Employee was changing out a punch screen on a screen deck with a pry bar. The pry bar slipped causing the screen to fall, pinching the right middle finger between the screen and screen deck structure. The finger suffered a fracture on the tip. Employee was released to full duty.

Mostrando los 22Employee was using a 2 pound hammer to remove a key stock from a bushing on a 400 HP motor that was seized to the shaft. While driving out the key, EE hit EE's thumb with the hammer, causing an injury to EE's left thumb. Employee received 7 stitches on left thumb.
Employee was lowering the belly pans so that EE could remove the engine oil pan because of crack. EE had a chain tied to the front lower eye weldment and ran the chain to the rear of dozer under the counterweight and then vertically tied to the hook on the crane boom to hold tension on the chain to keep the pan from dropping. The pan dropped causing EE's finger to be pinched.
The silo feeding the 68 SBS crusher was bridged over the discharge feeder. After attempting to unclog the material with a sledge hammer on back of chute which didn't work. The employee unclogged the chute using 2X4. As the chute unclogged a rock hit the 2X4 caused it to smash employee's finger against chute.
Employee was assisting installing a conveyor belt. The belt got stuck and employee attached a belt clamp and cable jack to try to free the belt. While cranking down on the jack the clamp slipped from the belt causing the employee to lose EE's balance, plant left foot & fracture a small bone in the ankle. Employee was transported to the hospital, treated and released.
Employee was using a sledge hammer to bang on the side of a chute to unclog the chute and his right ring finger was caught in-between the hammer handle and the chute. He received a laceration to his finger along with a hair line fracture.
Employee was using utility knife to cut flashing for conveyor feed chute skirting and cut left wrist requiring sutures.
EE was repairing a water leak in a PVC line. He cut the line using a utility knife and was cutting towards himself, when he struck his right thumb. He was taken to the Powell County Clinic and the injury required 6 stitches. He retruned to work the next morning.
Employee was using a 2' long pry bar to open the inspection lid on the door of a USI crusher. He was pulling bar toward himself when the bar slipped striking employee in the right corner of his mouth. The corner of the employee's mouth was cut and required 4 stitches.
Employee was cleaning chute feeding conveyor belt & struck side of conveyor support with left wrist & bruised hand. Went to Urgent Treatment Center for x-rays & found small bone in left wrist bruised. EE struck side of support brace when exiting the conveyor. Will be on restricted duty until 6-19-08.
Employee was using a sledge hammer to knock a bin door down to adjust it to another position. The hammer glanced off causing his finger to get caught under the hammer handle.
Employee was priming water pump & found ice formed in the pump. He was attempting to remove the ice with his pocket knife & when he closed the knife he struck his finger, requiring stitches.
WORKING ON HOE RAM, DRIVING PINS OUT OF HAMMER. MISSED SWING WITH SLEDGE HAMMER AND TWISTED AND SPRAINED BACK.
EE WAS ASSISTING ANOTHER EE TO CHANGE O-BELT ON THE DRIVE OF A IMPACT CRUSHER. HE THINKS THAT WHILE LEFT THE MOTOR USING A PRYBAR HE STRAINED A SMUCLE IN HIS BACK. AFTER X RAYS DRS. DETERONINE D THAT HE POSSIBLY HAD STRAINED MUSCLE AND PRE-CRIBED PAIN MEDICINE TO EASE SYMTONS AND EXCUSED HIM FROM WORK UNTIL 6-6-99.
WAS CUTTING TIE STRAP WITH POCKET KNIFE WHEN KNIFE SLIPPED AND CUT LITTLE FINGER ON LEFT HAND. IT TOOK 10 SUTURES TO SEW FINGER UP.
EE WAS INSTALLING THE SCREEN SUPPORT COLUMNS ON A NEW SCREENING TOWER. HE WAS ALIGNING THE BOLT HOLES UP WHEN HE HAD GOT OUT OF THE BUCKET TRUCK AND CLIMBED ONTO THE SCREENING BINS. HE ATTACHE D A 2 TON COMEALONG TO A LEG AND WRAPPED THE CABLE AROUND THE LEG HE WAS ALIGNING. HE POSITIONED HIMSELF ON A 12" H-BEAN AND STARTED TWISTING THE LEG. HE APPLIED TOO MUCH PRESSURE TO THE COME
EE ATTEMPTED TO BREAK AN AUGER FREE WITH A PIPE WRENCH. HE PLACED ONE FOOT ON A RAIL WHICH WAS ABOUT A FEET HIGH AND LEANED HIS WEIGHT ON THE WRENCH. THE WRENCH BROKE FREE CAUSING EE TO FALL O N HIS LEFT SHOULDER.
EE WAS TIGHTENING BOLTS ON A CRUSHER AT QUARRY AND WRENCH GAVE WAY AND HE FELL ON A PROTRUDING BOLT, STRIKING RT ARM.
HAD COME A LONG HOOKED ON ANGLE IRON ANGLE IRON SLIPPED CAUGHT INDEX FINGER ON LEFT HAND BETWEEN METAL
CLEANING OUT UNDER A CONVEYOR SHOVEL HANDLE STRUCK CONVEYOR JAMMING WRIST CAUSING MILD SPRAIN.
EMPLOYEE WAS REMOVING BEARING FROM A SPEED REDUCER ON THE #1 CONVEYOR. EMPLOYEE REMOVED HIS SAFETY GLASSES TO LOOK AT THE BEARING AND TAPPED THE BEARING WITH A SMALL BALL PEEN HAMMER. A METAL FRAGMENT FLEW INTO HIS LEFT EYE PENETRATING THE PUPIL.
EE HAD TO OPEN 7HE HAMMERMILL TO CHECK THE WEAR ON THE HAMMERS AS HE WAS TIGHTENING THE LID BACK DOWN HE USED A PIPE WRENCH TO TIGHTEN THE NUTS BACK & THE WRENCH SLIPPED OFF HE FELL ABOUT 4' TO THE PLATFO9M-BRUISED RIBS.
