In occurrence while EE was assisting an EE, this EE stepped backwards onto EE's left foot causing a closed displacement fracture of the third metatarsal bone.

On November 20, 2025, at about 3:00 p.m., the Maintenance Clerk was repositioning a 24x20-inch picture frame when it broke. Falling glass struck the employee's left kneecap, causing a one-inch cut. The incident was reported immediately. The employee received four stitches; no restrictions were issued.

Subjective c/o generalized back pain after transferring 1/2-full 5-gal buckets ~3-4' from 3'H ledge to coworker; no lifting, stooping, or twisting required. Mandatory weekend shift; EE wanted to leave early, complaint is suspect. EE refused post-incident drug screen, admitted THC/marijuana use. Terminated for cause per zero-tolerance drug/alcohol policy.

EE was in the process of ascending on to a closed cab skid steer. As they attempted to open the door of the cab of the skid steer, the EE lost footing, pivoted to the side as they held on and then fell on to the ground resulting in a laceration to the upper part of the EE's head.

Employee was shoveling material at the secondary screen when they felt pain on left forearm and noticed swelling. The initial injury occurred on 06/20/2025 but EE declined medical treatment. On 06/23/2025 asked to see a doctor and was taken to a minor emergency clinic for treatment. Diagnosed with strain of muscle left forearm and place on restrictions.

Employee opened a toolbox drawer when it fell down onto their right thumb and tool box door causing a fracture to their right thumb

Employee was standing next to the hood doors on K2 Burner Floor when a 6" metal braided hose that was being suspended broke loose causing it to swing striking the employee in the face.

EE felt a sharp pain near EE's sciatic nerve when EE pushed a plate.

Injured Employee burnt EE's hands while putting a cover in an open pipe in K1 Preheat Tower.

Maintenance Mechanic was replacing an elevator bucket, when handling the bucket, the bucket shifted causing a laceration on EE's left ring finger.

EMPLOYEE WAS CLEANING AROUND THE PRIMARY CRUSHER WHEN THEY SPRAINED THEIR ANKLE ON AN UNEVEN SURFACE.

Employee was trying to unplug a pug mill. EE opened an inspection port and noted materials in the port and began to tap of the port when dry materials came out getting into EE's eyes behind EE's safety glasses. Employees eyes were flushed out but were still irritated and EE was seen a physician and prescribed medications.

A mechanic was using a piece of pipe to prop open a dust lid on an air conveyor so the mechanic could access the belt to do cleaning. The mechanic had just finished cleaning the section of conveyor when the support pipe came loose and struck the employee on the left side of face just below the jaw. This resulted in the employee needing stitches.

Employee was removing rotor from Clay Crusher, rotor shifted and caused right thumb laceration and fracture.

Troubleshooting Silo 12 gate and hit EE's head on the lower part of the guard, causing a laceration.

The EE placed right hand inside a pin hole to remove grease before installing a hammer attachment on an excavator. The hammer attachment slipped pinching fingers.

Employee and witness were rolling a section of conveyor belt off a roll that is suspended on a set of I-beams with a 3" shaft. After cutting off the section needed they rolled the belt back up. The shaft fell off of the I-beam and struck employee on the shoulder, knocking them to the ground.

A maintenance employee reported to EE's supervisor that while lifting a bucket of tools out of a basket with EE's right arm EE heard and felt a pop. On the 11th the employee requested to be seen by a physician where EE was diagnosed with a strain to the shoulder

Our laborer was shoveling the walkway next to the conveyor belt in our secondary concrete tunnel. While EE was shoveling a fan attached to the tunnel ceiling which was over the conveyor belt came loose and swung down and contacted the laborer on the right side of the face. EE had a bruised cheek and a scratch. The fan was guarded 100% from moving parts.

A plant mechanic was changing out bags in the kiln bag house when ppe failed and allowed kiln dust which is slightly alkali, to make contact with a hand. The employee was wearing a Tyvek suit with gloves taped at the wrist but the tape failed allowing the material to break through without them realizing this. This resulted a burn requiring medical treatment.

A supervisory employee was retrieving a heavy bearing from the warehouse, EE leaned over and under the warehouse shelving to retrieve the bearing which weighed approximately 40lbs. At the time EE noticed a pull to EE's groin, EE was later diagnosed with a hernia

EE was performing maintenance at Wash Plant on C8 conveyor and wash screen 4 replacing a roller assembly. The assembly hit the handrail and caused it to rotate and pinching the EE's tip of ring finger between the roller assembly and tail pulley frame, causing a laceration to the finger.

EE picked up a chipping gun from a rack and felt pain in lower back. On 08/02/2021 EE was seen by a Dr. and given restrictions for a strained lower back.

A plant mechanic was assisting 3 other mechanics to splice a conveyor belt when the guide cable failed & struck the employee's left middle finger causing a minor laceration at the top of the nail bed. The following day the employee was seen by a physician for observation & was diagnosed with a mid phalangeal fracture to left middle fingertip, a finger splint was applied.

While working on the K2 PHT, the employee failed to close their PAPR shield all the way. Material and hot water was discharged from an 8 inch inspection door causing burns to the face and eyes

Miner was sent to clean material from raw mill weigh feeders, there was a calibration chain buried under material the miner tripped over and landed on hands but mainly on left pinky finger.

As a production loader operator was exiting the loader using the hand railings. EE's hand got caught between the ladder railing and the mudflap which caused a pull to EE's shoulder leading to torn tendons.

An employee was closing an inspection door on the K1 pre heater tower. While closing the door the employee's right middle finger was pinched between the latch of the door and the handle. The employee had 2 pairs of gloves on, but still sustained a laceration to EE's finger which required sutures to close the wound.

Miner was walking between work areas when a piece of dust got into EE's eye behind the safety glasses. The miner flushed the eye and removed the dust. About an hour later there was still discomfort. Miner was sent to the clinic where a 2 mm abrasion was found on the eye.

Employee injured wrist while using a shovel to clean up spillage at the secondary crusher plant

Employee was attempting to close the door on rotor feeder of the Raw Mill #1. The employee started to lift up on the door, when the door stuck and the employee felt a pain in left shoulder. The employee was prescribed physical therapy on 7/16.

3 employees and 1 contractor were working on the freshwater pump. They uncoupled the pump from the pipe to turn the valve around when the barge started to sink on one side. The employees/contractor climbed off the barge and swam to shore. The injured employee cut L middle finger while climbing off, requiring 2 stitches.

Production mechanic was changing out the sleds under the impact zone of conveyor. Mechanic flipped the sled frame over with assistance from contract mechanic to finish installation. During this process mechanic felt a pull and then noticed a knot around EE's groin area.

Employee was moving I-beams when EE felt a pop in EE's right shoulder.

Electrician was changing out motor for a magnet belt during the day. EE went home and felt swelling in right knee. EE found a small gauge 1/2 inch wire in the knee. After going to the med clinic EE was found to have septic arthritis of the right knee.

Employee was removing the hood of their 40 cal suit used to rack out 4160 switch gear when they felt a foreign particle drop in their eye. After examination by local clinic they were found to have a 2mm X 1mm abrasion above the right cornea. No particle was found, and no time was lost from work.

Employee was down in the clinker transport drag conveyor area and clinker material got under the employees goggles and into eye

EE was pulling/dragging a heavy blower down a ramp, it got caught up, EE continued to pull until the blower came loose and hit their knee.

Employee working under a conveyor when the electrician's pulling cable and knocked a rock off the cable tray and struck employee's hard hat. Rock punctured the hard hat and cut the back of employee's head. Employee received 4 staples in head.

Employee called and stated they were working at cemex quarry pulling product (caps) from magazine and felt a sharp pain in back.

Employee was cleaning out a conveyor chute when EE reached down onto the flow of material to turn on a water valve and was struck on the right pinky finger by a 5 - 10" rock.

EE was air lancing ports on preheat tower, EE said EE turned all the air off and bled the lines. When port door was opened air canon went off blowing the smock portion of PAPPER unit up which caused hot dust to burn through material at the neck and to go into the face shield burning EE on the left side of face, neck, and ear

Employee was closing the door to the loader at the end of shift, had lunch box and bags in hand, turned around to shut the door and caught tip of left middle finger and artificial nail in the door.

A maintenance mechanic was using rigging to move raw mill shell liners. The employees right pinky finger was pinched between the shackle and magnet that was being used to move the liners. This resulted in a fracture on the tip of the pinky finger.

Employee pulled on a spreader bar in the back of employee's truck to roll it off of a bucket and felt a sharp pain in employee's left middle finger. Following that, a soreness persisted followed by numbness in the fingers/palm.

Employee was helping repair flights on the sand screw. Employee was turning the screw using the V-belts so that the flight could be welded. Employee said that they felt a pull in their shoulder and reported it to the supervisor at the end of shift. Weather was clear and sunny. Employee was in front of screw pulling down on v-belts. Machine was locked out and tested.

Employee was on the forklift transferring a cooling pipe from the front of plant to the back side where the other crew members were waiting for it. Pipe began to roll off the forklifts hooks, employee then proceeded to get off the forklift and tried to stop the pipe from rolling completely off. The pipe fell and landed on right foot.

While lugging on a tire, tech was using crane hands to line up tire with bolt holes. Guiding tire into place ee applied down pressure on tire. The tire slipped in the loose gravel causing the boom to surge forward hitting the tech in back, causing ee to lose breath and pain in the lower back.

Employee was trouble shooting an issue with the kiln bag house. The employee bent over at the waist and reached around the main air damper to turn an air valve, when the employee felt a sharp pain in the lower back area. Employee was placed on restrictive duty and prescribed physical therapy.
