At approximately 10:50 PM, two employees were cleaning beneath the screw conveyor when one employee came into contact with the other employee's tool, resulting in an injury to the affected employee's right eye.

A maintenance employee slipped on the 3rd step from the bottom landing rolling their left ankle.

Employee asked another employee to release bucket on skid steer. employee raised bucket tilting it down. employee stood in front of bucket (in the line of fire) released the pins and the bucket slid off striking employee in their left foot. EE sustained fractures to 4 bones in their foot.

In occurrence while EE was assisting an EE, this EE stepped backwards onto EE's left foot causing a closed displacement fracture of the third metatarsal bone.

On November 20, 2025, at about 3:00 p.m., the Maintenance Clerk was repositioning a 24x20-inch picture frame when it broke. Falling glass struck the employee's left kneecap, causing a one-inch cut. The incident was reported immediately. The employee received four stitches; no restrictions were issued.

Subjective c/o generalized back pain after transferring 1/2-full 5-gal buckets ~3-4' from 3'H ledge to coworker; no lifting, stooping, or twisting required. Mandatory weekend shift; EE wanted to leave early, complaint is suspect. EE refused post-incident drug screen, admitted THC/marijuana use. Terminated for cause per zero-tolerance drug/alcohol policy.

While cutting belt at tail pulley the blade slipped out and hit employees left arm. Left arm was positioned in a bad area when this happened. The cut was on EE's forearm 3 inches past base of thumb. Cut got fatty tissue and some mussel.

On 10/03/25 at 8:35 am, the coal mill operation began. At 9:26 am, the control operator noticed a fire on the coal mill line. Employees secured the area and tried to extinguish it. When it persisted, the Hygiene Fire Dept. was called at 9:44 am and arrived at 9:56 am. MSHA was notified the same day and visited on 10/06/25 to review the area and follow up on the investigation.

EE was in the process of ascending on to a closed cab skid steer. As they attempted to open the door of the cab of the skid steer, the EE lost footing, pivoted to the side as they held on and then fell on to the ground resulting in a laceration to the upper part of the EE's head.

Electrician modifying a threaded nipple with portable bandsaw for conduit installation to transformer and lacerated EE's right middle fingertip.

A plant maintenance employee was splicing a conveyor belt when their foot slipped out from under them as they were dismounting off of the frame and strained their right knee.

The coal man elevator stopped between floors with one person in the elevator car. The shift supervisor and maintenance personnel tried to reset the elevator but were not successful. Thyssen Krupp Elevator company was called out to safely lower the elevator. The employee was in the elevator for longer than 30 minutes and the event was called in. No injury occurred

Employee was shoveling material at the secondary screen when they felt pain on left forearm and noticed swelling. The initial injury occurred on 06/20/2025 but EE declined medical treatment. On 06/23/2025 asked to see a doctor and was taken to a minor emergency clinic for treatment. Diagnosed with strain of muscle left forearm and place on restrictions.

The plant tower elevator stopped between floors due to a defective safety switch which failed (as designed) causing the elevator to stop. An employee was in the elevator and notified the control room that EE was not able to get the elevator to move. The employee was in the elevator for more than 30 minutes and the event was called in to MSHA. No injury associated with the event.

Employee was removing a conveyer belt when the right outrigger on the forklift they were using shifted downward causing the forks to hit an electrical box where they had their hand. The impact caused a fracture / laceration to their left thumb.

Employee stepped on a concrete floor scraper causing the wooden handle striking their thumb on 4/4/2025, medical treatment was sought later on 4/15/2025 and they were diagnosed with a strain to the thumb and was placed on restrictions and physical therapy.

Employee opened a toolbox drawer when it fell down onto their right thumb and tool box door causing a fracture to their right thumb

EE was cleaning under a conveyor belt when EE stood on the bucket of the skid steer EE was operating. EE began to cut part of the belt and material fell on EE causing EE to lose EE's balance and fell aprox. 24 inches on EE's left side injuring EE's shoulder, left hip and rib.

Employee was standing next to the hood doors on K2 Burner Floor when a 6" metal braided hose that was being suspended broke loose causing it to swing striking the employee in the face.

Two employees were attempting to light the Kiln burner when it flashed blowing hot gas and dust onto the employees. One employee received first and second degree burns to the left of their face and neck, the second employee received only a minor burn.

Employee smashed and lacerated right hand ring finger attempting to unplug feed chute.

Electrician was removing spillage buildup on a decommissioned (de-energized) cable tray which had collapsed. While working to pry and break spillage buildup, the material released causing EE to hit the side of their hand on metal box adjacent to cable tray smashing the right small finger (pinky) between the box and the tool handle.

Employee smashed EE's left index finger with a hammer

An operator was handing an impact wrench over to another operator that still had pneumatic pressure. When the receiving operator grab the socket end to take possession of the impact the operator handing it over accidently engaged the trigger.

EE felt a sharp pain near EE's sciatic nerve when EE pushed a plate.

We had a fire on a storage building that is used to charge electric golf carts for employees. We believe a charging station malfunctioned and got hot causing a fire on one of the golf carts that then spread to the surround carts and area. No injuries. Believed by the Fire Department who responded an MSHA Inspector to be an electrical fire.

EE visited EE's personal doctor due to feel strength loss in EE's grip on both hands. EE was told EE had carpal tunnel in both hands caused by repetitive motion. EE has worked for CEMEX USA as a Production Assistant Laborer for the last 16 years.

Operator of a Caterpillar D6 Dozer was repositioning on a narrow strip and was looking behind them while travelling forward and ran the left track over a berm causing the dozer to fall off of a 15 foot tall highwall. The operator struck their head on something in the cab and was taken to Urgent Care. The physician asked that they be off work for two days.

Employee slipped coming down the ladder of a 980H. Employee landed on EE's rear end and EE's left elbow. Employee has been placed on light duty.

Injured Employee burnt EE's hands while putting a cover in an open pipe in K1 Preheat Tower.

The Homogeneous elevator stopped between floors with one employee in the elevator car. Maintenance personnel on shift tried to reset the elevator but were not successful. Thyssenkrupp Elevator was called out to lower the car to allow the employee to exit the elevator. The employee was in the elevator for longer than 30 minutes and the event was called in. No injuries occurred.

Employee stepped across a drainage area, slipped on some mud and fell backwards injuring their right elbow

The employee stepped off a walkway approximately one foot in height. EE stepped into some mud and material with their right foot that had collected in a drainage area. EE's foot slipped out from under EE, straining their left knee.

Employee was cutting the old rubber endcaps off of the spray bars at the Wash Plant. While cutting one of them, the retractable-blade utility knife they were using slipped off the rubber and contacted their left inner thigh. This caused a laceration about 1 inch long and 1 inch deep. 3 Sutures required.

Employee was cleaning at the Water reclaim Plant (Sand Plant) and went to retrieve a water hose and slipped on wet slippery surface and twisted left knee.

A layer of concrete dust and debris had collected on silo roof obscuring hatches lids, and other obstructions. Walking on roof tripped on one of the obstructions. Fell to right knee and struck something in the dust and twisted some on the way down. Local clinic did x-rays but nothing broken. Received brace for sprain to unspecified site of right knee.

Employee was operating a Sky Trak forklift and rolled backwards. The left rear wheel went up on a berm causing the forklift to lay on its side.

Injured employee used a bar hook with a T handle to snare the cable of an air hoist to pull it towards themself to retrieve the ball. The employee's left index finger was caught in a pinch point between the bar hook T handle and the rosette of a vertical scaffold leg. This resulted in a pinch laceration requiring stitches

Contractor miner was tasked with putting the guards that go over a feed belt back on. After leaning over to tighten the guard EE relaxed their weight on the handrail and the horizontal arm on the handrail gave. The contractor then lost their balance and their torso fell on toe plate. The toe plate was 45 degrees outward when the accident occurred

EE was working inside of the K1 Mill Welding out bolts, while working in the lower section of the table a hammer fell through a 3x2 foot opening from approximately 8ft and struck EE's rear right shoulder. When the accident occurred plywood was covering the opening but the hammer slipped through a small section of the opening.

A fire broke out on the K1 burner floor coal system when a spark ignited a wooden shed that had accumulated coal.

Tires in the tire station of K1 Preheat Tower caught on fire.

Maintenance Mechanic was replacing an elevator bucket, when handling the bucket, the bucket shifted causing a laceration on EE's left ring finger.

Ladder not secured; working in awkward position

Employee was working on a cone liner change at our secondary plant. They were hitting the bowl liner with a sledgehammer to dislodge the liner when a sliver of metal broke off the liner and struck them in the right calf area. Wound bled and scabbed over. No movement difficulties or pain reported by employee.

Production Assistant employee was called to reset a disconnect in Electrical Room 5 when the first attempt failed to reset the panel. The employee then proceeded to pull the handle, halfway through the motion the panel blew up causing an arc flash striking and electrocuting the employee on EE's left hand.

employee stepped up on oil containment wall. EE's boot caught the top of wall causing employee to trip and fall. Employee extended EE's right arm to catch self pulling something in EE's right bicep.

The coal elevator stopped just after lift off from the first floor with two employees in the elevator car. The on shift supervisor and maintenance personnel tried to reset the elevator but were not successful. Elevator company was called out to lower the elevator. The employees were in the elevator for longer than 30 minutes and the event was called in. No injuries occurred.

EMPLOYEE WAS CLEANING AROUND THE PRIMARY CRUSHER WHEN THEY SPRAINED THEIR ANKLE ON AN UNEVEN SURFACE.
