Miner was using bottle jack to align steel plate on conveyor. A pipe was being used as an extension to the jack. As the miner began to raise the jack, the jack shifted and the extension pipe kicked out and the end of the pipe glanced his upper lip and struck the end of his nose.

Miner was holding wrench on bolt head. Wrench slipped off bolt head and the end of the wrench flew back and hit miner on left brow.

WORKING ON PUMP, INTAKE HOSE AND CLAMP, EE WAS CUTTING INTO CLAMP TO FIND THE AIR LEAKE. WHILE CUTTING HIS SHUT SLEAVE BECAME TANGLED IN BLADE WHICH PULLED HIS HAND INTO SAW.

HE WAS ADJUSTING TABLE #5. HE PUT THE WRENCH ON THE ADJUSTMENT BOLT BEFORE THE TABLE CAME TO A COMPLETE STOP. HIS FINGER WAS MASHED BETWEEN TABLE AND HEAD MOTION.

HE WAS DRIVING A WEDGE IN A BUCKET TOOTH WITH A HAMMER. THE WEDGE PIN BROKE AND A PIECE OF STEEL HIT HIM IN THE MOUTH.
