While cutting belt at tail pulley the blade slipped out and hit employees left arm. Left arm was positioned in a bad area when this happened. The cut was on EE's forearm 3 inches past base of thumb. Cut got fatty tissue and some mussel.

Electrician modifying a threaded nipple with portable bandsaw for conduit installation to transformer and lacerated EE's right middle fingertip.

Employee was removing a conveyer belt when the right outrigger on the forklift they were using shifted downward causing the forks to hit an electrical box where they had their hand. The impact caused a fracture / laceration to their left thumb.

Employee stepped on a concrete floor scraper causing the wooden handle striking their thumb on 4/4/2025, medical treatment was sought later on 4/15/2025 and they were diagnosed with a strain to the thumb and was placed on restrictions and physical therapy.

Employee opened a toolbox drawer when it fell down onto their right thumb and tool box door causing a fracture to their right thumb

EE was cleaning under a conveyor belt when EE stood on the bucket of the skid steer EE was operating. EE began to cut part of the belt and material fell on EE causing EE to lose EE's balance and fell aprox. 24 inches on EE's left side injuring EE's shoulder, left hip and rib.

Electrician was removing spillage buildup on a decommissioned (de-energized) cable tray which had collapsed. While working to pry and break spillage buildup, the material released causing EE to hit the side of their hand on metal box adjacent to cable tray smashing the right small finger (pinky) between the box and the tool handle.

Employee was cutting the old rubber endcaps off of the spray bars at the Wash Plant. While cutting one of them, the retractable-blade utility knife they were using slipped off the rubber and contacted their left inner thigh. This caused a laceration about 1 inch long and 1 inch deep. 3 Sutures required.

Employee was cleaning at the Water reclaim Plant (Sand Plant) and went to retrieve a water hose and slipped on wet slippery surface and twisted left knee.

Ladder not secured; working in awkward position

EMPLOYEE WAS CLEANING AROUND THE PRIMARY CRUSHER WHEN THEY SPRAINED THEIR ANKLE ON AN UNEVEN SURFACE.

Employee arrived to work, had parked car and was getting out when some debris in the wind entered their eye. They flushed eye but it didn't get better so they then notified management.

While working on a tire, the tire manipulator slipped catching the technicians thumb between the manipulator and the driver key the technician was holding in place by hand.

The EE placed right hand inside a pin hole to remove grease before installing a hammer attachment on an excavator. The hammer attachment slipped pinching fingers.

EE was in the process of driving T post when right thumb struck the top of the post while holding the post driver handle. The EE was wearing cut level 4 gloves at the time of the incident.

EE was going up in the man-lift basket and approaching the railing of the screen 3 and 4 deck. EE had EE's left hand gripping the handrail of the man basket, EE's middle two fingers were curled over the rail of the basket. EE leaned forward and pinched EE's two fingers between handrail of basket and handrail of wash plant.

Employee and witness were rolling a section of conveyor belt off a roll that is suspended on a set of I-beams with a 3" shaft. After cutting off the section needed they rolled the belt back up. The shaft fell off of the I-beam and struck employee on the shoulder, knocking them to the ground.

EE was removing barricade on a slight incline when EE slipped backwards. As EE was falling, EE placed hand behind to catch self. In doing this EE injured their finger on rocks in the area.

EE was performing maintenance at Wash Plant on C8 conveyor and wash screen 4 replacing a roller assembly. The assembly hit the handrail and caused it to rotate and pinching the EE's tip of ring finger between the roller assembly and tail pulley frame, causing a laceration to the finger.

The mounting plate for the Secondary Crusher (deflector plate) had built up material behind it creating a force on the plate. When the end of the plate was cut loose from the crusher housing, the plate rapidly released this tension like a spring and swung out impacting the mechanic. The mechanic suffered 2 lacerations on the face and as EE fell back landed on the tail bone.

EE had a tool in a portable vise on the back of a utility truck. Upon pressing with both hands on the lever to release the pressure of the vise it suddenly broke loose and EE put force on the inside of EE's right ring finger causing a fracture.

Employee called and stated they were working at cemex quarry pulling product (caps) from magazine and felt a sharp pain in back.

Employee was cleaning out a conveyor chute when EE reached down onto the flow of material to turn on a water valve and was struck on the right pinky finger by a 5 - 10" rock.

EE works the evening shift and the injury occurred around 4am. The employee was shoveling material out from under a conveyor belt when ee hit a hard spot in the material and felt pain in elbow. The employee was taken to minor emergency. EE was released back to work with non-prescription medication and restrictions of motion to the right arm.

While lugging on a tire, tech was using crane hands to line up tire with bolt holes. Guiding tire into place ee applied down pressure on tire. The tire slipped in the loose gravel causing the boom to surge forward hitting the tech in back, causing ee to lose breath and pain in the lower back.

Employee was moving the chocks on a radial stacker and was bumped by another employee who was operating the skid steer. Employee thought EE was OK but EE went to the doctor the next day and had a hairline fracture to one rib.

Employee was changing out hammers on the crusher. He put one of the old hammers on the ground and left it standing up. it fell over and hit his foot behind his steel toe on top of his foot.

IW was standing on ladder installing a part on a 777 truck bed using truck hoist. IW thought brackets were engaged (not visible while mounting part) so released tension from hoist. Using a hammer to knock part down better bracket came loose and fell knocking the ladder out from IW. IW fell to ground catching self with right arm causing injury to right arm and shoulder

The employee was shoveling on the 10th, felt pain at end of shift, Employee worked until the 28th. At that time he came to the supervisor saying it had been hurting since the 10th and that he thought he could work through it. The employee was diagnosed with shoulder strain.

The employee stated that after climbing down the ladder of the water truck, when he went to turn to go to the back of the truck his leg planted when he twisted and he went to the ground hearing a pop in his knee. He refused to go to the doctor that day and on 12/1/2014 he asked for medical treatment and was diagnosed with a Sprain on his right knee with work restrictions.

ELECTRICIAN SUFFERED FLASH BURNS TO THE NECK AND HAND WHEN HE WAS INSTALLING AN ELECTRICAL BOX THAT CAME IN CONTACT WITH AN ENERGIZED BUSS BAR.

The water truck door was opened the EE was reaching in to get his gloves when the door shut pinching his right ring finger.

Contractor Vehicle eletrical fire

EE was lifting at work and strained low back. He experience pain and was taken to Texas Medical Clinic. He was given prescriptions and light duty. EE returned a week later for follow-up and was feeling much better and able to touch toes.

Dizzy feeling, heart racing, feels weak, feels worse after eating, dehydration from working in extreme heat. Effects all over body. No equipment, nothing directly/indirectly effected this employee.

Driller said the wind blew door of drill shut on his hand. Crushed the end of his right index finger.

LOADER STRUCK RAILCAR WHILE CLEANING MATERIAL AROUND TRACK.

STRUCK IN HEAD BY FALLING ROCK.

SPRAYING OFF CATWALK, NEEDED TO SHUT WATER OFF, PLACED HOSE DOWN WENT TO THE SHUT OFF THE VALVE QUICKLY AND TWISTED RIGHT ANKLE.

EE WAS SETTING MOTOR DOWN TO GET READY TO INSTALL THE MOTOR, IT SLIPPED, EE REACHED FOR IT AND RIGHT SHOULDER POPPED.

Driller was checking hole depths. He slipped and fell. Slipped on rocks around hole

TRYING TO SHUT A DOOR TO THE FEEDER USING A HAMMER AND HIT FINGER.

EE was working on the VSI to remove the table and had the crane attached to the table. The table was broken loose with a hyd jack and when pressure was taken off the jack, the table went down on the shaft. As the crane applied pressure, the table jerked off the shaft and rotated catching EE's foot against an anvil in the VSI.

EE was removing a bearing from a piece of machine when it slipped and cut his left thumb.

While bent over and unaware of a piece of metal, the EE walked into it causing a laceration to the face.

EE cleaning chute with a scraper bar when he felt a sharp pain in his right shoulder.

Employee kneeling down welding steel for a junction box, gust of wind blew something in his eye.

EE was shoveling in tunnel, lifted shovel up and turned, felt sharp pain in back.

EMPLOYEE WAS CUTTING THE RIDING BAND. WHEN HE LOOKED UP, LIME DUST FELL IN HIS EYE.

EE was removing a blockage in the #2 chute, hand slipped causing the right ring finger to get lacerated, requiring seven stitches ( SUTURES ONLY).

Employee stated he was climbing up a step ladder when he fell off the ladder resulting in a right knee sprain. Underwent arthroscopy right knee surgery on 1/16/2008.

Employee was being familiarized on a loader, when he stood up employee felt pain in his lower back. As a result employee sustained a lumbar spine sprain. Medical Treatment Only - Physical therapy starting on 8/17/07; 6 sessions, 2 per month for the next three months.

Employee was cutting skirt rubber, blade slipped, cutting left hand between thumb and forefinger, requiring stitches.

Employee fell off of ladder while patching a scurry line hitting left elbow on angle support structure.

While cutting rusty metal with a cutting torch, a piece of slag flew into the employee's left eye.

When employee was pulling come along he caught his left thumb between the guard and shift. This resulted in a hairline fracture to his left thumb.

EMPLOYEE WORKING AT SECONDARY PLANT, PUSHING ROTOR, HIS HAND SLIPPED OFF & WAS CAUGHT BETWEEN THE HAMMER & WALL. EE RECEIVED 13 STITCHES ON HIS RING FINGER ON HIS LEFT HAND.

STANDING, LOOKING UP TRYING TO DISCONNECT COUNTERWEIGHT ON C9 WHEN MATERIAL FELL OFF OF THE CONVEYOR AND INTO EE'S EYE.

EMPLOYEE WAS CLEANING UNDER CONVEYOR BELT FEEDING ALPHA BLENDING PLANT. THE CONVEYOR FEEDING THE PLANT RAN EMPTY WHEN FEED WAS STOPPED, RESULTING IN ONE ROCK TUMBLING BACKWARDS DOWN THE BELT AND STRIKING EMPLOYEE ON THE LOWER BACKSIDE OF THE RIGHT SIDE OF HIS HEAD. REQUIRED FIVE STAPLES.

EE GETTING IN POSITION TO CUT BELT, LANDED ON A ROCK CAUSING HIM TO JERK KNIFE BACK AND AWAY FROM BELT, CATCHING HIS LEFT INDEX FINGER. REQUIRED NINE STITCHES.

EE BECAME DIZZY, LOST HIS BALANCE AND FELL DOWN THE STAIRS IN THE SHOP SUSTAINING A LACERATION TO HIS LEFT KNEE. REQUIRED LIGHT STITCHES.

EE WAS CUTTING CONVEYER BELT, KNIFE SLIPPED LACERATING HIS RIGHT THIGH.

BREAKING OFF FLEXCO BOLTS WITH BREAKER BAR AND HEARD POP IN LEFT ELBOW. ARM STARTED HURTING AFTERWARDS.

EMPLOYEE WAS CLEANING THE CATWALK WHEN A ROCK FELL OFF THE BELT STRIKING HIS LEFT CHEEK.

THE EE WAS MOVING A FEEDER BLOCK WITH A PIPE. HE SLIPPED AND SMASHED HIS LEFT FOURTH FINGER BETWEEN THE PIPE AND THE FEEDER BLOCK. HE SUSTAINED A CONTUSION.

LACERATED RIGHT HAND ACROSS THE PALM AND RIGHT INDEX FINGER. HE WAS MEASURING BEAM ABOVE PULLEY WHILE BELT WAS MOVING WHEN THE TAPE MEASURE GOT CAUGHT IN PULLEY GRABBING HIS HAND AND CUTTING I T TO THE BONE. HE HAS TO HAVE SURGERY TO REPAIR NERVE DAMAGE.

WHILE WORKING IN AN AREA USED FOR WELDING PREPARATION EE FELT AN INSECT STING AND CONTINUED TO WORK THINKING NOTHING MORE OF IT. EE FINISHED HIS SHIFT AND THE NEXT. HE REPORTED THIS TO HIS SUP ERVISOR. HIS SUPERVISOR IMMEDIATELY SENT HIM TO THE DOCTOR. THE RESULTING STING SITE HAS GOTTEN BADLY INFECTED AND NOW REQUIRES FURTHER MEDICAL ATTENTION.

WORKING ON SCREEN, DOOR FELL ON EMPLOYEE, BRUISING NECK.

EE WAS WORKING ON THE DREDGE (WELDING) WHEN AN INSECT CRAWLED INTO HIS BOOT AND BIT HIM ON THE ANKLE. HE DID NOT SUFFER ANY PAIN UNTIL WORKING THE NEXT DAY AND AT 5:20 PM HE ALERTED MYSELF THE QUARRY MANAGER AND I IMMEDIATELY SENT HIM TO THE DOCTOR. HE WAS ABLE TO RETURN ON THE 7TH OF OCTOBER & DID RESUME FULL DUTIES. HE DID NOT RETURN THE NEXT SHIFT AS DR TOLD HIM NOT TO WEAR BOOT

LIFTING SCREEN FROM CATWALK: IN THE PROCESS HIS LEFT HAND CAME INTO CONTACT WITH THE HANDRAIL, SMASHING FINGER WITH SCREEN AGAINST RAIL.

CHANGING SCREEN, PULLING SCREEN FROM SHAKER, HE PULLED A MUSCLE IN HIS BACK.

SPIDER BITE - WORKING ON PRIMARY BELT.

PUTTING GUARD ON C11 CONVEYOR & WAS BITTEN.

JACK SLIPPED & SPRINGS CAUGHT FINGER.

HE PULLED MUSCLE IN LEFT LEG/HIP WHILE SWINGING SLEDGE HAMMER WITH FULL BODY WEIGHT.

LOOSENING COMEALONG, IT GAVE WAY SENDING MOTOR FRAME DOWN TOO QUICK TO RESPOND.

STRAINED ELBOW LIFTING & PULLING SCREENS.

CUTTING GREASE LINE WITH A UTILITY KNIFE, KNIFE SLIPPED AND CUT HIS HAND, REQUIRING FOUR STITCHES.

SHOCK ON FUEL TRUCK SEAT IS BROKEN CAUSING HIS BACK TO BE JARRED.

MAKING A RAMP ON AGGREGATE PILE, WHEN HE PUT THE 926 LOADER IN REVERSE HE HIT A HOLE WITH A TIRE AND IT CAUSED HIS HEAD TO SNAP BACK HE THEN FELT A PAIN IN HIS SHOULDER AND BACK AREA ON RIGHT SIDE.

PROCEEDED TO CLIMB ON EAST OF FEEDER #4, SHOULD HAVE CLIMBED ON NORTH SIDE OF FEEDER INSTEAD WHEN CLIMBING DOWN ON EAST SIDE, SLIPPED AND RIGHT FOOT CUGHT IN HEAD PULLEY.

THE TRUCK DRIVER WAS CLIMBING ONT O THE TRUCK, HE LOST HIS FOOTING CAUSING HIM TO COME DOWN AD HITTING HIMSELF. ON THE FRAME PART OF THE STEPS ON HIS RIB CAGE, ALSO SCRAPE HIS LEG AND ELBOW CR .

THE OPERATOR WAS COMING OUT OF THE CONTROL HOUSE WHEN HE HIT HIS HAND ON THE LIGHT SWITCH THE CORNER HAD A SHARP EDGE ON IT CAUSING THE INJURY. THIS PROBLEM HAS BEEN TAKEN CARE OF.

MAN STANDING ON ROTAR-CLEANING BOULDERS AND MUD FROM BETWEEN THE ROTAR AND CRUSHER BACKWALL. WHEN ROTAR BROKE FREE AND STARTED TO ROTATE, HE STARTED TO STEP OFF ROTAR WHEN HE GOT FOOT CAUGHT.

OPERATOR ERROR. OPERATOR WAS NOT PAYING ATTENTION TO FLAGMAN OR EXISTING TRAFFIC CONDITIONS. WE ARE NOT AWARE OF ANY PERMANENT IMPAIRMENT AS A RESULT OF THIS ACCIDENT.
