On 8/29/2008, the employee was swinging a 10 pound sledge hammer against the side of a rail car to unload stuck limerock roadbase and suffered a strain in his R shoulder. The shoulder was sore for a couple of weeks and did not improve. On 9/15/2008 the employee was taken to the doctor for evaluation and diagnosed with a shoulder sprain and placed on light duty.

Mostrando los 21On 1/7/2008 Employee was unloading rail cars. As he was closing the rail car hopper door with a spud bar, he lost his footing and twisted his knee. He was taken to the doctor to be examined the following day, placed on restricted duty and given a prescription for pain.
Employee had complained about a lump in the palm of his right hand on 9/28 at 2:00. There was no problem at the time and he requested to see Dr. Joy who had seen him before. The doctor's next available date was October 6th. On Oct. 6 the doctor placed him on prescription drugs and refered him to a specialist. This is now a reportable accident as of October 6.
Employee cut his hand with a sharp knife. He was attempting to score and break a plastic tray. The knife slipped and cut his hand. Sutures were required.

Ee complained of sore arm about a week after the event (4/10/06). On 4/12 he went to see his own physician who told him that he had a strain and that it was work related. On 4/13 he was sent to our dr. who diagnosed a strain. PT and light duty prescribed.
WORKING ON SCRUBBER PUTTING BELTING ON THE INSIDE. HE WAS USING A SHOVEL TO PUSH DOWN ON IT AND IT SLIPPED AND CAUGHT HIS LITTLE FINGER BETWEEN SHOVEL AND SIDE OF BELT.
Ee developed callous on right hand from using hand tools. Callous is impinging on movement of right little finger.
EE WAS OPENING A DOOR ON AN EXPANSION CHAMBER (1ST STAGE- 7TH HOUR). USING A PRY BAR, HE WAS PULLING AGAINST A STUCK HINGE. THE BAR SLIPPED, HE STEPPED BACKWARD & TWISTED HIS FOOT, FRACTURING A BONE IN HIS LEG.
EE WAS LOOSENING THE NUT ON A DRAW CYLINDER FOR THE GRAVEL BED FILTER SYSTEM. HE WAS TURNING A NUT WITH A WRENCH. THE NUT CAME TO THE END OF THE THREAD AND THE SHAFT FELL PULLING THE WRENCH DO WN, CRUSHING HIS FINGER.
WHILE EE WAS CUTTING A CONVEYOR BELT THE RAZOR KNIFE HE WAS USING SLIPPED OFF BELT, CUTTING HIS LEFT MIDDLE FINGER.
EE CUT THE BACK OF HIS HAND WITH A PUTTY KNIFE WHILE CLEANING THE WAX OFF OF A CEMENT MOLD IN THE LAB AREA.
CLEANING OUT GRIZZLY. HE HIT HIS SMALL FINGER ON ONE OF THE RAILS WHILE USING A BAR TO CLEAN BUT BUILT UP MATERIALS.
EE STRAINED HIS BACK WHILE USING A HEAVY HAMMER OVERHEAD IN THE FINISH MILL.
WHILE CLEANING CONE CRUISHER @ BASE PALNT WITH IRON BAR THE CONE CRUSHER CUASED THE BAR TO COME UP AND HIT HIM UNDER THE CHIN.
EE CUT THUMB W/ KNIFE WHILE CUTTING RUBBER BELT SHEET.

ON 12/21/00 INJURED WAS ON PROPERTY TO REMOVE A B.E. MODEL 38B RUBBER TIRED TRUCK CRANE/CLAM SHELL MACHINE. THEY REMOVED THE BOOM & SHIPPED IT OUT. THEY THEN BEGAN TO REMOVE COUNTER-WEIGHT. TH ERE WERE 2 STUDS SECURING WEIGHT THAT WERE RUSTED. THEY SPLIT NUTS W/TORCH & USED PIN & HAMMER TO BREAK OFF. THE WEIGHT FELL 6' ONTO INJURED LANDING ON THIGHT/LEG. 911 CALLED, EE CONCIOUS/TAL
EE WAS WORKING AT BARE PLANT WHEN A ROCK GOT SDTUCK. HAD TO BREAK IT WITH A SLEDGE HAMMER AND BAR AND STRAINED HIS SHOULDER. EE DID NOT VIOLATE ANY SAFETY RULES DURING THIS INCIDENT.
EE WAS USING REMOVEABLE CHUTE HANDLE. THE HANDLE CAME OFF AND STRUCK THE EMPLOYEE IN THE CHIN.
