employee's Rt. middle finger against the spud wrench.

Kiln stopped due to Kiln Feed PLC failure. Calciner pressure became positive with ID fan stoppage. Alternative solid fuels in the injection screw started to smolder. Due to PLC failure, fire protection system was not available to stop smoldering. Smoke observed in preheater from ground level and fire department called. No one was injured as a result of the fire.

While employee was on his afternoon break, he was slicing up a watermelon to eat. When he sliced into the watermelon he accidentally lacerated the tip of his left pinky finger.

The employee was laying on his back grinding the underside of handrail. Though he was wearing safety glasses and a full face shield a foreign object got into his left eye.

Employee was loading a truck out of silo 8. The gate was stuck open and material did not stop flowing. Employee went out of the office, thru the center bay door and up the ladder to close the gate. As he was coming back down the ladder he pulled/strained his right calf muscle.

Employee was loosening a port cover with a pipe. In the process a piece of welded rod broke off and struck him in the nose.

Reportable Hearing Loss to Lab Employee

An employee of Love Mechanical Cut the back of his leg with his pocket knife while cutting a rope. The cut required stitches.

Employee was with 3 production assistants removing a support beam from the kiln firing hood. They were using a metal cable, pulley block and forklift to pull the beam. Employee was holding the cable to guide it through the pulley block. The cable moved and his hand was pulled into the pulley block causing serious crush injury to right hand. Job was stopped and EMS was contacted.

Employee was inspecting a "SK" type valve on storage silos and requested another mechanic to assist in changing the valves position. Controls for the motor where on the opposite side of the pipes from the inspection location. Device was jogged 3 times and fingers were pinched in the valve. Assistant disconnected the mechanical arm to release, EMS was called and 1st aid was applied.

EMPLOYEE WAS CLIMBING THE STAIRS WITH AN AIR HOSE IN HIS HAND AND MISSED A STEP AND FELL.

Employee was working on conveyor for Finish Mill 3. Complained of trouble breathing and needing a break. Coworker took him to break room where employee complained of cramping. Call EMS and was given fluids intravenously and taken to hospital where he was given additional fluids and released.

EMPLOYEE WAS WORKING WITH A JACK HAMMER AND THE DUST GOT ON HIS SKIN CAUSING IT TO ITCH. WHILE ITCHING IT STARTED TO BURN WHICH CAUSED BRUISING.

EMPLOYEE WAS WEIGHING CEMENT BAGS IN THE PACK HOUSE WHEN HIS ABDOMEN AREA AND SCROTUM STARTING SWELLING.

Employee was checking on lack of flow of cement at silo(#3). Opened pie plate on feed box and it was full of feed. Started to remove feed and once pressure was released the feed blew back into his face. Employee had on safety glasses but still got dust in both eyes.

MOVED A DRUM THE WRONG WAY HEARD HIS BACK POP. THE FOLLOWING DAY HE WAS LIFTING SOME BAGS AND AGGRAVATED THE BACK PAIN.

HELPING A PA PULL A PIPE OUT THE TA DUET. HIS HAND WAS ON THE TIP OF THE PIPE AND WHEN THEY WERE PUSHING THE PIPE HIS FINGER HIT THE RAILS.

DEHYDRATION

A glass stem on an impinger broke while the sampling specialist was assembling a sampling train. The impinger apparently had a weakness that was not spotted prior to its use. The individual required seven stitches to his hand. All glassware used in sampling trains was inspected following this accident. All glassware will be inspected more closely in the future.

Two employees were cleaning the pre heater on the 3rd floor. Employee tried to close a door, which required working above his head. Door latch inconveniently placed and could not be closed it with one hand. Using his left hand, he attempted to hold the latch out of the way and slammed the door shut with his right. The door nipped the tip of his left thumb.

EE WAS WORKING ON THE FINISH MILL FEED CONVEYOR REMOVING CLINKER BUILD UP FROM UNDER THE BELT. HE WAS PRYING A PIECE OF CLINKER WITH A SHOVEL AND ANOTHER EMPLOYEE WAS ON THE OTHER SIDE DOING THE SAME THING WHEN HIS SHOVEL SLIPPED HITTING EE IN THE LEFT KNEE.

Fire started in the Alternate Solid Fuels facility in the area of the magnet belt for the west shredding line. The dry chemical fire suppression system discharged at the sensing of fire, but did not extinguish the burning conveyor belt. The entire facility was deemed no longer usable and in need of demolition. No one was injured as a result of the fire.

Employee was working to seal an opening in a meal pipe in the Preheater Tower using ceramic wool. While preparing to apply sealing materials, he disturbed dust from an overhead air line and some material entered his right eye. The employee was wearing a face shield, Nomex hood & jacket and safety glasses, but was turned to the side when material fell from above.

JACKHAMMERING ON THE CATWALK A LIGHT GLOBE FELL FROM ABOVE HITTING EMPLOYEE ON THE NECK.

EE WAS ANOTHER WORKER IN THE FM2 COOLER. WHEN HE WAS WALKING DOWN TO THE BOTTOM COOLER WHEN HE YELLED FOR SOMEONE TO DROP THE BRUSH. SOMEONE DROPPED A PIECE OF STEEL KNOWN AS A FLAT BAR. WHEN IT FELL IT HIT HIS RIGHT WRIST

Employee was walking on a 8" wide concrete curb by 18" high. He tripped over a piece of rebar that was covered by weeds and fell. He bruised his knee and wrist. Following a visit to the doctor a couple days later, an x-ray revealed a fracture in the wrist. The employee did not lose work hours as a result of the incident.

While emptying pulverized coal from storage tanks as part of a kiln shutdown procedure, an excessive amount of coal dust was pumped to the precalcer, resulting in a significant puff. No injuries were sustained as a result of the incident. There was no equipment damage as a result of the incident.

Driver stepped into cement dust that overflowed his work boots.

Employee was walking down a flight of steps on the PreHeater Tower at Lafarge Cement Harleyville, SC and missed a step. He fell down approximately 5 steps and suffered compound fracture to his leg.

Temp. worker was performing cleaning task when he indicated his back was sore but could not identify a specific incident other than repetitive twisting motion.

Employee was helping putting a scaffold together when an X brace came off and hit the employee in the mouth chipping front teeth.

A mechanic was rigging a bearing cap with 2 come alongs, when side loading caused the load to fall from support, striking the employee on the foot. Xrays of the foot revealed no breaks - just swelling.

While lifting the bed of a gator to check the oil an employee felt a pain in his stomach.

There was a minor fire started when a piece of heated metal was discharged from a belt magnet into a plastic collection bin. The piece of metal was likely coming from a plastics shredder, where it was likely caught and discharged - then collected by the belt magnet. The damage was minor and corrected in the same day. No one was injured during or as a result of the incident.

An Industrial Designer, employed by Tegron Industrial Automation, apparently opened an MCC disconnect and alleges to have received an arc flash burn while measuring for a control transformer. He apparently sealed the enclosure back and left the plant without notifying anyone of his injury. Tegron contacted the plant on Monday, December 7, 2009 and reported the injury.

Employee was shoveling material when a jackhammer, which was propped near by,fell and struck him on the foot.

Employee was found unresponsive in office trailer. No work related incident occurred. Coroner's report stated employee had heart attack.

Employee was walking down a flight of stairs when he lost his footing and fell coming to rest on the landing below. He fell approximately 10 feet down the stairs.

Oil leaked out of the HGG due to pooled oil in HGG combustion chamber caused by pumping oil into the HGG, the oil not burning and extended pumping time. **Excessive oil had pooled at the Hot Gas Generator, while at the same time the pilot failed to ignite.**

Employee was using cutting torch and molten metal ran down sleeve, into glove.

Employee was inspecting 8t carrier deck. Lifted engine cover but failed to use cover rod to hold engine cover up. Engine cover slipped out of his hand and fell on his left hand. Fractured 3 fingers.

EE sustained chipped tooth resulting from being struck by ratchet handle he was using to apply force to loosen nut on bolt.

Replacing sheet of steel liner inside the VRM using a come-along and prybar. The sheet shifted pinching mechanic's hand between steel frame and door frame.

Employee was using pry bar and felt sudden pain in lower back

Employee was using a drill on the ID Stack railing for modifications to be made. When drilling, the drill caught in the hole and twisted. The employee reported the incident as a first aid, but requested medical attention on 10/21/08. X-rays revealed a fracture to his right hand

Employee was loosening a fitting on a 2" pipe and noticed a pain in his lower back. Employee contacted medical provider by phone and was given home care instructions. The employee called again on 7/5/08 and still had tightness in his lower back. He was seen by a physician on 7/7/08 and prescribed medication for the muscle strain.

Employee was using a razor knife to cut a belt in order to put in a new splice. While using a framing square to guide the knife, the knife slipped and lacerated the employee's thumb.

There was a hot spot in the precal coal fines bin that was stirred up. The stirring conditions were created when attempts were made to withdraw the hot material to the HGG and when the Pre-Cal Coal Mill DC started. The start of the DC introduced new aerated coal into the bin. At this point all conditions were met to form the unplanned ignition.

Employee performing new task, utilizing the wrong tool for the job.

Employee was checking an air slide on a silo to make sure it was not plugged up. While doing so he notice the pin had backed out of the cylinder head that operates the gate that feeds the air slide. When he tried to replace the pin back in the head of the cyclinder it went closed & wedged his finger between the cyclinder head & a sharp metal edge.

Employee tripped exiting a bobcat. His foot became caught between the bobcat's bucket and frame, resulting in a sprained ankle.

Technician was straightening fender on d-11 using 20 lb sledge hammer. As technicim was making a downward blow w/hammer. the head struck the suppressiom system on the d-11 caught the handle to come toward the technicion. The end of the handle struck the left inner thigh area between the groin and knee cap, resulting in severe contusion.

Employee tripped on plastic banding, while unloading a box, and fell on floor (same level).

Employee was cutting banding when knife slipped and struck hand.

Employee strained back while removing dust collector bags.

Preliminary investigation indicates a mechanical failure caused material to come in contact with the precalciner housing, igniting the fire. It appears the fire suppression system was triggered but failed to extinguish the fire. A complete investigation report when complete will be forwarded to the local MSHA field office.

Employee's fingers caught in closing hinged door.

Employee was adjusting motion switch on #1 cooler drive when he looked up and leaking clinker dust blew behind his safety glasses (prescription with side shields) and in eye. Was seen by medical doctor and was prescribed antibiotic eye ointment.

Employee was closing clinker gate and smashed finger against frame.

Employee complained of chest pains near the end of his shift and was instructed to rest for a few minutes. After a short rest employee returned to normal activities. Approximately 10 minutes later employee again complained of chest pains and being light headed. Employee was sent for medical attention.

Employee slipped in grinding aid area and rolled right ankle.

Employee was working in building under construction when contractor's aerial work platform had to be moved due to engine malfunction. Engine was blown which emitted heavy exhaust smoke in work area. All employees evacuated building until fumes were gone. Employee was only one exposed that complained of migraine symptoms. He also had a sinus infection at time of exposure.

Preliminary investigations indicate that residual heat from the kiln caused plastic residue to catch fire in the tower. The system possibly went positive forcing the flames into the CDF screw and weigh feeder. A complete investigation report when complete will be forwarded to the local MSHA field office.

Got hot clinker dust in right eye causing corneal burns and abrasions. Eye was cleaned and patched for 24 hours. Company was unaware that employee was given prescription drops until 1/19/06.

Worker reported back pain while lifting a bag of mortar.

The employee was lifting a steel plate off the ground. The employee felt a minor burning in his back. He report the incident to his supervisor but did not get medical treatment until late January 2005. The employee recieved physical therapy treatment the last week of February 2005.

WHILE WALKING ON GROUND LEVEL CONCRETE FLOOR IN PACKHOUSE, EMPLOYEE INTENDED TO STEP-UP TO NEXT LEVEL. EE MISJUDGED HIS STEP HEIGHT & STUMBLED FORWARD, STRIKING HIS LT. WRIST AGAINST A SHARP EDGE OF METAL, THUS CAUSING A MINOR CUT TO HIS LT. WRIST.

Employee was accessing front end loader and reported a "pop" in his knee. Employee was sent to doctor and returned to work the same day withouth restriction. Employee's knee did not seem to heal and in December an MRI was conducted identifying a minor tear requiring surgery to repair.

Employee stepped across a hose before climbing up a ladder. When he came down he stepped on the hose, became unbalanced and he came down with all his weight on the side of his right foot causing him to twist his ankle.

Removing and putting on access ports on preheater tower causing lumbar strain. DID NOT BEGIN LIGHT DUTY OR PHYSICAL THERAPY UNTIL 07/23/04 THUS THE REASON FOR REPORTING IT NOW.

Employee had completed cleaning the preheater. As he removed his face shield, dust from his hard hat fell into his left eye. Employee was wearing safety glasses with side shields; however, material still got in eye.

EE was cleaning preheater tower when hot material got between his gloves and shirt sleeve, burning each of his wrists.

On 02/19/04, ee reported to supervisor that he had backed the dozer into a small depression causing the dozer to jerk. The ee began to feel pain & numbness in his shoulder & arm. He did not feel he needed medical attention. On 02/26//04 he did go to the company doctor who suggested therepy. This did not help & the ee had surgery on 05/20/04, making it a lost time accident.

The employee drove a D8 dozer off a quarry bench. At the time no injuries where reported except a buised leg and sore shoulder. The employee was examined at a local hospital with no injuries found. The employee's shoulder continued to be sore. The employee was sent for an MRI discovering a slight tear and bone spurs that will not allow healing to take place.

EE WAS CUTTING A PIN OFF F33 BELT. SOMEONE TRIPPED OVER THE GAS LINE WHICH PULLED THE TORCH THAT WAS IN HIS RIGHT HAND DOWN ON HIS LEFT WRIST.

EE WAS CROSSING PALLETIZER LIFT TABLE TO REMOVE BAGS THAT WERE STOPPING PRODUCTION. HE MADE A MISSTEP, CAUSING HIM TO LOSE BALANCE & FALL FORWARD OFF PALLETIZER TABLE. REPORTED-6/12; TO ER 6/ 15; TO DOCTOR 6/17.

HIGH TEMPS SHUTE GENERATOR FOR A PORTABLE CRUSHER DOWN. EE WANTED TO CHECK WATER LEVEL IN RADIATOR. HE RELEASED THE CAP SLIGHTLY TO BLEED OFF STEAM AND THEN REMOVED THE CAP. HOT WATER SPRAYED OVER THE EE'S RIGHT SIDE.

EMPLOYEE WAS WELDING IN THE CLINKER COOLER DUST COLLECTOR. THE DUST COLLECTOR "DUMPED" CAUSING MATERIAL TO FALL OFF THE DUST COLLECTOR BAGS. IN TRYING TO GET OUT, EE STATES HE THREW OFF HIS WE LDING GOOGLES (CAUSING MATERIAL TO GET INTO EYES) AND TRIPPED OVER THE TORCH BURNING.

EMPLOYEE BENT OVER TO PICK UP AN OIL ABSORBENT RAG AND STATES THAT WHEN HE TRIED TO STAND UP, WENT TO HIS KNEES WITH BACK PAIN. NOT REPORTED UNTIL 11-15-01.

PRIED DUST COLLECTOR DOOR OPEN WITH CHANNEL LOCKS TO BREAK THE SEAL, PULLED DOOR OPEN & THEN PULLED BAG OUT. WHEN TRIED TO STAND, FELT PAIN IN BACK. DID NOT REQUEST TO GO TO DOCTOR UNTIL 8/10.

EE WAS HITTING A PIN ON THE STAMLER CHAIN WITH A SLEDGE HAMMER. ANOTHER EE LEANED DOWN. WE THINK HE DID THIS TO HOLD THE CHAIN. WHEN EE HIT THE PIN, HAMMER BOUNCED OFF THE PIN HITTING THE OTHE R EE IN THE FACE.

PUSHING RODS IN TO CLOSE SILO. LAST ONE WENT IN EASIER THAN THE REST AND EE'S HAND HIT SILO CUTTING FINGER (3 STITCHES).

EE WAS IN CLINKER COOLER DUST COLLECTOR TO REMOVER SCREW. ONE EE ASKED THE CONSOLE TO CUT BACK ON F26 FAN. WHEN THEY DID THE DUST IMMEDIATLY BEGAN TO SETTLE GETTING INTO EE EYES.

HE REMOVED PLUD ON KILN FEED PUM & INSERTED PIPETO LOOSEN MATERIAL. WHEN HE REMOVED THE PIPE THEKILN FEED SHOT OUT & INTO HIS EYES.
