Installing steel plates and shoring in open excavation to perform work on plant drainage lines.

The ee was performing the routine task of cleaning the feed shelf in the PHT. EE was using an air lance through a poke hole to move material when some material exited the poke hole and fell into the gauntlet of EE's left glove and burning EE's exposed skin. EE was not wearing all mandatory PPE provided for the task.

Employee was grinding a shim plate on top of the kiln (9' grinder). The grinding wheel flew off of the grinder, struck the Tyre, deflected back and struck employee in the right thigh. First aid was administered. Employee was taken to urgent care where they received 5 stitches to right thigh.

Crew member was cutting a piece of intermediate diaphragm. As employee was cutting through the piece, the piece broke free, swung and hit the employee on the head and hand.

EE was using an air lance to clean the grizzly at the discharge of K2 coal belt when the steel tube unthreaded itself from the ball valve installed after the Chicago fitting. EE attempted to pinch the air hose but was struck on the rt. forearm by the air stream receiving a high pressure air wound. EE was transported to a local medical center for treatment and observation.

EE was cutting out damaged re-bar with a burning torch. EE did not realize that the piece he was cutting had stored energy. As he cut through on one end, the re-bar sprung up and struck him on the rt. forearm. The wound is a laceration requiring stitches

EE was changing our wear shroud on a CAT 990 loader bucket. The bolt dropped past the shank allowing the bolt to spin. The EE used his gloved hand to push the bolt back up and inadvertently hit the trigger to the impact wrench. This caused the bolt to spin, cutting through the glove and cutting the EE's hand.

EE using a bench grinder to dress 1/8" thick steel band. Work piece, glove and then right hand was pulled into the grinding wheel. Right pinky finger broken in two places and badly lacerated.

Hand drill became bound while drilling and twisted employee's hand.

Employee was using cutting torch and molten metal ran down sleeve, into glove.

Employee was using a drill on the ID Stack railing for modifications to be made. When drilling, the drill caught in the hole and twisted. The employee reported the incident as a first aid, but requested medical attention on 10/21/08. X-rays revealed a fracture to his right hand

Employee was in the mill while air arcing mill liners. The employee was wearing safety glasses but reflection off of the mill liner was too bright.

Employee was pushing material with dozer when rock struck dozer and he jerked backwards.

WORKER WAS USING AIR TORQUE WRENCH TO FIT STEEL. HE ACTUATED WRENCH W/OPPOSITE HAND NEAR TORQUE ARM. TORQUE ARM PINCHED UNDERSIDE OF LEFT MIDDLE FINGER.

EMPLOYEE WAS GRINDING THE BEVELS ON THE KILN SHELL WHEN A PIECE OF GRIT GOT INTO HIS EYE. RX GIVEN.

Air hose separated from connection and struck employee in right ear.

Two employees were being hoisted in a manbasket by a crane (approx 10 ft) when the operator noticed the cable unwinding from the drum. The employees were lowered to the ground by the operator as the cable became fully unwound, thus dropping the block and cable down into the basket. The employees were able to get out of the basket, however, one was struck in the foot by the cable.

Employee was assisting contractor with a plasma cutter. Contractor was turning equipment on and off in an attempt to make it work. Employee was holding torch in his hand when it fired unexpectedly.

Employee was welding on 3rd pier---welding stop block against tire. When pulling lead across catwalk, lead grounded against steel causing flash.

EE states that he was operating a drill, when the bit kicked back, causing a right hand sprain.

Employee was working on a pump. Employee removed grease plug from a bearing under pressure. Hot grease shot out of bearing covering employees face and right hand. Employee sustained first degree burns to face, neck, head and right hand.

Employee was jackhammering around a conveying line and cement dust went into left eye causing irritation.

On 6-12-2006 employee reported a work related injury to his neck. On 4-19-2006 while running #2 crane, after several hours of working noticed neck pain.

A bag of cement burst and struck employee in face.

Employee was trying to remove counterweight guide from bottom coal elevator. He used a torch to be able to stand out of the way while bolt was burned out. When bolt was freed, the guide came out and hit employee in forehead.

MECHANIC WAS LOOSENING BOLTS ON DRIVE COUPLING WITH BRAKEOVER. PUT BRAKEOVER AGAINST CHEST AND PUSHED TO LOOSEN BOLTS WITH CHEST WHEN MECHANIC FELT SOMETHING POP IN CHEST. IMMEDIATE PAIN IN CHEST. REPORTED INJURY TO SUPERVISOR. WORKED REST OF DAY THINKING MUSCLE WAS STRAINED AND WOULD GET BETTER.

Tightening liner bolts in raw mill with impact wrench----had to tighten full set of bolts in mill inlet end using impact wrench. Noticed soreness and tingling in hands.

Employee was removing cover plate to install sampler from designated work platform. Approx. 5 gallons of hot dust fell out. Dust hit triggering arm with force and ricocheted back to employee, hitting him on right forearm and entering glove.

Employee's right index finger caught in fan belt in drill rig motor causing partial amputation.

While jackhammering through an opening on top of a silo, the jackhammer bit droped through the material causing the jackhammer to fall pinching a finger between the silo roof and the jackhammer handle. The right ring finger finger tip required 5 stiches.

When claimant was jackhammering in the transport screw dust collector area, his jackhammering action must have caused some clinker dust to fall on him from above his work area.

Lifting vise on drill press and vise slipped smashing knuckle on drill press table.

The employee walked up behind an operating backhoe. He did not make contact with the operator. The backhoe operator made a move with the bucket and machine lurched. The outrigger pad came down on the employee's right foot breaking the bones of the large toe.

Employee was looking into an air slide to observe how it was working. The dust collector pulsed and blew cement dust towards his eyes. Employee was wearing eye protection, however his eyes felt irritated. His eyes were flushed with water, he then proceeded to go to the doctor. The doctor found no abrasions on the eye and EE was able to return to work at full duty.

An oxy acetylene torch backflashed. The employee was burned on the inside of his right forearm.

USING A JACKHAMMER TO BREAK UP CLINKER, HAMMER GOT STUCK. EMPLOYEE WAS UNABLE TO PULL IT OUT. HE THEN FELT A MINOR TWITCH IN HIS CHEST. HE WAS SENT TO A LOCAL CARE FACILITY.

On 02/19/04, ee reported to supervisor that he had backed the dozer into a small depression causing the dozer to jerk. The ee began to feel pain & numbness in his shoulder & arm. He did not feel he needed medical attention. On 02/26//04 he did go to the company doctor who suggested therepy. This did not help & the ee had surgery on 05/20/04, making it a lost time accident.

The employee drove a D8 dozer off a quarry bench. At the time no injuries where reported except a buised leg and sore shoulder. The employee was examined at a local hospital with no injuries found. The employee's shoulder continued to be sore. The employee was sent for an MRI discovering a slight tear and bone spurs that will not allow healing to take place.

INSPECTING PREHEATER AIR CANNONS FOR INLEAK AIR. IAR CANNON FIRED. HOT DUST BLEW OUT OF CRACK ONTO EMPLOYEE'S ARM.

EMPLOYEE WAS USING JACK HAMMER THE ENTIRE SHIFT TO CLEAR CHUNKS IN AKWARD BODY POSITION. FELT MORE SEVERE PAIN THE NEXT DAY AND WANTED MEDICAL ATTENTION.

EMPLOYEE WAS CUTTING W/A CUTTING TORCH. A PIECE OF HOT SLAG FROM THE METAL OT INTO HIS EAR AND CAUSED A BURN INSIDE OF HIS EAR. HE SAID THAT THIS TYPE OF THING HAPPENED ALL THE TIME AND HE DID NOT THINK ANYTHING ABOUT IT. HE DID NOT REPORT THE ACCIDENT UNTIL 11/22/02 WHEN HIS EAR BEGAN TO HURT HIM WORSE. HE HAS A PERFORATED EAR DRUM.

TAKING CALCINATION SAMPLE OUT OF PREHEATER FEED PIPE. DID NOT ISOLATE VALVE. AIR CANNON SHOT BURNING HOT FEED ONTO EE.

EE WAS CUTTING A PIN OFF F33 BELT. SOMEONE TRIPPED OVER THE GAS LINE WHICH PULLED THE TORCH THAT WAS IN HIS RIGHT HAND DOWN ON HIS LEFT WRIST.

CUTTING CONDUIT WITH A PORTABLE BANDSAW. HAND SLIPPED & CUT LEFT INDEX FINGER.

EE'S RT. ARM SHIRT SLEEVE BECAME ENTANGLED IN THE MOTOR COUPLING WHEN HE REACHED OVER THE MOTOR DRIVE COUPLING TO PURGE THE LUBE SYSTEM. DONDITION: THERE WAS NO GUARD IN PLACE. SECOND, STOP SW ITCH TOO FAR AWAY. THIRD, NO BLEEDER VALVE INSTALLED.

WHILE REMOVING HATCH FROM MILL INJURED SIGNALED THE CRANE TO GO UP WHILE HE WAS HOLDING WIRE ROPE IN HIS RIGHT HAND. HE LOOKED AWAY FROM THE PICK AND HIS GLOVED HAND WAS DRAWN INTO THE SHEAVE.

EMPLOYEE WAS SUPERVISING THE COMPLETION OF THE RETAINING WALL AT THE PRIMARY CRUSHER. HE WAS AT THE TOP OVERSEEING THE PLACEMENT OF PRECAST CONCRETE PANELS (STACKING) USING AN EXCAVATOR TO SET THEM. THE MACHINE WAS SETTING ONE DOWN, WHEN IT CAME FORWARD TOO FAST CAUSING SLACK IN THE LIME. IT HIT (THE SHACKLE) HIS FOREHEAD, CAUSING THE LACERATION.
