While using sledge hammer in an attempt to switch over a stuck flop gate, employee caught his little finger between the hammer handle and the lever for the flog gate resulting in a laceration.

Two employees building cribs in 4-East area of mine, as one employee tightened his crib, the sledge hammer handle hit the crib, bounced out of the employee's hand causing the sledge to fly, striking an employee on the left side of his face causing a 3/4-inch laceration on the left side of his face between his lips and nose requiring sutures to close the wound.

Mine Maintenance Chief was working a come-along to pull pipe together when he felt a pain in his right shoulder. Initially diagnosed as a right shoulder sprain and he was released to regular duty. A recent MRI shows anterior labral tear of his right shoulder requiring surgery to repair. He become a LTA on March 1, 2013.
Injured claims that when tightening a bolt on the cable tender, the 8-inch crescent wrench slipped causing him to lose his balance falling backward and twisting his left knee. Initially diagnosed as a strain and he was released to regular duty. A recent MRI shows a left meniscus tear requiring surgery. He became a LTA on March 1, 2013.
Employee cut his left thumb with a utility knife while making a splice on B Main Belt. He was wearing gloves.
Employee was using a rock bar to scale down a piece of sand rock when the rock shifted allowing the bar to suddenly rise up and strike the employee in the mouth causing a laceration of the lower lip requiring 3 sutures.
Injured was repairing a broken coal chain on the miner using a chain brake tool, chisel, and a hammer. He struck the chisel with a hammer causing the chisel to fly up and strike him in the head causing a laceration. The wound was closed with sutures.
While using a razor knife held in his left hand to cut zip ties off of hydraulic hoses in scoop tractor center section, the knife skipped across a zip tie and cut into his right wrist positioned approximately 18" away.

As the motor was turned from from horizontal to vertical, it slid on the chain pinching the finger of the employee between the shaft and the motor base causing a fracture to the finger tip.
Employee was knocking CAT pin out when a piece of steel dislodged, striking employee on lower lip. Employee did not seek medical attention until 07/09/07.
Employee was pulling the belt pin (spindle) on a splice using a claw hammer when he was struck in the eye with a sliver of metal which required eye surgery.
Employee was breaking a chain with a hammer and a pice of metal came off the pin in the chain. This metal became lodged in employee's right hand.

EE WAS HOLDING A PIN REMOVAL BAR, MECHANIC WAS STRIKING BAR IN ORDER TO REMOVE THE PIN. MECHANIC MISSED THE PIN REMOVAL BAR & HIT LEFT WRIST OF EE'S ARM.
