Electrician modifying a threaded nipple with portable bandsaw for conduit installation to transformer and lacerated EE's right middle fingertip.

An operator was handing an impact wrench over to another operator that still had pneumatic pressure. When the receiving operator grab the socket end to take possession of the impact the operator handing it over accidently engaged the trigger.

Operator of a Caterpillar D6 Dozer was repositioning on a narrow strip and was looking behind them while travelling forward and ran the left track over a berm causing the dozer to fall off of a 15 foot tall highwall. The operator struck their head on something in the cab and was taken to Urgent Care. The physician asked that they be off work for two days.

Injured was removing a section of duct with mobile crane. When duct was free the duct struck the injured in the right arm and jaw.

A production Coordinator was investigating a plugged air slide on a bag house. While trying to unplug it with an air lance, the air slide built up pressure which resulted in cement/dust blowing out of a sample port towards the back of EE's head getting behind the employees glasses and face mask.

Employee was using a shear in the maintenance shop to cut a piece of metal, during the task the metal shifted causing EE to lacerate EE's index finger. The employee was seen by a physician and treated for a laceration, receiving stitches.

The mounting plate for the Secondary Crusher (deflector plate) had built up material behind it creating a force on the plate. When the end of the plate was cut loose from the crusher housing, the plate rapidly released this tension like a spring and swung out impacting the mechanic. The mechanic suffered 2 lacerations on the face and as EE fell back landed on the tail bone.

While removing the bolts for the uphill thrust roller on K1, EE had their fingers pinched between an impact drill and the structure of the thrust roller resulting in 6 stitches to the 4th and 5th fingers on left hand.

Employee was adjusting the jaw crusher when the spring retaining washer failed causing a laceration and broken right pinky finger.

Employee got a metallic foreign body in right eye from welding.

Employee was using a 4" grinder with a cutting wheel to cut angle iron when grinder bound up causing it to kick back creating a laceration on the employees mid left thigh

Employee was using an air lance to clear material form inside the kiln hood area. The employee was poking and pushing the material and stated felt a pop in left shoulder. The employee was taken to the occupational doctor and was diagnosed of having a sprained shoulder. Placed on restricted duty.

Employee was was on the cat walk of the cone crusher checking for bowl float when the cone over flowed and an 8" rock fell down cutting ee left little finger that required three stitches

Employee was in the line of fire of a welder and received a burn to EE's right ear.

Employee was using a rivet buster to break up material behind the maintenece shop for aproximatly 4 hours. Next day the employee complained of burning sinsation in his shoulders. The employee went to the med clinic where he was diagnosed with a right shoulder strain. The employee returned to work with work restrictions of no overhead lifting and PT 3 times a week for 2 weeks.

Employee was cutting bolts from a bearing house when the wheel of the grinder kicked back causing his left hand to loose grip with the grinder. The left hand came in contact with the cutting wheel causing a 1" laceration to his left index finger knuckle. Injury required 3 stitches.

While cleaning about a plugged vessel with an air lance, the vessel flushed dust out and contacted the employee in the face getting dust in his left eye. The employee washed out his eye and completed his shift. Then he went to the clinic and a floating particle was removed. He received prescription eye drops.

EE was cutting a metal piece at ground level, when getting up changing position he didn't turn the torch off and lost grip of it; he reached it by the hoses and the burning tip got pointed at his right knee causing a 1.5" burn. EE received first aids at site and refused medical attention. On Jan 27th he asked for Medical treatment, went to the doctor and received modified duty.

EE was power washing the exterior of the finished cement tube, when a foreign particle got in his right eye. Particle was probably a spec of metal or paint from the power washing. (EE was wearing Safety Glasses during that job-His Supervisor has informed the office today that Safety Goggles have been provided for the crew.)

Piece of steel fell on the miners left foot just above the metataral area.

Employee states with the repetative movement of operating a dozer, he sustained an unknown injury to the neck. Employee completed a WC case with Industrial Commission on 12/4/08, we did not receive confirmation on this injury until 1/8/09. This case is currently under investigation and an amendment may be submitted at a later date.
