Employee was opening a conveyor torque shaft tube for a continuous miner when the screwdriver EE was using struck EE's left hand between the thumb and index finger.

Employee was using a prybar to remove the tail shaft from a shuttle car. When the tail shaft dislodged, the employee fell forward as the shaft dropped causing their hand to be pinched between the shaft and the mine floor.

Employee was using a chain ratchet on the slope belt take-up when the hook and load chain came loose from the anchor point and struck the employee in the left side of abdomen.

The employee was removing tie wraps from a socket set with a pocket knife, the knife slipped and the blade tip punctured left hand.

The employee was removing a broken continuous miner bit by trying to knock it out with a hammer and a punch. Unsuccessful with the hammer and punch, EE used another miner bit in an attempt to remove the broken one. As EE was striking the miner bit with EE's hammer, EE missed the bit and struck EE's left thumb.

Employee was attempting to level the impact bed on the belt discharge, using a track jack and slate bar as a handle. EE was operating the jack bar with one hand, trying to insert the leg pin with the other hand, when EE's hand slipped from the bar and the body of the bar sprang up, striking the employee behind the left ear.

Employee was rolling track rails with a metal slate bar and the bar kicked back striking ee in the R cheek area causing a laceration requiring stitches.

Employee was making a splice on a roof bolter cable and struck left thumb with a hammer. Employee went to the hospital and x-rays showed no broken bones. Employee continued to work until 7-18-2017 when they returned to the doctor for a follow up visit. No new x-rays were taken, but the employee was diagnosed with a fractured left thumb based on the initial x-rays.

While installing a new head shaft on a shuttle car, injured was using a sledge hammer to position shaft into place when a piece of metal broke off and went through injured's clothing and lodged beneath the skin in employee's lower left leg. The metal was removed by a doctor and the injured was placed off work for 7 days.

The employee was on knees pulling back on a slate bar EE was using to get slack out of miner cat pads. The bar slipped off and the end of the slate bar stuck EE in the forehead above the left eye.

Striking steel without gloves

Employee was using a knife to trim continuous miner conveyor lead, the knife slipped forward cutting the employee's hand.

Employee was changing a fitting on face shield using a socket and ratchet. The socket broke causing employee to strike elbow on the adjacent shield. The employee continued to work until April 20, 2017 at which time a physician applied a cast to immobilize the elbow to aid healing which makes this incident reportable. The employee continues to work under restrictive duties.

While pulling a shuttle car to the end of the track, a chain broke, lashing back and hitting employee on the right side of his face, cutting his eye and nose.

While hammering in anchor pins in the mine floor while anchoring the tailpiece on the 8 left section belt. His legs went numb causing back injury.

EE was making a bolt splice when his right index finger was struck with a hammer which resulted in sutures to his finger.

Employee was in the process of detrashing the 2 left belt drives, while cutting a cardboard box to put into trash bag the injured over-exerted causing the knife to slip and cut his left ring and pinky finger. The injured received sutures to close the wounds.

The employee was in process of re-installing a bucket jack pin when the jack moved pinning his right finger against front of scoop. He sustained a mashed finger requiring sutures.

Striking against another hammer, a chip came out of the hammer, penatrating his left forearm.

The employee was using a hammer and chisel when a piece of metal struck him in the head, resulting in a small piece requiring removal and sutures.

The employee was trimming the longwall belt with a utility knife when the knife slipped and he raked it across his right thigh resulting in a laceration.

The EE stated he was attempting to cut a piece of belt, when the knife slipped, causing the EE to cut his upper thigh. Required 4 sutures.

Employee was helping make a belt splice - slipped and fell on knife cutting his left hand.

The employee was disassembling a truss, a sleaving bar was inserted to pry the sections apart. When the sections seperated, the bar forced the employee's left hand into the steel structure.

While pulling together a broken belt, the employee was struck in face when chain broke.

Injured employee was holding a 2" hose with channel lock pliers. Another employee was striking a coupling with a hammer. The hammer slipped from his grasp and struck the victim in the mouth.

The injured EE was using a pry bar to break some concrete forms. The bar slipped and hit him in the mouth. The result was a broken tooth and a fractured jaw.

Employee was lifting 60" belt structure roller with belt with bar.

EE was working on 72" Conveyor drive assembly on the surface supply yard. While attempting to loosen bolts he struck his left index finger requiring sutures.

Installing bolts in structural steel at the loadout system and hit his finger with his own hammer.

While kneeling on his left knee the employee was hammering a concrete expansion bolt into a hole when he felt a "sharp" pain between his shoulders.

The employee was using a pipewrench to tighten a 3/4" diameter pipe. The pipe was overhead - the employee was pulling down - a "pop" was heard and pain was felt in the right shoulder.

THE INJURED EMPLOYEE WAS MOVING A 42" BELT, DRIVE ROLLER, WITH A SLATE BAR. THE BAR SLIPPED & THE ROLLER ROLLED BACK ONTO HIS FOOT.

Incident reported on 5/5/05, no medical treatment sought until 5/16/05. Employee was loosening a bolt with a boxed end wrench. He jerked on the bolt to loosen it - pain was felt in his RIGHT shoulder.

Employee was using a bar to close doors on empty RR cars. The bar slipped, striking him on the right side of the face.

EE struck his lt. thumb with a 3 lb. hammer while attempting to remove a broken bit from the continuous miner head.

USING CHAIN HOIST TO LOWER AIR LINER PLATES, CHAIN CAUGHT & LOOSE CHAIN HIT EE IN THE MOUTH.

THE EMPLOYEE WAS CHANGING WATER SPRAYS ON THE SHEAR DRUM. A CHIP CAME OFF OF HIS HAMMER AND LODGED IN THE TOP OF HIS RIGHT HAND.

The injured employee was assisting with the removal of a tail roller bearing. Another employee was using a hammer and chisel to remove the outer bearing race. A piece of the back chipped off and struck the victim in the hand. The employee did not start missing work until 6-30-04.

The injured ee was tightening bolts on a belt discharge roller bearing, his ratchet slipped causing him to strike his chin on the discharge roller, his upper teeth bit through his lower lip.

EE WAS CUTTING NYLON BAND ON CINDER BLOCKS WHEN HIS KNIFE (UTILITY TYPE) SLIPPED & CUT HIS LEFT LEG.

EMPLOYEE WAS CUTTING A PIECE OF RUBBER CONDUIT WITH A UTILITY KNIFE. THE KNIFE SLIPPED AND CUT HIS LEFT LEG.

EE WAS USING A SET OF CHANNEL LOCKS TO BREAK A METAL BAND ON A PALLET OF ROOF BOLTS THE BAND FLOPPED UP WHEN BROKEN & CUT THE TOP OF HIS LEFT HAND. RESULT LACERATION 6 STITCHES.

INJURED WAS USING A CHAIN HOIST TO REMOVE A ROOF BOLT FROM THE BELT STRUCTURE. THE CHAIN SLIPPED AND HE CAUGHT HIS FINGER BETWEEN THE BOLT AND BELT STRUCTURE.

EE WAS USING A SLEDGE HAMMER TO PUT CONVEYOR CHAIN TOGETHER, A PIECE OF METAL SLIPPED FROM CHAINAND WENT INTO ARM.

CUTTING CABLE AND KNIFE SLIPPED OFF THE SIDE AND CUT PALM OF LEFT HAND.

CUTTING EXCESS ZIPTIE AND KNIFE SLIPPED CUTTING RT. 2ND FINGER. DUE TO NO LOST DAYS EE REPORTED THIS INJURY WHEN EE CONFIRMED THAT HE HAD STITCHES. WHICH WAS 5-30-02.

CUT FIRST FINGER ON LEFT HAND WHILE SPLICING CABLE. REPORT IS LATE DUE TO EMPLOYER JUST NOW BEING MADE AWARE OF MEDICAL TREATMENT.

PULLING BELT TOGETHER TO MAKE SPLICE AND ROPE BROKE, LETTING CLAMP HIT RIGHT HAND.

THE INJURED WAS USING A UTILITY KNIFE TO CUT A 54" BELT. WHILE PULLING THE KNIFE TOWARD HIMSELF, IT SLIPPED AND STRUCK HIM IN THE LEFT THIGH RESULTING IN A LACERATION REQUIRING 6 SUTURES.
