A Foreman had connected a chain ratchet to the #2 belt conveyor and a previously installed pillar rib bolt in order to pull the belt over in the track entry for clearance. After pulling the slack out of the ratchet, a piece of rock that the rib bolt was anchored in came loose and forced EE into the belt conveyor resulting in a broken right femur.

A group of employees was attempting to remove a broken continuous miner conveyor torque shaft with a chain ratchet. The hook of the chain ratchet came loose from the torque shaft striking an employee on the left hand middle knuckle. The strike resulted in a fractured knuckle.

The employee was scaling draw rock in the #5 entry. When the piece of draw rock that EE was pulling came loose, the forward momentum caused the end of the bar to come down on left thigh resulting in a cut that required stitches.

Employee was helping set a belt drive. He was trying to remove a jack from under the drive and while moving it back and forth, the jack bar came out and struck him in the right jaw, causing a contusion and chipping a tooth.

Employee was assisting another employee remove a rock from underneath the roof bolter. The other employee was using a bar to strike the rock and the bar glanced off and hit the employee in the hand causing a gouge wound requiring 3 stitches. Gloves were being worn at the time.

The employee was helping make a belt splice. While attempting to remove the belt clamps and rails, he received an avulsion to the end of his left hand index finger when it became pinched between the belt clamp rail and the belt clamp.

Employee was using chain hoist to pull belt together and the handle of the chain hoist flipped around and hit him in the head. He required 3 stitches to close the laceration.

Employee was trying to cut a hole in a piece of belt near the air lock doors when the knife slipped and hit him on the left hand at the heel of the thumb causing a laceration that took 6 sutures to close.

Employee was cutting a hole in a piece of belt when the knife slipped and hit him on the right leg two inches above the knee. He was going to use the piece of belt to drag rock dust to a trickle duster a few breaks away. Three sutures were used to close the laceration.

Employee jacked up a three wheel buggy and took the tire off to check for a noise he heard. He had two jacks plus some blocking under the buggy when it killed over and hit him on the left leg in the shin area causing a contusion.

Employee was trimming a belt splice, when the knife slipped and hit him on the inside of the left knee, causing a laceration that took 6 sutures to close.

Employee was cutting a belt with a razor type knife when it slipped and hit him on the back of the left thumb causing a laceration that required 4 sutures. Employee did not miss work.

Employee was hitting a metal shaft with a hammer when a piece of metal flew off and hit him in the mouth knocking his upper front tooth out. Employee has continued to work and has missed no days.

Employee was in the process of making a belt splice. He was holding the belt up with his hand and the knife cut through the belt and cut his little finger on his left hand.
