Employee was attempting to get an ore bin to feed by poking the feed with a metal bar. EE accidently hit a support beam with EE's hand pinching EE's finger between the beam and the bar and causing a bruise.

Employee was pulling on a wrench while working on the jumbo drill and felt something pull in left shoulder. The employee did not get medical treatment when it first happened. EE decided to make an appointment with a doctor for February 23, 2016 because the shoulder was not getting better. Employee is scheduled for 4-6 weeks of therapy.
Employee was standing on a 3 step stool using a 24'' pipe wrench loosening a 2'' pipe. As the pipe came loose the wrench slipped and he lost his balance having to step down off the stool quickly straining his right calf. Employee was seen at the emergency room and lost 3 days work. Returned to work 03/18/2011.
Accident 9/25/08, but didn't receive medical attention until 9/30/08. Employee was unplugging wet clay conveyor for Mill 1 with an iron bar. The clay loosened, causing his left hand to hit the side of the bin, leaving a contusion.
Employee stated that he was swinging a large hammer trying to free up material in dry ore silo. As he was swinging he said he felt pain in the lower back. He was seen at the emergency room. Employee lost one work day returning on 3-3-2008.
Employee was climbing a ladder and was at the top of the ladder when the hammer fell out of his pouch, striking another employee in the face / nose while he was at the bottom of the ladder. The employee broke his nose in 2 places.
EMPLOYEE WAS HELPING MILLWRIGHT TAKE UP NO.1 CRUSHER USING A 1-1/2' WRENCH TO LOOSEN 3" NUTS ON CRUSHER. EMPLOYEE TRIED TO LOOSEN THE NUT TWO TIMES WITHOUT MOVING NUT. ON THIRD TIME NUT RELEAS ED CAUSING LEFT THUMB TO BE PINCHED BETWEEN WRENCH AND 8" PIPE.
NO 4 HARDINGE DISCHARGE SCREW WAS PLUGGED. EE TRIED TO CLEAR PLUG BY TURNING SCREW CONVEYOR WITH 3' PIPE WRENCH. AS HE WAS TRYING TO TURN SCREW CONVEYOR WITH PIPE WRENCH HE FELT PAIN TO HIS LO WER BACK.
EE, OUR MECHANIC, WAS CHANGING THE OIL IN A VEHICLE WHEN A PIECE OF RUSTY METAL DROPPED IN EYE. EE WASHED EYE OUT AND CONTINUED WORKING. THIS WAS ON FRIDAY. THE WEEKEND CAME AND HIS RIGHT EYE BOTHERED HIM. MONDAY MORNING WE SENT HIM TO THE EYE DOCTOR.
