Two employees were bolting in a narrow belt line area when the bolter tripped the e-stop cable. While discussing reset options, one employee attempted to squeeze through a tight area. While doing so, the bolter shifted, pinning them between the bolter and rib. They called for help, and the other employee moved the bolter to free them.

Large piece of salt got stuck in head chute. Employee was jack hammering the salt piece loose, when the salt piece came loose the employee slipped causing their left hand ring finger to get pinched in between the chute and jack hammer handle.

Employee was using a drill press to drill out a hole larger in a bearing housing. The bearing housing was in a vice on the drill press table. The bit caught in the hole and pulled the housing out of the vice, striking the employees thumb causing a laceration, resulting in 4 stitches.

Employee in hi-lift, booming down was struck in the back of the neck by 3/4" socket that fell from above.

Employee was driving a bolt with an impact wrench. Brake clean solvent was in the hole and when compressed, combusted and blew the bolt out of the hole. This drove the impact upwards. The employees left hand was on top of the gun. EE's left index finger was mashed in between the gun and the equipment breaking it, requiring surgery.

Core hole drilling was taking place at the unit 10 drill site. While an employee was in the process of reattaching the quill rod to the drill string a piece of core rock unexpectedly exited the end of the drill steel and ricocheted off of the drill structure contacting the employee in the left side of the neck causing a deep laceration that required treatment.

EE was removing a drill rod from a drill. EE was holding the drill rod on the very end instead of the middle. When the threads finished the rod fell to the ground jolting EE's left elbow. This inflamed tendonitis in EE's elbow. EE was shown stretches and released to full duty until the injection on 6-13-17.

As the bolter wrench was tightening the bolt in the hole, the bolt was a spinner and did not properly take in the hole. As the wrench was lowered, the bolt came almost all the way out of the hole. When employee reached up to pull the bolt out of the roof by hand (with gloves), it fell out of the roof and pinched the employee's left ring finger. Employee received 3 stitches.

Employee was cleaning a plugged cyclone chute in Screen Plant. Employee reached into an open access hatch and finger got caught in Rotary Air Lock Valve. Tip of Right middle finger was amputated.

An employee was removing a sling from a fork while standing adjacent to a undercutter bar. The outrigger for the material handler that was holding the bar on a set of blocks was raised. The bar slid off of the blocks landing on the left foot of the employee.

Employee was tramming 106 bolter from the basket when the steering controls froze while tramming south. The hydraulic spool that controls this steering action jammed casuing the failure of the system. The employee's back struck the side of the basket when the steering cylinder reached the stops. The employee was treated and written a prescription for a muscle relaxer.

Employee was holding two boards (for alignment purposes) to be nailed together with air nailer. Hands were approximately 18" from point to be nailed. Air nailer double shot and the second nail struck employee in the left index finger between the hand and first joint in finger. Nail was removed from finger by the doctor and employee returned to work. Nail did not contact bone.

Miner was sharpening bits when the bit and tool slid off the rest. Miner's hand moved forward and struck the rotating grinding wheel, causing laceration.

A maintenance mechanic was doing miscellaneous cleaning in the surface maintenance shop. He attempted to dispose of an old chlorine gas cylinder (over 15 years old). He checked cylinder pressure by turning top stem valve. He then took the cylinder to the band saw inside maintenance shop to cut in half for disposal. When he cut into the cylinder chlorine gas was released.

Roof bolter was installing his second roof bolt. Employee then inserted the roof bolt into the hole and was holding the bolt in position to bring up the drill motor carriage to accept the roof bolt. He inadvertently grabbed the wrong handle, which activated the drill steel centralizing mechanism. Centralizer clamps closed on his left index finger.

While changing a bit, operator was struck by a rotating bit-wrench resulting in: broken rib and bones of the face; damage to eyeball; lacerations, contusions, abrastions to ribcage and face; sprain/strain of left arm/hand.

Employee was cutting with a chop saw and a shard of metal struck his right eye. Wearing PPE but face shield and hat combination was ill fitting. Shard bounced off inside of face shield, past his safety glasses and into his eye. Shard was found by physician on worker's eye lashes. Two days later a second trip to doctor revealed the need for prescription for slight eye infection.

Employee was training another employee to operate M-36 Fletcher Scaler. Trainee was in the operator's compartment; trainer was standing on the platform. Trainer reached into the operator's compartment to reduce throttle. He contacted the cab rotation control, causing the cab to rotate clockwise, striking him in the right leg, spraining his right ankle.

Employee was tramming M-88 Scaler from face back to UG Main Shop. While driving employee encountered a low roof outside of our Unit 40-B Entrance way. He struck the low roof area with the canopy of the Scaler, resulting in one of the support bars on the canopy breaking loose and striking him in the face. The employee recieved 5 stiches as a result.

Employee received shoulder strain while pulling a hose connected to a hose reel. Hose is used on air impact wrenches. Immediate cause was stress due to improper speed, and body position while removing the hose. He saw a physician on the day of the accident. No medical treatment; evaluation only. He was placed on restricted duty 10/28/04 after seeing his physician again.

EE WAS IN MAN BASKET ATTACHING J BOLTS ON ROOF OF MINE. OPERATOR ATTEMPTED TO TRAM TO NEW LOCATION WITH BASKET ELEVATED. EQUIPMENT TILTED CAUSING EE TO FALL AGAINST HANDRAILING.

EE WAS TRYING TO DRILL A HOLE IN A PIECE OF PIPE WITH A 1/2" CHUCK ELECTRIC DRILL. HE HELD THE PIPE WITH HIS LEFT HAND AND AS HE WAS DRILLING, THE BIT CAUGHT CAUSING THE DRILL TO RAPIDLY TWIST HIS RIGHT HAND. WHICH BROKE A BONE IN HIS RIGHT HAND.

EE OPERATING MOBILE CRANE FELT PAIN IN LOWER ABOMEN AREA WHEN APPLYING BRAKES. DIAGNOSED WITH SMALL HERNIA. DECIDED TO DELAY ANY SURGERY. ON 1-15-04 HAD LAPSOPIC SURGERY AND ALLOWED TO RETURN TO WORK WITH RESTRICTIONS.

WHILE ROOF BOLTING THE DRILL STEEL BROKE LEAVING A PORTION IN THE ROOF. THE EMPLOYEE REACHED TO MOVE THE STEEL GUIDE OUT OF THE WAY TO BETTER REMOVE THE STEEL FROM THE ROOF. JUST AFTER THIS IT FELL STRIKING THE BACK OF HIS LEFT HAND, FORCING HIS HAND DOWNWARD TO THE ROOF BOLTER HEAD, PINNING HIS HAND BETWEEN THE END OF THE DRILL STEEL AND THE BOLTER HEAD.

LEAKING COUPLING APPEARED TO HAVE COME LOOSE ALLOWING GAS TO LEAK. TORCH WAS NOT PROPERLY TIGHT TO PERFORM THE TASK. LACK OF SKILL RESULTED IN INADEQUATE PRACTICE OF CHECKING ALL FITTINGS BEF ORE STARTING WORK.

EMPLOYEE WAS CLEANING UNDER 023 CONVEYOR TAKE UP USING A JACK HAMMER. THE EMPLOYEE WAS STANDING ON A PLATFORM TO REMOVE SALT BUILDUP. PIECE OF SALT SLID AND STRUCK HIM IN THE RIGHT ARM.

EE WAS ON BERM ROAD IN HIS PERSONAL VEHICLE TRAVELING TOWARD THE PARKING LOT WHILE A 631 SCRAPER OPERATOR WAS LEAVING THE PARKING LOT. IT WAS 7:05 A.M., JUST BEFORE SUNRISE. BOTH VEHICLES HAD THEIR LIGHTS ON. THE SCRAPER OPERATOR REACHED UP TO TURN HIS WINDSHIELD WIPERS ON (OVERHEAD DASH). WHEN HE LOOKED BACK DOWN THE PAN STRUCK THE LEFT REAR PANEL OF THE TRUCK.

GOING TOO FAST DOWN RAMP, HIT HOLE IN ROAD.

WRENCH HAD BEEN LAID UP ON ANGLE IRON WHILE MECHANICS INSTALLED THE BOLTS. NORMAL VIBRATION APPEARS TO HAVE BEEN ENOUGH TO SLOWLY CAUSE WRENCH TO FALL APPROX. 2'.

DURING A BELT SPLICE AN EMPLOYEE WAS LACING THE MACHINE ON TOP OF BELT WHILE ANOTHER WAS ADVANCING THE MACHINE AND RAN OUT OF ROOM ABOUT HALFWAYACROSS. VICTIM CLIMBED ON BOARD, GRABBED FRAME O F SPLICE RIG, REACHED OVER WITH HIS RIGHT HAND, AND PLACED IT ON TOP OF FRAME AS LACING WAS PROGRESSING. VICTIM'S THUMB WAS IN THE WAY AND THE TIP GOT SEVERED.

AN EMPLOYEE WAS USING AN IMPACT GUN ATTEMPTING TO SCREW A 1" BOLT ONTO A 1" STUD THAT APPARENTLYHAD SOME DAMAGED THREADS. THE NUT SPUN OFF STUD AND FLEW IN AIR ~2-3' AWAY, HITTING INJURED EMPL OYEE IN MOUTH CAUSING A FRACTURED TOOTH.

AFTER INSTALLING THE SUCTION NIPPLE ON THE NEW SUCTION PLATE TO THE PUMP, EE BENT OVER WITH IMPACT WRENCH TO TIGHTEN BOLTS AN EXPERIENCED PAIN IN HIS LOWER BACK. EE REPORTED INCIDENT ON 11/26 /01.

NOT REALLY TOO SURE, BUT OPERATOR OF HEAVY EQUIP. MAY HAVE BUMPED HIM TOO HARD BUT THINKS HE HIT A BUMP IN HAUL ROAD TOO HARD, THEN WAS AGGRAVATED BY CONTACT WITH EQUIPMENT.

EMPLOYEE WAS WELDING IN A TIGHT AREA AT THE MINI-STAMLER IN THE SCREEN PLANT. MOVING INTO POSITION TO RESUME WELDING, THE WELDING ROD CONTACTED THE PLATE AND ARCED, WHILE HELMET WAS IN THE RAI SED POSITION. THIS HAPPENED A FEW TIMES RESULTING IN FLASH BURN TO THE EYES. EMPLOYEE WAS TREATED THE NEXT DAY & RELEASED.

UPON DRILLING PRELIMINARY HOLE DEPTH, DRILL HIT VOID WHEN DRILLING UP. THIS CAUSED DRILL TO HIT ROOF CAUSING A SMALL SALT SLAB TO CRACK AND FALL ON LEFT INDEX FINGER, APPARENTLY CAUSING A LACE RATION.

EMPLOYEE WAS OPERATING A FLETCHER ROOF BOLTER. WHILE MOVING BOOM TO DRILL ANOTHER BOLT HOLE A SPARE STEEL WHICH WAS STORED IN A TRAY ON THE BOOM SOMEHOW BOUND UP AND SPRUNG LOOSE STRIKING EMPL OYEE IN THE RIGHT EAR AND NECK AREA. SEVERAL STICHES IN RIGHT EAR.

MAINTENANCE MECHANIC WAS BURNING BOLTS OFF PIPE FITTING. WHEN HE MOVED HIS HAND BEHIND THE FITTING, HAND CAME IN CONTACT WITH TORCH FLAME.

EE WAS REPLACING GRATING ON A FLOATING SEALING WATER PUMP. THE HOSE WAS STRETCHED TAUT, HAVING CAUGHT ON EDGE OF SLED AND AS EE TURNED HIS BODYTO MAKE A CUT TO TRIM GRATING, THE MOVEMENT OF H IS BODY CAUSED THE TORCH TO PASS OVER HIS LEFT HAND RATHER THAN THE CUT HE INTENDED TOMAKE. THIS RESULTED IN 1ST & 2ND DEG BURNS TO APPROX 25-30% OF HIS HAND FRONT & BACK INCLUDING LITTLE, RI

EE SUFFERED BROKEN LEFT HAND WHEN HE INADVERTENTLY HIT THE WRONG LEVER ON HIS ROOFBOLTER(CANNON). THE STEEL GUIDE CLAMP CLOSED ON EMPLOYEE'S HAND.

BACKED DOWN SLOPE IN 3RD GEAR & HIT BOTTOM TOO HARD; HIT HIS TAILBONE ON SEAT

EMPLOYEE WAS BEING TRAINING ON A NEW BELT SPLICE MACHINE. EMPLOYEE WAS PULLIN GDOWN UP ON SPLICE MACHINE LEVER WHEN SHARP PAIN WAS FELT IN SHOULDER.

OPERATOR WAS FOLLOWING THE ROTATING TABLE OF THE STRETCHWRAP MACHINE TO MAKE SURE WRAPPING FILM STAYED ON THE PALLET. AS HE WAS RETURNING TO THECONTROL PANEL SIDE HE CAUGHT HIS FOOT IN THE RUB BER MAT AT THE OPERATOR'S STATION. THE MAT HAD A BUCKLE IN IT. THE TABLE CAME AROUND, STRIKING THE OPERATOR'S LEG. HE GRABBED THE PALLET AND ITTHREW HIM TO THE GROUND.

AFTER INSTALLING A ROOF BOLT THE OPERATOR, MOVED THE KITCHEN TO POSITION FOR NEXT BOLT WHEN A PIECE OF SALT 3'X1'X8" FELL THROUGH THE HOLE IN THE CANOPY AND DEFLECTED TOWARDS THE EE, STRIKING HIM ON THE LEFT THUMB.

WHILE CLEANING SCALE OFF OF CONTACT POINTS FOR NEW LOAD CELLS INSTALLATION USING A 41/2" GRINDERGRINDER KICKED & STRUCK EE'S LEFT INDEX FINGER EE WAS LAYING DOWN TO BRING UNDER LOADING POCKET PEDESTAL & GRINDING WITH ONE HANDLACKING TOTAL CONTROL OF GRINDER

EE WORKING IN SHOP. IT IS BELIEVED THAT WHEN LOCAL VENTILATION FAN MOVED, MOVE CAUSED SALT DUST TO BLOW INTO EE'S EYES.

WHILE PREPARING TO USE THE BOOM ON THE BOOM TRUCK THE LEFT HAND WAS POSTIONED ON TOP OF THE STABILIZER CYLINDER. HIS RIGHT HAND ACTIVATED THE BOOM CONTROLS. WHEN THE BOOM STARTED TO MOVE THE B OOM FRAME (BASE) SHIFTED. PINCHING THE INDEX FINGER OF THE LEFT HAND BETWEEN THE TRUCK BED AND THE STABILIZER CYLINDER.

EE WAS RETRIEVING CUTTERS FROM BACK OF POWDER RIG. DRIVER OF THE RIG MOVED THE RIG. THE BACK OF THE TRUCK BOTTOMED OUT ON THE EE'S LEFT FOOT.

EE WAS SHARPENING BOLTER BITS AT A GRINDING STATION. HE WAS ATTEMPTING TO REMOVE A BIT THAT FIT SNUGLY IN THE BITHOLDER WHEN IT SUDDENLY CAME LOOSE AND HIS RIGHT THUMB CONTACTED THE GRINDING W HEEL. THIS GRINDER IS A SEMI-AUTOMATIC UNIT THAT WAS MODIFIED 4-6 WEEKS EARLIER TO ACCOMODATE THE TYPE OF ROOFBOLTER BITS CURRENTLY IN USE. NO CHANGES WERE MADE IN GUARDING DUE TO LIMITED USE

EE AND HIS PARTNER WEREPWDERING THE LOWER HOLES IN 48 ST FACE WITH THE POWDER RIG BACKET GAT UP. HIS PARTNER MOVE THE BASKET CAUSING THE GATE TO FALL. THE LATCH ON THE GATE PINCHED EE'S LEFT H AND, CAUSING THE LACERATION.
