Mostrando los 6Employee was using a a cheater pipe ratchet to adjust the belt on the tail pulley at the hopper. Employee was pushing away from EE's body on ratchet when it sprang back and struck EE on the right lower part of EE's face and jaw. EE had a split lip with contusion and some tooth damage. EE received stitches in the lip and went to a dentist.
Employee was changing teeth on backhoe bucket. He struck a tooth with a hammer to loosen it and a piece of steel broke off striking him in the forehead above his safety glasses. Small piece of steel lodged under skin. Went to ER where steel was removed and the wound glued shut. No lost time happened on Saturday just before plant shut down.
EE was trying to loosen a lug nut on a tire. As he put all the pressure on the socket, it slipped. His momentum caused the left side of his forehead to strike the rim of the tire. A 3/4" long gash was opened on the left side of his head, just above his eye. Continued to work with just a bandaide, until he determined he needed stitches. He received 4 stitches.
EE WAS HOLDING A PIECE OF CONVEYOR BELT ATTEMPTING TO CUT IT WITH UTILITY KNIFE. AS HE PUT DOWNWARD PRESSURE ON THE KNIFE TO CUT, HE LOST HIS FOOTING IN THE GRAVEL HE WAS STANDING IN CAUSING THE KNIFE TO CUT HIS RIGHT THUMB & INDEX FINGER.
EMPLOYEE WAS BINDING DOWN A PIECE OF EQUIPMENT ON THE LOW BOY USING RATCHET TYPE BINDERS. ONE OF THE BINDERS SLIPPED AND HIT EMPLOYEE UNDER THE CHIN CAUSING A LACERATION THAT REQUIRED 3 STITCHES.
