EE was working in tandem with a co-worker removing a bearing race. EE was cutting off the bearing race while a co-worker was tapping on the back side of the bearing race to remove it off the shaft when a chip of metal struck the EE on the lower left side of his neck going under the skin.

Employee was using a utility knife to cut a piece of skirt rubber. The knife slipped and cut the employee's right thigh. Employee sustained a three inch laceration that required 6 stitches. Employee was returned to full duty with no restrictions.

Employee was turning handle to a winch and let go of the winch handle. The handle to the winch spun backwards, hitting the employee on top of the left hand, breaking two bones in the hand.

Removing Bag-house inspection door with chain-fall, handle pulled out of its anchor and flew into scaffold crossmenber then into worker's uppper lip. Created a laceration from base of nose to edge of lip and required 5 stitched to close the laceration.

Employee was removing a pin from a pneumatic actuator utilizing a sledge hammer and a punch. Upon striking the punch with the hammer, a small piece of metal from the hammer punctured the employee's left hand. The metal had to be removed by a physician.

Employee was using a spud wrench to do an alignment. The wrench slipped and came into contact with his hand between the thumb and index finger resulting in a laceration which required stitches to close.

Employee was wrecking forms. When he was taking a clip out with the hammer, the clip came flying out and hit him in the lip and tooth.

Employee was driving a T-post stake into the ground with a sledge hammer when he struck his right ring finger resulting in a laceration. He went to the ER after work requesting they remove his fingernail for comfort. He was prescribed pain medication for comfort and antibiotics for preventive reasons.

Employee was chipping slag from a weld with a hammer. A piece of the slag penetrated his left wrist. He visited the doctor and the incident was initially treated as a first aid. Upon returning to the doctor (8/17/05) for a follow up visit, the doctor recommended the piece of metal be removed. The metal was removed on 8/19/05 and a stitch was placed over the wound.
