Employee was changing shuttle car tire. Employee put bar under tire to raise up tire and rim to place on wheel assembly. When employee pulled up on the lifting or pry bar, employee felt a pain in lower back and left groin area.
Employee was holding Separator Speed Reducer while C0-worker was hammering on Sprocket. A small piece of the sprocket broke off and a sliver of the metal hit the injured in the forearm and the sliver of metal was imbedded into the Rt. Forearm / wrist area.
Employee was in the area where a bolt machine axle was being beaten out of the machine with a hammer. A small shard of metal broke from axle hub and struck the employee in the right forehead.
Employee was pulling a piece of belt with vise grips when the belt got caught on something, over extended EE's arm and shoulder causing a pop in EE's left shoulder and pain in EE's arm.
EMPLOYEE WAS WORKING ON A RAM JACK PIN. WHEN EE WAS LINING UP THE PIN EE'S RIGHT INDEX FINGER WAS AMPUTATED JUST ABOVE EE'S MIDDLE KNUCKLE ON EE'S RIGHT HAND.
EMPLOYEE WAS CLEANING ROCK OFF THE MINER WITH A SHOVEL WHEN LEFT FOREARM CAME IN CONTACT WITH A SHARP PIECE OF ROCK CAUSING A LACERATION THAT REQUIRED 7 STITCHES, NO LOST WORK DAYS.
