Employee stated that while loosening bolts on spiral pump motor he felt pain in his left shoulder. Employee finished the shift and did not notify his supervisor until two days later.
Employee was using a slate bar to remove a rock from on top of the continuous mining machine. In the process of prying the rock off of the continuous mining machine, the rock slid down the slate bar and smashed the employees little finger on his left hand.
Employee was cutting belt in order to make a splice in the belt. He was using a utility knife to cut the belt and he was positioned next to the rib and movement was limited. As he pulled the knife and it cut thru the belt he struck his left thigh with the blade causing a laceration to his leg that required 4 stitches to close properly.
Williamstown contractor stated that while assisting other miners in placing a locomotive back on the track rails he sprained his left knee while using a lifting jack.
Employee was helping set timbers at 13 brk on 3 South belt line. He was using a pick hammer to set the cab wedges. He swang to hit the cap wedge with his hammer. When he did the hammer bounced back and the sharp end of his pick hammer struck him in the face.
Employee was using a pair of chain hoists to pull in belt on 4 South. In the process of operating the chain hoist the clamp came loose off of the belt and caused the employee to fall off the "A" frame where the employee was standing beside of the rock box. This caused the employee to strain his right knee, ankle, and put a small bruise on his face.
The employee stated that he was attempting to cut a 200' piece of CO cable from a 500' spool with a utility knife. He was holding the knife in his right hand and using his left palm to apply pressure when the knife flipped over cutting the palm of his left hand. The employee sought medical attention and received 6 sutures due to the laceration & returned to finish his shift.
Injured employee was attempting to release a chain hoist after completing a belt splice. The employee left his left forth finger on the chain and as the hoist suddenly released his finger was caught between the chain and the belt resulting in a fracture.
crew was pulling rail, employee was using a slate bar to pry the moving rail away from the existing rails when the slate bar came back and struck the employee in the face.
Employee was adjusting the height of an adjustable Caterpillar jack stand. The tube slipped from his hand and his little finger on his right hand was in the bottom adjustment hole and the tip of his finger was amputated. Employee was transported to CAMC General but the tip could not be re-attached.
Injured employee sustained a laceration to his left middle finger while assisting in moving the 10 Right section conveyor belt. While attempting to position a bottom roller he placed his hand on a bed rail. A fellow employee was attempting to bend the tab on the bed rail and inadvertently struck the injured's hand with a hammer.
Employee received a laceration to the lower lip that required 5 sutures to close the wound. Employees states that he was tightening the bolts that secure the cutter motor to the frame of the cutter boom. Employee fell into the machine as he was pushing down on the ratchet handle.
EE was attempting to replace a 5" roller in the 4 East take-up by using a slate bar to pop the roller up from the roller bracket. He had both hands & one knee on the bar. When the roller popped out of the bracket it caused him to strike the back of his right hand against the frame of the take-up. He was diagnosed with a hairline fracture on the back of his hand.
The employee stated that he was assisting with the removal of a scrubber hood on continuous miner when the slate bar he was holding came loose and the weight of the bar lacerated the webbing between his thumb and forefinger. The incident is a medical reportable due to the employee receiving 7 sutures.
Injured employee sustained a laceration to his left hand while cutting metal bands from a cube of block. Three sutures were required to close the wound.
Employee was struck on the top of the head by a chain hoist. Hoist was hooked to a belt hanger bracket and around a feeder conveyor chain that was fouled. The belt hanger failed while ratcheting the hoist resulting in the hoist striking the employee. Employee was transported to CAMC General Hospital, where he received two staples in his head and was then released.
Employee stated that he was prying down loose rock. His feet became entangled in some draw rock. Causing him to fall forward injuring his shoulder.
Employee was using a utility knife to cut a dust hose for a roof bolter. The employee cut his left wrist as he was cutting the hose.
Employee was helping another employee remove a tram motor from a shuttle car. The other employee was tapping on the gear with a hammer in order to try to remove the gear. A piece of the gear penetrated the employee's right forearm.
Employee states that he was repairing L brackets on an FCT with an 8lb sledge hammer and after completing the job his lower back hurt.
EE stated he was prying on the take up dolly with a slate bar, when the bar slipped, his left thumb struck against the I-beam. Medical treatment when the thumb was set with a splint. X-ray reports received on 8/12/14 showed "no evidence for fracture". This injury was submitted on 08/13/14, but on 09/18/2014, there is no record of the submission. Therefore, it is being resubmitted.
Employees were removing lagging from a counterweight pulley and bolting off excess lagging. The belt was secured by clamps, counterweight picked, and system de-energized (i.e. locked out) prior to starting work. A (1) ton come-a-long chain link broke allowing the slack to be taken up quickly in the system. Employee was caught between rollers/pulley & belt
Employees were removing lagging from a counterweight pulley and buffing off excess lagging. The belt was secured by clamps, counterweight picked, and system de-energized (i.e. locked out) prior to starting work. A (1) ton come-along chain link broke allowing the slack to be taken up quickly in the system. Employee was caught between the roller/pulley and belt.
Employee cut a metal band on a roll of belt and the band struck him in the face chipping his tooth. *Management did not deem the accident to be reportable until 7/20/2014.
Electrician was removing the tape and splice jacket from the 208 continuous miner cable. As employee cut across the cable, the utility knife slipped out of the boot and this resulted in the employee receiving a laceration to his left hand. The laceration required 8 sutures to close.
Employee stated that he attempted to trim a tree limb using his pocket knife, when the knife slipped, and cut his left pointer finger. This resulted in a laceration that required 4 sutures to repair the wound. This incident resulted in no lost time.
The employee stated that he was prying a metal plate away from the feeder conveyor chain with a slate bar when had pain in his shoulder. The employee had a previous injury at another mine that he was being treated for, but this incident is being reported due this occurrence which resulted in a strain and follow-up visits.
Employee stated that he was installing a cable stand off on a continuous miner. While installing a bolt the ratchet, 1/2 inch drive with a 30 mm socket, malfunctioned and caused him to stumble forward. His lower back was sore the next day.
Employee was making a cable splice with a spice knocker. He struck the splice knocker with a ball peen hammer and a piece of the splice knocker chipped off and embedded in his left hand.
The EE stated he was ballasting track outby the 1 South Section track with the ballast car. He stated he had already dropped 2 of the 4 compartments and as he dropped the 3rd one he pushed the wrench down with his right hand to release the locking mechanism with his left hand. Hand slipped with the lock disengaged causing wrench to strike EE on R side of face, sutures required.
Employee stated that he was using a slate bar to straighten the front basket of a mantrip. As he pulled on the slate bar, he felt pain in his right shoulder causing him to lose his grip on the slate bar and fall to the ground. This resulted in a back and right shoulder strain.
Employee was removing a screen from a clean coal vibrator. He was using a utility knife to cut a portion of the plastic screen when it slipped contacted his small finger of the left hand causing an incision that required 2 sutures to close.
Employee was using a cut off tool to cut a copper bar. Employee was holding the copper bar with a pair of channel locks and as he was cutting the bar the cut off tool kicked and struck the top of his finger with the rotating disc causing a laceration that required 3 sutures to close.
EE stated he was knocking the lugs loose from the track rails while retreating the track off the #2 section. During the course of the process ee reported pain in his lower back. This incident became a lost time inj on 3/7/14 when the ee missed work associated with the inj. A sought med treatment on 8th was diagnosed with a strain. EE missed 4 days and retuned on the 12th.
Employee was jacking a tub back onto the rail. In the process of pushing the tub back on the rail, the jack creeled over and struck employee on the right middle finger causing a laceration.
Employee was cutting a small notch out of a roll of belt to determine the carrying side of the belt, which was to be added into the existing belt-line. He was using a Lutz, Model 82 utility knife to cut the notch. As he made the last cut, the knife slipped and the blade struck his left forearm. He sustained a 2" laceration to his arm which took 7 stitches to close.
The employee was making a cable splice and started to crimp the burndy clamp. He was using a splice knocker to crimp the sleeves. When he struck the knocker, a piece of metal chipped off and went into his hand. The employee thought that he removed the metal but went to the doctor on 1-13-14 and underwent surgery to have it removed.
Injured employee sustained a laceration to his left wrist/hand while operating a roof bolter on 1 North Mains. The employee was inserting a 12' cable bolt into the hole when the plate caught on the bolt. He used the bolt wrench to strike the plate when the plate came loose and moved down the bolt striking his hand. Three sutures were required to close the wound.
The employee stated that he was helping to construct a stopping when he struck his right index finger with a hammer. The finger was splinted as a result of the incident.
Employee was cutting a piece of ventilation curtain with a belt knife. He was not wearing gloves and was cutting toward his hand with the knife. The knife slipped through the curtain and the blade struck the back of his left hand causing a 2" laceration. He received 7 stitches.
EE was trimming a piece of belt off the bottom of a door when he was finishing the cut. The knife came through the belt and into his left arm causing the injury. EE will return to work when the results of a drug screen are available.
Injured employee was prying on a take-up cart to break it loose. After he broke it loose he stated his back was sore. He finished the shift with pain in his back. Employee did not begin to miss work until 12/10/2013.
Employee was changing bits on the continuous mining machine. One of the bits was broke off in the bit lug. Employee was using a hammer trying to beat the broken bit out of the lug. At this time a piece of metal flew back striking the employee in the left forearm.
Sunny & Hot. Stuck bushing was using a sledge hammer and a pin hammer to get it on in. When hammer chipped off and went in left hand.
Employee stated that he was using a belt knife to cut a piece of belt rubber and as he pulled the knife slipped and cut him on the top of the left hand. This resulted in a laceration that required sutures.
Employee was installing the cutter head pin on the continuous miner. As he struck the pin with a sledge hammer a piece of the pin broke off and imbedded into his right leg just above the knee. The foreign body was not removed and the wound was closed with sutures.
EE was spotting belt to make splice when he spotted a flapper on the belt. He stopped belt and started to trim flapper with a belt knife. He pulled the knife toward his leg and when the knife cut through the belt, it went into his right inner leg just above the knee. He cut two blood vessels in his leg. The wound required 6 stitches to close.
The injured was cutting open a rock dust bag when the razor knife slipped causing him to cut his forearm.
Employee stated that he was hitting the core breaker pin when a piece of metal went behind his safety glasses and into his right eye. Medical received on 12/19/2013 confirmed a medical reportable incident.
Employee was helping change bits on the shear. Another employee was using a pick hammer to get mud out of a bit insert. A piece of the pick hammer chipped off and hit the employee in the face.
