EE was entering EE's haul truck when EE heard EE's right knee pop when sitting down into the seat. Miner had an MRI done on 5/19/2026 where a tear in EE's meniscus was found. The miner is scheduled for surgery to repair the knee.

Mostrando los 44When pulling the center bar out, the miners lost their grip on the bar and dropped it on a miners foot just above the steel toe portion. X-rays were done at the time and no fractures were detected and fully released. The miner was still having pain and sent for a follow up on 1/11/25, a x-ray was redone and small fracture found.
EE was operating a skid steer and noticed the attachment wasn't secured. EE stepped out of the machine to secure the attachment and EE's finger came across a metal burr on the attachment cutting EE's finger resulting in stitches.
Employee was opening a steel access door on the Wash Screen Deck when the door was found to be heavier than anticipated and smashed employee's left, index, and middle fingers between the door and another piece of metal.
EE was using a water hose washing off the concrete pad under the Plant. Employee stepped backwards off the concrete pad and felt knee pop. Employee continued to conduct regular activities while being monitored but pain has not went away. Physical Therapy was prescribed on 7/14/2022 making it reportable at this time.
A miner was attempting to unplug a bridged rock out of the the portable jaw crusher when the employees finger got caught between a rock and the metal plate. The miner was wearing gloves but the rock smashed EE's ring finger causing EE to have to receive stitches at the end of EE's finger.
EE was adjusting carriage after team members had issues with busting bags. EE lowered carriage manual adjuster twice, the 3rd time EE did this EE felt a strain in EE's left lower back.
While climbing into the CAT truck, something popped in the back of EE's left calf. EE was unable to straighten leg completely or walk more than a few steps unassisted. EE was seen at an occ health clinic for a muscle strain, who recommended PT, which EE has started.
Performing maintenance on catwalk of a shaker/screen that was broken down. EE walked under a belt & must have struck the shaker. A 4-6in rock somehow shifted & came down, striking EE's hard hat, then bouncing onto EE's glasses & nose. We just got confirmation of a fracture.
Escorting a visitor to a rented portable eagle crusher to verify the serial #. EE was holding the gate open at the top of the stairs, the visitor grabbed the gate for balance. EEs finger was pinched in the equipment. Occ Health referred EE to the ER as EE's nailbed had to be removed.
Using hammer to loosen wedge pins on desander. After removing wedges, leaned down to remove rubber spacers. Sand material fell from above, landing on the right side of face, entering an eye. EE was wearing safety glasses at the time. They tried flushing eye, but they needed to be seen by an eye doctor, who had to use a tool to get the eye cleared of debris.
Changing plates on 8x20 screen, tried to free a piece of metal, took gloves off to get a better grip, metal flew off, striking hand.
Clearing weeds & small trees, pushing them up a hill. When he reached the top of the hill, there was a dozer approaching. He lunged forward and put his arms out, striking the mirror with his LT forearm. No sutures, given RX. Released from care 8/21/15.
Employee inside screen deck, another employee slid screen down to him. While pulling screen towards him, the screen cloth caught employee's right index finger. The sharp edge of screen cloth cut right index finger, requiring sutures.
Employee was putting a guard on the horizontal swingshaft. Bent over to line up the bolt hole and felt a pain in his lower back. Thought to be a minor pain. Completed his shift. Didn't report it until next morning that he was having severe pains in his lower back & could hardly walk and he couldn't come in to work. Was sent to clinic. Diagnosed with Lumbar Sprain.
Employee was changing jaws in crusher. Employees were lifting the jaws out with a crane when the jaw got stuck. Injured employee stepped in to knock it loose. When jaw came loose it hit his right foot. Received a deep contusion on his foot & returned to regular work. Later on 7-22-12, employee's foot became infected and was given Keflex for the infection on 7-23-12. No lost time.

Driver had the tip end of his right index finger cut off. He is off work. The driver is employed by Kokomo Gravel, Inc. Material & Trucking Service
Employee was going to change a screen and was looking for the correct size of screen at the Secondary Wash Plant. The screen was in a stack and 2 of the screens fell and smashed his left hand into an I-beam breaking left index finger & cutting left thumb. Was given a splint and is restricted to "no use" of left hand.
Employee was working on the jaw crusher. Employee lifted a 30-pound take-up bearing and stepped back and had a sudden pain in his middle back causing him to double over. Employee received a strain/possible muscle spasm to his middle back. Employee has lost time and restricted time. He has not returned to full duty at this time.
Employee was in the pit building a water stand. Employee was flipping an I-beam over. When he picked up the beam, something popped in his left index finger. Received a sprain to his left index finger. Employee is restricted and is able to work while restricted.
On 7-6-10 employee was lifting gas cylinders and twisted lower back but did not report it until 7-21 to his supervisor. Employee thought his back was better and no action was taken. On 7-26, employee could not come to work due to back pain. He was sent to the doctor to be checked and treated. He received a lumbar strain. He is off work.
Employee was using a jack hammer to break rock out of the tail pulley at the #2 Conveyor. When the rock broke, the conveyor moved about 3 inches and pinned employee's hand and arm against the conveyor guard. Employee received a contusion/sprain to his right wrist. Employee was restricted.
Employee was spraying underneath the roll crusher. The hose started slipping from his hand. Employee readjusted his weight to get a better stand. As employee went back and then forward, he twisted his right knee causing him to go back and forward again and then falling. Employee received a strain to his right knee. Employee was given a brace and was restricted.
Employee was checking the radiator on the front end loader. Employee took the cap off and the antifreeze coolant sprayed out on his left hand. Employee received 1st & 2nd degree burns on his left hand. Employee is restricted.
Employee was putting a chain over conveyor to lift beam and dirt fell off conveyor into his face and eyes. Employee received a corneal foreign body to his left eye. The foreign body was embedded and had to be drilled out. There are no lost or restricted days.
Employee was cleaning out trays under drums on the crane. Employee was holding drag cable with left hand and reaching under the drum with right hand with small shovel scooping dirt and throwing the dirt behind him and strained his right upper back. He received a Thoracic Strain. He is restricted.
Employee was working on the processing plant. He was guiding a new metal chute into position when his finger got caught between the chute and an angle iron. Employee received a laceration to his left index finger. He received stitches. There is no lost or restricted time.
Employee was bending over in the process of moving a ring (appox. 40 lbs.) on a pump. When employee rose back up, he felt a sharp pain in the middle of his lower back. Employee received a LS Strain. Employee was taken off work.

EMPLOYEE WAS HELPING A CO-WORKER REMOVE A REAR GUARD ON THE SAND STACKER AND FELT HIS RIGHT SHOULDER POP OUT OF SOCKET. EMPLOYEE RECEIVED A DISLOCATED RIGHT SHOULDER WITH SHOULDER STRAIN.
EMPLOYEE WAS REMOVING A RETURN ROLLER FROM THE CONVEYOR. EMPLOYEE HAD HIS RIGHT HAND ON THE END OF THE PINCH BAR AND HIT ROLLER. EMPLOYEE DIDN'T FEEL ANY DISCOMFORT FOR A DAY OR SO. HE THOUGHT THE PAIN WOUDL GO AWAY. HE HAD PAIN IN HIS WRIST WHEN HE WOULD GRASP OR PICK UP AN ITEM. HE REPORTED IT TO HIS SUPERVISOR 11-20-03 AND WENT TO THE CLINIC. EE RECEIVED FRACTURE TO RIGHT WRIST.
EMPLOYEE WAS MOVING THE CUTTING EDGE FROM LOADER TO KEEP FROM BLOCKING THE DRIVEWAY FOR INCOMING TRUCKS. AFTER HE MOVED THE CUTTING EDGE (WEIGHT APPROX. 80 LBS) HE FELT DISCOMFORT IN HIS LOWER BACK. HE WORKED ABOUT TWO HOURS IN LOADER. WHEN HE STEPPED DOWN FROM LOADER, HE HAD SEVER PAIN IN HIS LOWER BACK & COULD NOT STAND UP RIGHT OR WALK WITHOUT A LIMP.
EE WAS WALKING UP LADDER TO ELECTRIC TRAILER IN THE YARD. ON THE LAST STEP EE FELT HIS LEFT KNEE POP & GIVE WAY. EE COULD NOT PUT WEIGHT ON HIS KNEE. HE HAD TO HAVE HELP TO THE CAR.
EE WAS PUTTING PADDLES ON THE LUG WASHER. EE TRIED ONE MORE TIME AND WHEN HE HIT THE PADDLE A PIECE OF METAL GOT INTO EE S RIGHT EYE. HE WORKED THE REST OF THE DAY WENT HOME, EYE GOT WORSE, HA D TO GO TO ER.
AIRLINE FROZE ON THE CRANE. EE TOOK THE AIRLINES APART. ICE CAME OUT OF THE LINES AND HIT EE IN THE FACE AND ARM. EE S FACE WAS CUT AND SCRATCHED. HE WENT HOME, THOUGHT HE WOULD BE D.K. LATER HE GOT SOMETHING IN HIS EYE & WENT TO AN EYE DR HE WAS GIVEN A PRESCRIPTION FOR HIS EYE.
EMPLOYEE WAS SHOVELING UNDER A CONVEYOR AND DUG TOO DEEP. THE SHOVEL TWISTED CAUSING HIS WRIST TO TWIST. EMPLOYEE RECEIVED A STRAIN TO HIS LEFT WRIST. HE WAS RESTRICTED FROM 10/30/01 TO 11/06/ 01.
EE WAS PICKING UP A PIECE OF BELTING AND THROWING THE BELTING ONTO THE PONTOON BOAT. EE FELT SOMETHING PULL IN HIS LOWER BACK. EE RECEIVED A STRAIN TO HIS LOWER BACK HE RETURNED TO WORK ON 9-1 0-01 WITHOUT RESTRICTIONS.
EMPLOYEE WAS CHANGING SCREENS AT THE SECONDARY PLANT. EMPLOYEE TRIED TO TAP ON BOTTOM OF SCREEN WHEN HE CUT HIS RIGHT HAND ON A SHARP WIRE. EMPLOYEE RECEIVED STICHES.
EE WAS REPAIRING CONVEYOR BELT & CAUGHT HIS LEFTRING FINGER BETWEEN TROUGH IDLER AND BELT. HE RECEIVED A CONTUSION & WAS RESTRICTED TO WEARING ASPLINT.
EMPLOYEE'S HARD HAT DROPPED AND EMPLOYEE REACHED DOWN TO PICK IT UP AND HE STRUCK IS HEAD ON THE STEEL GUARD ON THE CRANE. EMPLOYEE CUT THE TOP OF HIS HEAD AND HAD TO HAVE 2 STITCHES.
EE WAS HELPING INSTALL A GEAR BOX. HE HELD CRIBBING WHILE A CO-WORKER WAS HITTING IT WITH A SLEDGE HAMMER. EE'S HAND WAS BETWEEN CRIBBING AND ANGLE IRON IN A PINCH POINT AND WAS MASHED.
EMPLOYEE WAS REMOVING ROCK FROM THE CRUSHER. WHILE LIFTING THE ROCK HE FELT SOMETHING PULL IN HIS LOWER STOMACH. HE WAS FIRST DIAGNOSED AS LOWER ABDOMINAL STRAIN, BUT NOW IS BEING CHECKED FOR KIDNEY STONES. EMPLOYEE IS RESTRICTED.
EMPLOYEE WAS GREASING A TRUCK. ALLEGES HE WAS CRAWLING OUT FROM UNDERNEATH THE TRUCK AND TRIED TO GET UP TOO SOON AND HIT THE TOP OF HIS SHOULDER ON THE TAILGATE BAR. HE DIDN'T GO TO THE DOCTO R UNTIL 4-21-00. HE DID NOT KNOW IT WAS WORK RELATED UNTIL 4-27-00.
EE WAS BUILDING A GATE FOR THE PIT ENTRANCE. EE WAS CLAMPING ANGLE TO TOP RAIL OF GATE, ANGLE SLIPPED OFF SMASHING HIS LEFT MIDDLE FINGER. HE HAD SURGERY.
