In the process of pulling electrical wire in a cable tray, EE was feeding the wire in the cable tray when unexpectedly it moved and caught his little finger of his right hand between the wire and the metal tray, lacerating the finger.
EE was working in tandem with a co-worker removing a bearing race. EE was cutting off the bearing race while a co-worker was tapping on the back side of the bearing race to remove it off the shaft when a chip of metal struck the EE on the lower left side of his neck going under the skin.
Employee was arc gouging, cutting with an electrode, when a spark entered his ear canal causing burning.
Employee was using a utility knife to cut a piece of skirt rubber. The knife slipped and cut the employee's right thigh. Employee sustained a three inch laceration that required 6 stitches. Employee was returned to full duty with no restrictions.
Employee was turning handle to a winch and let go of the winch handle. The handle to the winch spun backwards, hitting the employee on top of the left hand, breaking two bones in the hand.
Removing Bag-house inspection door with chain-fall, handle pulled out of its anchor and flew into scaffold crossmenber then into worker's uppper lip. Created a laceration from base of nose to edge of lip and required 5 stitched to close the laceration.
Employee was walking through the cooler area - he bent down to walk under low connector plate. He came up too soon and his helmet struck the bottom of the plate causing a sore neck.
Employee was removing a pin from a pneumatic actuator utilizing a sledge hammer and a punch. Upon striking the punch with the hammer, a small piece of metal from the hammer punctured the employee's left hand. The metal had to be removed by a physician.
Whilst inspecting the 331 area, the employee, EE twisted his ankle on the debris and uneven ground.
Employee was assisting co-workers with removal of track beams. When beams were removed off of the support beam, the support beam fell and struck her left foot, resulting in a fracture.
While flagging equipment transporter (vehicle ID#8024 Ottawa), a loaded flat bed with hot gas return duct, employee's right ring finger was caught between scaffold pipe and duct brace, breaking and lacerating ring finger.
Employee was standing on top of an air slide, the air slide is 6? 6? high, the employee slipped and fell to the floor. Employee had a broken arm from falling.
Employee was using a spud wrench to do an alignment. The wrench slipped and came into contact with his hand between the thumb and index finger resulting in a laceration which required stitches to close.
The person involved says he was adjusting the water valve when the bull hose came off the gunning machine and caught him on the elbow. At the time, he only reported a cut to the forearm on Friday 9/28/07 but then was reported as a fractured arm on Monday 10/1/07.
Employee was lifting the hood on the Mack Blasting Truck and felt a sensation in his L lower abdomen. Later that day he began to experience pain in the area. He was in pain the next day and sought medical treatment. He was diagnosed with a hernia in his L lower abdomen.
Employee was wrecking forms. When he was taking a clip out with the hammer, the clip came flying out and hit him in the lip and tooth.
On 8/11/07 employee was welding a discharge lip to a shaker screen. He was supporting the metal lip with his right arm. He turned his body and felt a pop in his shoulder. He felt minor pain in the shoulder for several months and on 1/2/08 he asked to see a doctor. Employee was released to full duty on 1/2/08 and given a prescription steroid for inflammation.
Employee was washing off scale with water hose. She lost her footing and fell backwards landing on her left hand and buttocks. She suffered a sprain to her left wrist.
The employee decided to assist with a crane load which was beyond his normal scope of duties. He was walking backwards with a tagline when he stepped off of the structure contacting a lower level.
The grinder the employee was using kicked back and struck the employee in the face shield. The face shield cracked and the grinder came into contact with the employee's face.

While in the process of water blasting the kiln, the preheater feed chute material was being water blasted & expelled back toward the employee & entered the protective hood. He gasped & inhaled (hot)dust into his mouth causing mouth burns.
Employee was driving a T-post stake into the ground with a sledge hammer when he struck his right ring finger resulting in a laceration. He went to the ER after work requesting they remove his fingernail for comfort. He was prescribed pain medication for comfort and antibiotics for preventive reasons.
While inspecting a expansion joint on a kiln exhaust duct. Hot dust came through fabric and contacted right hand forearm causing 1st degree burns to right arm & top of right hand and neck.
Employee was climbing up the ladder of a Cat 777 Haul unit and her hand slipped on the railing, but she did not fall. Later that evening she complained of back pain. The back pain persisted for several days. On May 24 she asked to see a physician because the back pain had not improved. Doctor reveiw revealed muscle spasm and she was treated with medication and placed on light duty.
While walking on a walkway beside a belt conveyor, he tripped on a piece of rope that is used to brack the conveyor. He never fell. The rope was removed. He broke a metal wire in his knee from an auto accident he was in back in 2001.
Employee and two other mechanics were lifting a metal skirtboard in place. During the lift the employee felt a pop in abdomen. A small lump in lower abdomen was noticed the next day. Employee visited physician for diagnosis. Employee was diagnosed with small hernia in abdomen. Employee placed on light duty and scheduled for surgery.
Employee was tightening bolts on screen deck with a 3/4 inch impact wrench. Socket and wrench separated during tightening and wrench traveled up employees arm inflicting a 1 inch laceration on his wrist requiring six stitches.
EE was fueling his loader. He stepped up on a step that runs along the fuel containment area & grabbed the fuel nozzle. When he stepped off the step, his left foot hit the ground & the ankle rolled inward. EE went to the ground in pain & called his supervisor. He was taken to the hospital. He says the foot just did not hit the ground level.
Employee was washing spilled material underneath plant with hose. She turned valve off on the hose she was using and another hose that was running underneath plant came loose from its location due to the pressure change and hit her elbow. Her elbow remained sore for a few weeks and she was taken to the doctor on 9/26/2005 for evaluation.
Employee was chipping slag from a weld with a hammer. A piece of the slag penetrated his left wrist. He visited the doctor and the incident was initially treated as a first aid. Upon returning to the doctor (8/17/05) for a follow up visit, the doctor recommended the piece of metal be removed. The metal was removed on 8/19/05 and a stitch was placed over the wound.
Employee was stepping off of a 920 Loader. When his right foot hit the ground, he felt a pain in his right knee. His knee continued to hurt throughout the day and he was taken to the doctor. He was placed on light duty and scheduled for further diagnostics of the knee.
EE WAS DESCENDING THE SOUTH LOWER STAIRWAY FROM THE PRIMARY CRUSHER DECK. ABOUT THE THIRD STEP DOWN, BOTH FEET SLIPPED ON THE STEPS, LEAVING CONTACT WITH HIS HANDS ONLY. EE TIGHTENED HIS GRIP ON THE HANDRAILS TO PREVENT BODY CONTACT WITH THE STAIRS. IN DOING THIS, EE CLAIMS TO HAVE TWISTED HIS LOWER BACK.
CONTACTED GUARDRAIL WITH UTILITY VEHICLE, SCRAPED DOWN THE LEFT SIDE OF VEHICLE STRIKING HIS HAND WHILE HOLDING ONTO SEAT BAR.
A Commercial Truck entered the scale and knocked the drain pan out of position with the tarp rack. Scale house operator climbed the ladder on the side of the truck to put the pan back into place. When the pan dropped back into position, the operator's thumb got pinched between the pan and the chute. Right thumb tip was cut, his nail was removed and he received six stitches.
EE was fueling his loader. As he twisted to replace the fuel nozzle he heard something in his back pop. He was in a great deal of pain and was taken to the hospital. He was prescribed medicine and placed on restricted duty by the physician. The physician states the injury may be a herniated disk.
Employee was walking down a flight of stairs in the Screenhouse and fell. Aparently, the employee missed the first step, fell on his back and slid down the length of the stairway. He dislocated his right shoulder as a result of the fall.
Employee was working on the Pond #12 pump platform. Another employee tossed him a bottle of oil from the bank (The toss was too high). Terry fell backwards trying to catch the bottle and landed on his left hand, breaking the wrist.
A piece of metal apparently entered employee's Left eye after wiping sweat off of his brow with a shirt sleave. He rinsed his eyes in the eye wash station and felt better. The next day he was in pain, so he was taken to the doctor where the object was removed. The cornea was scratched.
Employee caught his wrist between a roll and a door while turning the roll on a portable crusher. His wrist was hyperextended backwards and he suffered a sprain.
REPLACING LINERS IN MILL, A ROLLER SECTION IS USED TO TRANSFER THE LINERS FROM OUTSIDE TO INSIDE. A LINER ROLLED OFF THE END OF THE ROLLER SECTION STRIKING ANOTHER LINER. EE WAS BENDING DOWN TO PICK UP A BOLT, THE LINERS STRUCK SENDING A SLIVER OF METAL ABOVE HIS SAFETY GLASSES.
INSPECTING EQUIPMENT, STEPPED ON PIECE OF DEBRI TWISTING RIGHT ANKLE.
EMPLOYEE WAS WATERBLASTING ON THE FEED SHELF WHEN HOT MATERIAL GOT UNDER HIS GLOVE AND SLEEVE CAUSING 1ST AND 2ND DEGREE BURNS.
TWO MECHANICS WERE WORKING ON THE #2 AIR SLIDE. THEY HAD NOT DISENGAGED THE AIR VALVE OR SHUT THE MANUAL SLIDE GATE. WHEN THE LOADOUT OPERATOR RETURNED FROM A SAFETY CLASS & TOOK OVER LOADING TRUCKS, HE DID NOT KNOW THE MECHANICS WERE WORKING ON THE AIR SLIDE. HE SWITCHED THE KNOB TO LOAD OUT OF #2.
