I was swinging a 16lb sledge hammer trying to install collets on a 993K wheel loader. I was in an awkward position when using the hammer.

Employee states that while making a belt splice he was using channel locks to pull on the belt, he felt pain in his right shoulder. After a couple of days, the pain and discomfort continued and he went to the Dr. and he was taken off work.
Employee was unloading supplies from a flat car with a chain and pallet puller. As the materials were being removed the chain came loose and pinched employee's hand between the chain and scoop bucket.
Employee states that as he was attempting to set a Hinzman jack he lost his grip in the jack body. The head of the jack came down and caught his Left hand between the jack head and body.
Employee was taking out a belt pin on an old splice. As he was striking the old pin with a hammer a piece of metal struck him in the left eye.
Employee was making a splice on the CM when the utility knife slipped and struck him in the right hand causing a laceration.
Employee was prying with a slate bar on a piece of metal. The metal slid off the 4x4 blocking and shifted all the weight on to the slate bar. the slate bar struck his (R) foot.
Employee states that while repairing an old splice, the utility knife that he was using slipped out of the splice and caused a laceration to his left forearm.
Mechanic was changing out the canopy lift cylinder on a roof bolter. He chained the canopy to the roof on a chain hanger and cut the pins out of the cylinder, they applied heat to the cylinder eye and applied down pressure with the boom of the bolter and then hit it with a sledge hammer. The cylinder broke loose and the miners foot was under the drill pot of the bolter.
Employee was installing a center pin in coal hauler 141. He struck the pin with a sledge hammer, when he did a small sliver of the hammer broke off and embedded in his right cheek.
Employee states that while making a cable splice for the CM, he cut his leg with a utility knife.
Employee was swinging the panel for the miner back in, using a pry bar for leverage. The bar slipped causing the employee to fall forward hitting his head on the top of the miner. The employee had a 1 1/2 inch cut above is right eye. He received 5 stiches.
While jacking up a derailed rail car, the employee was using a lifting jack, the jack slipped out and the rail car fell on both feet.
Employee states that while working on the conveyor chain he was cutting a metal snap ring off with a hammer and chisel, the ring broke and a piece went into his chin.
Employee was changing a bit cone on the miner. He was trying to put the snap ring on the cone, while he was having another employee hit the cone with a sledge hammer. He got his hand in between the top of the cone and the sledge hammer, smashing his left ring finger.
miner was struck in the forehead while attempting to removing a belt splice pin using a claw hammer, he hit the claw hammer with another hammer causing a piece of metal to chip off and hit him in the forehead just above his safety glasses.
Employee states that he was cutting tape off the drill chuck when he cut his (L) hand with a utility knife
Employee was slicing an apple with a razor knife and the knife slipped cutting the palm of his left hand. stitches were applied.
Repairman was repairing a feeder conveyor chain. He was driving the keeper back on the pin when a piece of the tool he was using as a punch flew off and went into his left lower thigh. He recieved 3 stitches and has not missed any work due to this accident. The piece of steel that went onto his leg was approx. 1/2" X 1/4" X 1/4".
Was unplugging chute in prep plant using a 3/8" rod, the rod slipped out of his hand. He reached out for it and at that time he felt pain in his lower back. Started missing work on 1/11/11.
Employee states that while removing old rope hangers off his lil mac, he cut his (L) hand with a utility knife.

EE was squaring the end of the belt for a splice. He was trimming off a piece of rubber, when his knife slipped, cutting his right leg above the knee. *Note: We were 1st notified of this incident by mail on 8/5/08 form Workers Comp. Then I received a report from the mine on 8/6/08.

He was working on #2 belt head drive chains. He was separating the straps on the chain by driving a chisel between the link & strap. He was going to move the chisel to the other side & hit it sideways & chisel flew up & hit him in the lip.
Employee was using a slate bar, assisting miner oper. in removing rock off of miner, when slate bar rolled smashing hand between slate bar & miner.
EMPLOYEE WAS MAKING A SPLICE IN CABLE WHEN HE HIT HIS LEFT THUMB WITH A HAMMER.
EE WAS LIFTING SPEED REDUCER INTO PLACE WITH CHAIN RATCHET WHEN LIFT EYE CAME OUT OF REDUCER AND CAUGHT EE'S LEFT HAND IN CHAIN.
Worker was changing discharge lip on #1 raw coal screen. He was pushing on a 6"x3/4 carriage bolt while another worker was using an impact wrench to tighten bolt. The bolt had a sharp edge. As they were tightening the bolt, it spun cutting through worker's glove causing a cut to the right index finger. Six stitches to close wound.
While making splice on miner cable knife slipped and cut right leg above knee.

INJ WAS INSTALLING J30 BIT INSERTS IN CUTTER DRUM USING HAMMER WHEN A PIECE OF METAL SEPERATED FROM THE INSERT STRIKING INJ IN THROAT.
PRYING ROCK WITH SLATE BAR. SLATE BAR SLIPPED OUT FROM UNDER ROCK. PINNED FINGER AGAINST CHUTE.
WHILE LIFTING 12" ELBOW WITH CHAIN HOIST, ELBOW BECAME LODGE. WHEN EE'S HELPER ATTEMPTED TO FREE ELBOW, IT CAUGHT EE'S HAND.
