employee experienced shoulder and elbow pain from overuse that was treated with first aid that helped but eventually worsened. The employee eventually requested to be seen by physicians where they received medical treatment required for reporting.

Employee was cutting a piece of corroded angle iron with a spear like point while inside of a chute at the wash plant. The employee tapped on the cut piece of angle iron with a hammer causing the entire 8-foot piece to come loose, falling and puncturing the top of EE's left foot a quarter inch above EE's steel toe boot.

Employee was cutting a liner with hand saw and cut index finger on left hand.

EE threw a track on EE's L8 drill rig. While compressing the track tensioner EE struck the tensioner with a sledge hammer. When struck a shard of metal broke off the tensioner and punctured EE's right thigh. EE received sutures and butterfly closures at the Urgent Care Center.

Employee was using a hydraulic wrench working on crusher when wrench slipped and pinched a finger against metal flange.

Employee was hand scaling rib wall when bar slipped and EE hit their right hand against rock surface. Resulted in bruise to middle finger of right hand.

Employee was trying to dislodge bearing in crusher shaft that was stuck, employee used prybar to move bearing, it fell down shaft and struck employee's finger and top of hand.

Miner was cutting packaged material

Employee was using torque wrench and did not release handle. Wrench jerked employees arm and shoulder

Employee was trying to loosen a nut on an oil screen and lacerated hand.

Employee was swinging sledge hammer repeatedly to remove drill bit and then felt soreness in lower back.

Employee was holding screen in place and was struck on finger with hammer.

Driller was hammering on drill bit and metal shard flew off and hit employees hand

Hydraulic pump was being lifted by chain, chain slipped and pump fell onto employee foot.

Employee was splicing conveyor belt and blade on utility knife broke off and cut finger.

EE was using a wrench to loosening a bolt on a screen guard. He pulled a shoulder muscle while pulling on the wrench.

EE was using a sledge hammer to tighten down nut on crusher mantle liner. He struck his right thumb nailbed with the hammer.

Employee was tightening up a come-a-long and the chain broke. Chain hit employees rt hand, and broke rt middle finger.

Pipe wrench slipped off screw and smashed L finger on side of screw shell. Finger was lacerated and required stitches.

Employee was hammering on a steel bar when a sliver of metal struck him in the chest.

4 Employees were tightening nuts on crusher using 20 lb sledge. A metal piece splintered off and struck employee in the lower R abdomen.

Employee was trying to open a clogged chute and hit his right index finger on a piece of sharp metal, cutting finger.

Employee was cutting belting, knife slipped and he cut the top of his left hand.

He was using a come-along to put steel plate in chute and developed pain in his right elbow.

He was helping set the tightness of a gyrapheric crusher and pinched his left ring finger. I reported this on a 7000-1 on 10/12/06 but it is not showing up in the system.

Side skirting was being re-installed to conveyor. A pry-bar techique is used to get rubber back into position. A slip occurred during prying process causing bar to pinch left ring finger tip.

In pump room of dredge taking bell housing off of the PTO. The housing was stuck on the shaft so they used a portapower to remove it. The housing suddenly shot off of the shaft about 8" striking the employees right hand.

Employee was using a pry bar and a spade to remove build up from underneath the primary motor base.

Employee was using a razor knife to cut tape. Knife slipped into the palm of his hand.

Track drill operator was using a hammer to help a drill bit into place. A piece of metal broke free from the hammer and struck him in the leg.

USING A KNIFE TO CUT HOSE FOR WATERLINE, SLIPPED AND CUT HIS HAND.

USING TWO WRENCHES TO LOOSEN BOLT. ONE WRENCH SLIPPED AND HAND STRUCK ANGLE IRON.

EE WAS USING A KNIFE TO OPEN THE LID ON A BUCKET.
