9 Dragline crew along with 2 Midstate employees were removing lugs on the drag drum to swap ends with cables. Nuts on the lugs had been removed on cab side cable and Midstate employee was using a bar to pry lug loose from drum. Dragline Operator was in drag pit and had his hand on cable. The lug came loose cable pinched operator's finger between cable and drum.

Float Crew leader-man was installing bolts in flange of a 42" HPDE pipe when the pipe shifted pinching his fingers between the pipe flange and bolt. Pipe was being held by a WA500 loader to align the flange ends. Employee was wearing leather gloves at the time. Employee received two small lacerations to his middle and index fingers.

While digging clays with the 14 D/L in the S1 clay dam the operator was dumping a bucket when a clay ball came back towards the operator cab. The clay ball shattered the window cutting the employees arms.

Operator was leaving the control room headed to ground floor. He did not have proper footing on last step, causing him to slip. He then grabbed the handrail with his left hand and felt a burning sensation in his left bicep.

Equip. Oper. hit a rut, causing tractor to bounce, jarring operator. He reported discomfort in neck, seen by DR 6/4 sent for Xray, no fractures found. Oper. returned next scheduled shift w/no restrictions. Later seen for pain complaint, placed on work restriction w/RX. Initially reported 5/31,became reportable on 6/12 w/RX & restrictions given.

While attempting to secure a nut on a bolt on the bottom of the BB float cell, the employee fell from ladder when he lost his balance. The employee was standing on the second step of a 4 foot step ladder on the ground floor on the East side of the Float Plant. He fell to the ground, striking his left side (rib area), on the water hose of the nearby water cannon.

Employee was manually priming a sealing water pump. The employee shut down the pump and added water by opening the valve. He then attempted to prime the pump without success. The water he added was converted to boiling hot water and steam when it came in contact with the hot sealing water pump, spewing out and striking him in the arm.

Dragline Oiler was moving a power cable with a hot stick when he felt a muscle pull in his left arm.First Aid was administered and reported.He was brought into clinic for evaluation on 09/11/12.Medical staff ordered an MRI for suspected partial biceps tendon tear on 09/12/12.Bicep tear was confirmed by Physician on 09/24/12.

The employee was in the process of replacing the radio for the Clay system. He had opened the radio box on the P2B Dam - North side when lightning struck in the vicinity of the employee. The employee fell to his knees.

GRINDING DISC BEING USED BY ee SHATTERED CAUSING PIECES OF DISC TO FLY BACK HITTING ee. PIECE OF DISC BECAME LODGED IN ee RIGHT SIDE JUST BELOW RIBCAGE.

Operator was leaving the restroom at the washer. When he opened the door, he hit his foot on the door while his body was still moving forward. Subsequently, he hit his front tooth on the corner of the door, knocking it out.

EE had been working in the plant all day. Supervisor asked how he was doing. He stated he had heat exhaustion & felt nauseated. He was offered fluids & opportunity to rest. After about 15-20 minutes he reported that he felt better. After he left the property for the day he began feeling ill again & went to the ER. He was given IV fluids at the ER & released back to work.

While reinstalling #1 swing generator guard on dragline, employee pinched his right hand second finger. Previously recorded as an first aid, after being placed on restricted duty, prescribed medication and follow up with physician it becomes a recordable injury on 02/14/2012.

Two employee's were unloading an oxygen bottle "+70 lbs" from a trailer to the drag line parts trailer. They were carrying the bottle horizontally and as they started lowering it into position the employee that was holding the bottom end felt sharp pain in his right arm on the inside just above his elbow.

HEART ATTACK Employee reported to work and was going up stairs to clock in, 2 employees witnessed him collapse on landing. CPR was immediately administered without success. HEART ATTACK

Employee was descending staircase from "B" circuit conditioner lever to main float plant floor and struck top of head on beam across walkway. Employee became dizzy and required assistance to get to the break room. Employee was wearing hard hat.

Employee was trying to attach a tractor cable hook to a gun sled. While twisting the hook to line it up with the pad eye on the gun sled the hook slipped and twisted back pinching employee's right pinky finger between the hook and the gun sled resulting in laceration to finger.

Employee was removing bolts from discharge pipe. He was pulling upwards on a bolt with a 24" ratchet. While pulling up he felt a "small pain" in his right shoulder. Originally treated conservatively as a minor first aid, became reportable on 8/12/2011 when physician placed employee on restricted duty.

EE was running the pit on Wed night shift. As he was standing with the operators chair arm hooked into the wiper water line. When he sat down the chair came loose. He caught himself with his left arm. He reported he was feeling pain in his left shoulder. Initially reported as a first aid incident only, became reportable on 5/29/2011 when restricted duty was indicated by doctor.

Employee was removing a roller assembly from under conveyor belt and during the process lost grip with right hand resulting in roller falling, pinching left index finger between frame and support structure.

While cleaning a piece of equipment employee felt sudden pain in right shoulder. Stopped work and went to see nurse. Originally reported as First Aid on 5/19 became recordable on 5/24 when Medical Staff prescribed medication and restricted duty.

Employee twisted and stepped down exiting tractor. He felt a "pop" and then pain in the back of his left knee during this movement. Employee notified supervision and was taken to nurses office for evaluation. Originally treated as a minor first aid, incident became recordable on 4/26 when restricted duty and further treatment was prescribed by Physician.

Emp. was approaching ditch to start pump. Ground was very slick. He proceeded to walk to the pump and slipped and fell, landing on a rock. EE complained of dull pain in lower back-left side. Originally treated as a minor first aid on 2/12, after further diagnostic testing and medical treatment, became a recordable on 3/2 when Physician prescribed RX meds and restricted duty.

While boring out a hole in a basket hoist for the clevis, the drill bit hung up causing employee to twist right shoulder. Originally treated conservatively as a minor first aid, became a recordable on 02/19/10 after further diagnostic testing and consultation with the medical staff indicated a need for more treatment.

Employee stepped on uneven ground and overextended knee, later in the same day employee was using shovel and felt sharp pain in right knee. Treated as a minor first aid event on the day of accident(11/18), no improvement was noted Physician. Became a reportable incident on 12/11 when restricted duty was initiated by the Dr., based on diagnostic test results.

Inattention to the possible hazards present when using a pry bar around sharp metal edges. The hand protection the injured employee was wearing was inadequate for the task being performed.

Employee was on his way to the pit in a company vehicle when he struck a power pole. Initially treated as a First Aid incident on 10/16. When no improvement was noted by medical staff, employee was placed on restricted duty on 10/30.

Employee was returning to vehicle after replacing board at spillway. In route to the vehicle employee lost his footing in a wash out and fell landing on his left shoulder. Incident became recordable after medical testing and review by physician indicated a need for restricted duty.

Employee strained a muscle in his back while storing the outrigger footpad for a field crane. Reported incident immediately and followed up with doctor on 08/17. Placed on restricted duty at that time.

Worker in dump truck dropped his cell phone into A/C slot. He removed panel to retreive it and got his fingertips nipped by a blower fan.

Maintenence mechanic was moving hub in shop to make room for another. Hub shifted and pinched left hand breaking middle knuckle.

The top portion of the A side of the plant washer caught on fire approximately 2 hours after hotwork (welding/cutting) had completed in that area. The fire is believed to have been caused by a piece of slag or burned metal that came in contact with unknown combustible material which may have smoldered and eventually caught fire. A detailed investigation is ongoing.

Employee was attempting to make up a flange with flexible pipe on the matrix pump. He had not completed threading all of the nuts on the bolts when the potential energy stored in the flexible pipe pulled the nuts off of the flange and the flange kicked up, striking him in the mouth.

Employee was descending stairs to spillway when riser pulled away from support post. This caused stairs to tilt suddenly resulting in employee losing his footing and falling backwards, landing on his hands and wrists.

During the course of normal job duties, employee stepped down from pump and strained his left knee. Contacted nurse who instructed employee to put ice pack on knee and monitor progress as day goes along. Initially treated as minor first aid and became recordable on 6/10/2009 when physician assigned light duty and scheduled surgery.

Employee was cutting duct type off a guard rail to replace the cable, the knife being recently sharpened was sharper than the employee figured, and when he cut the tape the knife cut the first knuckle of the left index finger. Employee received stitches in finger. There was no lost time involved with this incident.

Person using a pry bar to release crate packing from part. Pry bar slipped out of hands, flew up and hit him in the forearm.

Operator got over heated in work boat while moving dredge in pit. Reportable due to the fact that an IV was administered at the hospital.

The incident was heat related.Was a hot day and he had been in the sun for about an hour, became disoriented and was assisted to an air conditioned area and rehydrated.He was taken to the hospital where he was examined.He had been drinking several sodas - the sugar and cafeene could have been contributing factors.Returned to work.

Two mechanics were tightening the bolts on the 14 pit pile gun. One was tightening the nut and the injured employee was holding the bolt head. The wrench slipped, causing injured employees wrench to lunge forward, pinching finger between flange and wrench.

While dismounting dragline cabside shoe, fingers were pinched between framework and cable stop plate.

Employee rolled ankle while offloading dredge resulting in moderate sprain. Accident is not attributed to any factor in the work environment and the employee did not miss any scheduled work days.

Mechanic was installing channel in the "A" Finishing Screen Bucker-Up frame. He was holding the channel with his Left hand, while hammering it into position for welding with his right hand. He missed the target and hit the wall adjacent to the piece of channel. The deflected hammer struck the employees left thumb.

While attempting to un-hook a 35 ton crane head-ache ball (100lb) which was connected to the rail load-out conveyor belt guard, the guard was just lowered from an upper level. When the mechanic attempted to un-hook the ball it shifted pinching hand between the ball and guard. Treated as 1st aid until placed on restricted duty 12/10/07

Employee was working to clear a drain line on the clay tank at the washer; a pipe nipple was removed from the discharge side of the valve. The urethane stuck in the valve was removed; the valve was closed. In the process of closing the valve the employee stepped into the washout caused by water washing out loose material under the valve.

while replacing pipe in matrix line, Employee used a nut with a bolt to spread a pipe to install gasket as the pipe was aligned. The nut was not threaded on the bolt completely so it deformed to a wedge shape. The wedge shape nut dislodged from the pipe as final adjustment to the pipe was made. The bolt and nut struck the employee on the left shin.

Employee was working on MG sets on #3 dragline. He stepped on a loose 1.25" diameter pipe on a walkway between sets, which rolled causing employee to strain his knee. Employee was placed on restricted duty on 7/27/07.

Employee was cutting an I-beam into pieces and the last piece cut fell from above, striking him on right hand and foot. Employee placed on restricted duty 7/6/07.

The employee was walking from investigating a location for a crane setup, and slipped on water and clay next to cleanup ditch. He stopped his fall with his right arm causing strained ligaments. This shoulder had been repaired 6 months ago.

The employee was attempting to relocate a pump power cable out of an elevated mound of sand. The power cable was buried approximately 6" and the area was covered with clay and mud from the pit. His left foot caught in the mud while he was falling he strained a ligament in his knee and a slightly sprained his ankle.

While welding a light bracket on back of tractor, welding rod stuck. While breaking rod loose, rod stub struck back of left hand. Employee was given prescription medication and placed on restricted duty 4/30/07.

Walking on uneven terrain. (twisted ankle)

Operator was in work boat when the boat moved he lost balance and fell, when he landed his left hand landed on the corner of the welding rod can, cutting the outside of hand on little finger side. Reportable only because the operator received 4 stitches and then returned to work. No Lost time.

Fell and twisted knee on 4/12/07. Received first aid on 4/13/07. On 4/18/08 complained to company physician and x-rays were ordered. On 6/2/08 employee was prescribed physical therapy.

Employee was pulling a wrench to tighten and it slipped. When it slipped, it mashed his right little finger. (fifth digit finger) It split open. Stitches were needed to repair finger.

While trimming interlock tab off bottom of CP Screen with utility knife, knife slipped off interlock causing laceration to thumb of left hand.

Employee was walking in front of maintenance shop. As he attempted to step on the sidewalk he slipped on mud & fell to the ground twisting his knee. He was placed on restricted duty 11/7/06.

EE and co-worker in process of drilling 4 bolt- holes in linetex cyclone apex. Injured EE was holding the part whith his hands to secure part while drilling. The drill wedged in the hole causing part tto spin clockwise and the EE's securing the part injured his left hand causing a laceration that required 4 sutures to close.

While operating dozer on pile, EE backed over rut that caused a jolt to his neck. Looking back over shoulder for long periods backing up. Jolt exacerbated condition of previous incident along with personal factors involved. Investigation of incident nd medical findings of 7/26/06 indicated acute musculigimental strain. Reported as such 7/26/06.

EMPLOYEE WAS ATTEMPTING TO CUT A U-BOLT USED TO SECURE DRAGLINE CABLE ON ITS SPOOL. CABLE TRAVELED BETWEEN END OF BLADE & SIDE OF SPOOL STRIKING OPERATOR ON THE FRONT OF HIS HARD HAT, CAUSING A 2"LACERATION TO HIS FOREHEAD.

While attempting to tighten an energized 440 volt starter, connection made contact with a ground within a starter cubicle chassis. Starter was not locked out or otherwise secured. Employee was wearing an electrical flash shirt which greatly reduced potential injuries.

EE was in the process of replacing 20 HP motor, weighing approx. 250 lbs, on a pivot base. While moving the base to put guard on, left thumb was caught between the base and the guard resulting in avulsion/amputation of distal tip of left thumb.

EE was mounting backhoe when back foot slipped on pile of material, causing ee to overextend pulling muscle in groin. poor ground conditon and rain where backhoe was parked contributed to accident. in attempt to save time ee used poor judgement on stabliity of step he was using to mount backhoe. ee placed on light duty 7/19/05.

Employee has been a maintenance mechanic for 33 years. He has had varied events of pain off and on for several years in one or both hands. Seen by personal physician recently and diagnosed with carpal tunnel syndrome attributed to many years as a mechanic. Reported diagnosis to company on 5-24-05 - surgery indicated; will be done in the near future.

Claimant was hauling - took curve too fast & rolled over causing windows to break.

HOUSEKEEPING AND INADEQUATE WORK STANDARDS. PRODUCT SPILLAGE FROM SEVERAL LEAKS CREATED CONDITIONS ON FLOOR COVERING THE ANCHORED HOSE WITH A SOFT MUD PARTIALLY OBSCURING IT FROM VISION, CAUSING A TRIP HAZARD

EMPLOYEE WAS LAYING ON HIS SIDE BURNING WITH A TORCH. HAD A BLOWBACK AND HOT SLAG HIT WELDING SHIELD AND DROPPED IN LEFT EAR. TREATED AS FIRST AID ON ACCIDENT DATE BY IN HOUSE PHYSICIAN. DUE TO ADDITIONAL COMPLAINTS, EE WAS PLACED ON ANTIBIOTICS 1/10/2005

EMPLOYEE INJURED LEFT SHOULDER AS HE FELL OVER CONTROL CABLES ON PUMP SLED. CORDS ARE CONNECTED TO MOTOR SWITCH GEAR ON SEPARATE SLED AND CONTROL CAR ON ANOTHER SLED. CABLES ARE DISCONNECTED AND ROLLED UP EACH TIME PUMP IS MOVED. EMPLOYEE WAS TREATED AS FIRST AID UNTIL PLACED ON RESTRICTED DUTY ON 1/31/05.

EE was holding an adjustable jaw wrench on a bolt head while another ee was attempting to remove the nut with an impact wrench. Wrench slipped causing ee's hand to strike pipe. Wrench handle pinched small finger, requiring two stitches.

Employee received shoulder strain while pulling a hose connected to a hose reel. Hose is used on air impact wrenches. Immediate cause was stress due to improper speed, and body position while removing the hose. He saw a physician on the day of the accident. No medical treatment; evaluation only. He was placed on restricted duty 10/28/04 after seeing his physician again.

EE was using a line truck to disconnect power line after storm. After cutting 2nd wire, pole broke striking ee across back, shoulders and head, knocking off hard hat. He received laceration to scalp and multiple insect bites.

POOR HOUSEKEEPING - AREA HAD BEEN TREATED FOR ALGAE GROWTH. IMPROPER PLACEMENT OF A PIPE THAT RESULTED IN CHANGING THE ROUTE TO A SUMP PUMP. PIPE CAUSED EMPLOYEE TO WALK ACROSS A SLOPED CONCRETE AREA LEADING TO A SUMP TO CHECK OIL LEVELS IN A PUMP.

Employee exerted force on a pry bar to install a 20" 90 degree fitting. injury was aggravated when he used a shovel as a normal part of his job, resulting in sprain of ligaments in elbow.

EMPLOYEE WAS STEPPING DOWN INTO DRAG CABLE BOX, MISSED SECOND STEP AND FELL, LANDING ON RT. ANKLE.

ELECTRICAL SUPERVISOR AND ELECTRICIAN WERE CUTTING A WIRE TIE OFF A SET OF BUNDLED WIRES WHEN KNIFE SLIPPED OFF THE WIRE TIE AND CUT ELECTRICIAN'S HAND.

EMPLOYEE WAS USING A POCKET KNIFE THAT DID NOT HAVE A LOCKING BLADE TO TRIM ON A URETHANE WEDGE GASKET USED IN SEALING 20"PIPE FLANGES. BLADE CLOSED ON FINGER, CAUSING THE CUT (REQUIRING SUTURES).

EMPLOYEE STEPPED DOWN FROM A PUMP SLED IN THE FIELD ON TO SAND IN MINING AREA. HE TURNED TO MOVE AND HIS FEET DID NOT MOVE WITH HIS BODY RESULTING IN A TWISTING FORCE TORIGHT KNEE.

DURING THE PROCESS OF SIGNALING TO GUIDE ASSEMBLY INTO PLACE ON PLATFORM, HE PLACED HIS LEFT HAND BETWEEN A STATIONARY I-BEAM AND THE BOLTS EXTRUDING FROM THE LOCK RETAINER, CREATING A PINCH P OINT WHICH RESULTED IN INJURY TO LEFT HAND.

EMPLOYEE WAS PUTTING GAS IN FIVEHORSE MOTOR WHILE THE ENGINE WAS HOT. (SERVICE TRUCK)

EE WAS CROSSING OVER 42" CLAY LINE AND SLIPPED, HITTING INSIDE OF RIGHT KNEE OVER NEXT SEVERAL DAYS EE LEG WAS STIFF AND SORE, BUT IMPROVING. FIVE DAYS LATER HE MOVED A LARGE TOOL OFF ONE OF THE TRUCKS AND FELT A PAIN IN HIS BACK. UPON RETURNING TO WORK, HE REPORTED BACK PAIN. WAS PLACED ON RESTRICTED DUTY 8/19.

In order to save time and effort ee crossed a pipe line. he did not have enought step to clear and cross safety, and tripped and fell. he shoulder have walked around the line until a crossing was in place. ee placed on restricted duty on 6-2-04.

LEAKING COUPLING APPEARED TO HAVE COME LOOSE ALLOWING GAS TO LEAK. TORCH WAS NOT PROPERLY TIGHT TO PERFORM THE TASK. LACK OF SKILL RESULTED IN INADEQUATE PRACTICE OF CHECKING ALL FITTINGS BEF ORE STARTING WORK.

EE AND ANOTHER EE WERE TRYING TO PUT ADJUSTING ARM ON A D8N DOZER. BLADE BAR SLIPPED CATCHING EE S LEFT LEG.

EMPLOYEE REACHED INTO A PARKED TRUCK NEXT TO A CURB TO RETRIEVE A CUP. AS HE BACKED OUT OF THE TRUCK HE TRIPPED OVER THE CURB.

EE WAS OPENING HOOD OF TRUCK, FOOT SLIPPED, HE FELL AND HIT LEFT KNEE ON BUMPER.

IMPROPERLY USED EQUIPMENT. TOO MUCH CABLE WAS MOVED AT ONCE WHICH PUT TOO MUCH STRAIN ON EE'S BACK.

STEP TO GET OFF PEDISTAL IS NARROWER THAN A NORMAL STEP. EE WAS PACKING A PUMP. HE STEPPED FROM THE BEARING PEDESTAL TO A STEP ON SIDE OF PEDESTAL TO MOVE DOWN OFF PUMP. HIS RT. FOOT SLIPPED FROM THE STEP AS HE SWUNG HIS LT. FOOT DOWN TO DISMOUNT PUMP. THIS RESULTED IN A 6" TWISTING FALL THAT EFFECTED HIS LOWER BACK.

IMPROPER HOUSEKEEPING, STEEL BOX SHOULD HAVE BEEN REMOVED FROM THE PIT CAR.

EE STATES SHE WAS SWATTING AT MOSQUITOES WHEN THIS INCIDENT OCCURRED. EE WAS PERFORMING A PRE-OPERATION INSPECTION OF RECOVERY PIT PUMP. AS SHE APPROACHED MOTOR DISCONNECT SWITCH SHE SLIPPED I N A PUDDLE OF SLIMES. SHE STATES SHE DID NOT FALL BUT MOMENTARILY LOST HER FOOTING. SHE COMPLAINED OF BACK PAIN A FEW HOURS LATER. SHE WAS SEEN BY COMPANY DR. AND PLACED ON RESTRICTIONS.

EMPLOYEE WAS WORKING IN SCREEN LAUNDER TO CLEAN AREA IN PREPARATION FOR MAINTENANCE. HE STEPPED BACKWARD INTO THE DISCHARGE CHUTE AND REACHED OUT AND CAUGHT HIS HAND ON METAL EDGE OF CHUTE.

INCIDENT APPEARS TO HAVE BEEN CAUSED BY AN S02 RELEASE AT A NEIGHBORING CHEMICAL PLANT (BREEZE WAS BLOWING FROM E/NE AND HIGH CLOUDS). EMPLOYEE WAS WORKING ON A DAM SPILLWAY, 2 MILES EAST OF THE CHEMICAL PLANT WHEN HE SUFFERED BURNED EYES AND A SORE THROAT. INVESTIGATION OF THIS INCIDENT STILL IN PROGRESS.

EE WAS APPLYING FORCE TO A COMEALONG HOLDING A PIPE THAT HAD MOVED AFTER THE BOLTS WHERE REMOVED FROM A FLANGE. THIS PIPE HAD MOVED MORE & IN A DIFFERENT DIRECTION THAN PLANNED. EE PERFORMED T ASK WITH LESS THAN DESIRABLE THOUGHT FOR BODY POSITION & EXERTION, RESULTING IN MUSCLE STRAIN. EE WAS PLACED ON RESTRICTED DUTY ON 6/4 & X-RAYS WERE ORDERED.

WHILE BOLTING A 20" FLANGE PIPE TOGETHER DURING WINDY WARM CONDITIONS, EMPLOYEE WAS OFTEN CLEANING SWEAT AND SAND FROM HIS FOREHEAD BY WIPING IT WITH WORK GLOVES RATHER THAN A CLEAN RAG. A SMA LL SLIVER OF STEEL WAS REMOVED FROM HIS RIGHT EYE.

AFTER WELDING NEW BRACKETS, EMPLOYEE RAISED WELDING HOOD AND BEGAN TO CHIP WITH HAMMER. A PIECE OF HOT SLAG FLEW INTO LEFT EYE.

AN EMPLOYEE WAS USING AN IMPACT GUN ATTEMPTING TO SCREW A 1" BOLT ONTO A 1" STUD THAT APPARENTLYHAD SOME DAMAGED THREADS. THE NUT SPUN OFF STUD AND FLEW IN AIR ~2-3' AWAY, HITTING INJURED EMPL OYEE IN MOUTH CAUSING A FRACTURED TOOTH.

GRATING IN AREA BETWEEN PUMPS WAS NOT INSTALLED IN A MANNER THAT WOULD PREVENT IT FROM SHIFTING TO ONE SIDE: THUS, SERVING AS POTENTIAL FOR FALLING BETWEEN BEAMS BY VIBRAITION ALONE.

AFTER INSTALLING THE SUCTION NIPPLE ON THE NEW SUCTION PLATE TO THE PUMP, EE BENT OVER WITH IMPACT WRENCH TO TIGHTEN BOLTS AN EXPERIENCED PAIN IN HIS LOWER BACK. EE REPORTED INCIDENT ON 11/26 /01.

EE WAS WALKING & TALKING WITH ANOTHER EE & STEPPED INTO A DEPRESSED AREA IN TERRAIN.

EE WAS ATTEMPTING T MOVE A 55 GAL DRUM BY HIMSELF HE PULLED THE DRUM TO TIP & ROLL WHICH RESULTED IN MUSCLE STRAIN DUE TO OVEREXERTION

EE WAS CHANGING A 12" 90 DEGREE OVERFLOW LINE WHILE WORKING FROM A PLATFORM WITH A SMALL SUPPORTON HIS KNEES. LATER IN THE DAY HE WAS NOTED LIMPING WHILE WALKING & NOTED SWELLING ON LEFT KNEE.

EE WAS USING AN EXTENSION LADDER TO ACCESS WHIRLSIZER TO CLEAN DEBRIS FROM SAME. READJUSTMENT OFLADDER CAUSED LATCHES TO SHIFT AND BECOME UNSTABLE. THE SUDDEN JOLT CAUSED EE TO LOOSE BALANCE A ND FALL. THE LANYARD CAUGHT EE AND PULLED HIM INTO SIDE OF TANK, STRIKING SIDE OF FACE AND HEAD AND LOSING CONCOIUSNESS MOMENTARILY.

WHILE ATTEMPTING TO ESCAPE FROM FALLING POWER LINES DUE TO TRACTOR OPERATOR HITTING GUY WIRE, EMPLOYEE JUMPED 3 FOOT OBSTRUCTION AND SUFFERED A SPRAIN/ STRAIN OF LEFT LOWER BACK. HE REMAINED ON REGULAR DUTY UNTIL 3/6/01 WHEN CONTINUED COMPLAINTS OF BACK DISCOMFORT RESULTED IN RESTRICTED DUTY.

MAINTENANCE MECHANIC WAS BURNING BOLTS OFF PIPE FITTING. WHEN HE MOVED HIS HAND BEHIND THE FITTING, HAND CAME IN CONTACT WITH TORCH FLAME.

EE WAS REPLACING GRATING ON A FLOATING SEALING WATER PUMP. THE HOSE WAS STRETCHED TAUT, HAVING CAUGHT ON EDGE OF SLED AND AS EE TURNED HIS BODYTO MAKE A CUT TO TRIM GRATING, THE MOVEMENT OF H IS BODY CAUSED THE TORCH TO PASS OVER HIS LEFT HAND RATHER THAN THE CUT HE INTENDED TOMAKE. THIS RESULTED IN 1ST & 2ND DEG BURNS TO APPROX 25-30% OF HIS HAND FRONT & BACK INCLUDING LITTLE, RI

BACKED DOWN SLOPE IN 3RD GEAR & HIT BOTTOM TOO HARD; HIT HIS TAILBONE ON SEAT

WHILE ATTEMPTING TO LOCK OUT PIT MONITOR HYDRAULIC SYSTEM FOR 14 MATRIX SYSTEM, SLID INTO AN 18" EARTHEN HOLE NEAR ELECTRICAL SWITCH. UNEXPECXTED MOVEMENT TWISTED RIGHT KNEE. THIS AGGRAVATED P RE-EXISTING KNEE PROBLEMS AND ON 9/7 AN ORTHPEDIST DRAINED THE KNEE.

HE WAS PULLING HIMSELF UP ON TRACTOR. PULLED MUSCLES--TRAPEZIUS STRAIN.

DOING UNDERCARRIAGE ON D8, PULLED MUSCLE IN LEFT ARM.

INJURED EE WAS ASSISTING ANOTHER EE TO PUSH A FLANGE TO A BACK UP RING ON 22' PIPE BY USING A TAPPING METHOD WITH A 10 LB SLEDGE HAMMER. THE NON-INJURED EE HAD HIS BACK TO THE FLANGE AND WOULD NOT SEE THE PLACEMENT OF INJURED EE'S HAND.

EE CHOSE TO MOVE AN ALUMINUM PLATFORM BY HIMSELF RATHER THAT USE BRIDGE CRANE THAT WAS AVAILABLE FOR MATERIAL HANDLING.
