Excavator was shutting down at end of shift. Excavator was top loading trucks, the operator cut a ramp to walk excavator off the level it had been operating from. While walking down ramp, excavator started bogging in dirt and suddendly stopped, caused cab to tilt over. EE released seat belt, hit forehead on windshield, deep cut. 2 witnesses called foreman(called 911).

Mostrando los 29EE was using a drill press in the shop to drill holes to a larger size in a piece of aluminum stock 4"X4"X4". The vice had been removed from the drill press. He attempted to hold the piece in his left hand while drilling, his hand slipped & the drill bit cut his hand between his thumb and index finger.
Mechanic was performing maintenance work on a bauer mill. A bolt was stuck in the bottom and when he tried to clear it, the rotator assembly moved and piched the distal end of his left little finger between the assembly and the bauer wall. Employee sustained a laceration to the distal end of his left pinky finger.
Employee was using grinder to cut metal pipe. Grinder contacted left index finger causing laceration requiring 4 stitches. **Addendum to previous report submitted on 6/17/08: Employee continued to have complaints and as a result he was referred to an orthopedic doctor that resulted in his return to modified duty effective 7/10/08.
Employee working with gasoline powered sludge pump used to transfer water. Employee noted pump operating but not producing flow (lost suction). Employee shut off gasoline engine driving pump and bled pressure. He slowly loosened plug at top of pump to further reduce pressure. Plug sprung loose and released hot water under pressure. His right forearm sprayed by hot water.
Employee was adjusting an Aseptrol packaging machine guide. Approximately 1/4" of the tip of the middle finger was amputated by the knife.
Employee was standing in an awkward position and did not have complete control of grinder. The grinder kicked back and cut his right knee 3 cm.
Employee was clearing a bag jam from the automated palletizer. He had left shoulder pain, right leg pain and a tightness in the back. There were no restrictions from his primary job duties so this was considered a first aid case.
MECHANIC WAS WELDING STEAM COILS IN BOTTOM OF RT-77 TANK FOR LONG PERIODS OF TIME. WHEN HE STOOD, HIS RIGHT LEG POPPED AND HE WAS UNABLE TO STAND ON HIS RIGHT LEG DUE TO PAIN.
WORKER WAS CUTTING LOOSE RING HEADER. WHEN THE RING BROKE LOOSE IT MOVED CAUSING THE ANGLE IRON TO SPRING BACK, HITTING THE END OF THE RING FINGER ON HIS LEFT HAND.
WORKER WAS GRINDING INSIDE OF TANK, WEARING SAFETY GLASSES UNDER FACE SHIELD. WHEN HE STOPPED GRINDING AND LIFTED HIS FACE SHIELD, DEBRIS FELL INTO HIS EYES. HE RINSED HIS EYES OUT AT AN EYE WASH STATION. DOCTOR TOOK A SMALL SPECK OF METAL OFF HIS EYE. WE ARE PROVIDING GOGGLES TO BE WORN UNDER A FACE SHIELD WHILE GRINDING & INSTRUCTIONS ON SHAKING OFF FACE SHIELD BEFORE LIFTING.
Ee was repairing a broken cable on bulk loading chute. When he went to lower the chute, he hit the wrong button and raised the chute instead. His right hand was caught in the cable resulting in laceration and fracture to the right index finger. He received treatment at ER.
A LAB OPERATOR WAS OPERATING A MIXER IN THE LAB. WHEN HE WENT TO REMOVE THE CUP FROM THE MIXER, THE MIXER CAUGHT HIS RUBBER GLOVE AND CUT HIS LEFT MIDDLE FINGER.
EE was removing a pressure washer from an upper level floor & it was being lowered down the steps to bottom floor for repairs. The EE used the hoist to raise the front of the unit to get it off the handrail when the hook connected to the handle & slid over pinching his rt. little finger causing an avulsion to the distal end.
EE was attempting to troubleshoot the autobagger's layer forming unit when the gate came into contact with his right hand causing a severe laceration to the back of the right hand. He received treatment at the ER and was found to have a severed tendon and bone.
CALCINER PRODUCT ACCUMULATED IN DUCT LEADING TO BAGHOUSE & PLUGGED IT. A 20' LONG AIR LANCE HAD BEEN FABRICATED TO BLOW BLOCKAGE FREE. HE PLANNED TO INSERT AIR LANCE INTO DUCT & SIGNAL FOR AIR TO BE TURNED ON. OTHER EE THOUGHT HE SIGNALED FOR HIM TO TURN VALVE BUT BEFORE HE COULD MOVE, WAS SPRAYED WITH HOT GAS & PRODUCT FROM CONVEYING DUCT. BURNED ON BOTH FOREARMS & LEFT FOOT.
Employee was welding in blunger. After his shift, both eyes reddened, became teary and painfull and vision blurred. Employee saw eye doctor and was diagnosed with kerato conjunctivitis secondary to welding arc. He was given prescription eye drops and anti-inflammatory.
EE WAS PREPARING TO BAG PRODUCT USING BAGGER. PRODUCT DROPPED ONTO THE EE FROM THE BAGGER WHEN THE DUST COLLECTOR PULSED. PRODUCT GOT INTO EE'S EYE (LEFT). EE WAS WEARING SAFETY GLASSES.
THE RAREEARTH MAKEDOWN SUMP USES A SAND PIPER PUMP AS A TEMPORARY PUMP. THE SUMP IS IN A RECESSED AREA UNDERNEATH THE MAKEDOWN REACTOR BUT THE TEMP PUMP IS POSITIONED ON THE BAG FLOOR ABOVE. T
OPERATOR WAS ATTEMPTING TO SHUT LEFT DOOR ON MOTORGRADER, WITH HIS RIGHT HAND HOLDING CAB POST HE LEANED OUT AND PULLED DOOR SHUT. THE DOOR CLOSED SHUT ON HIS RIGHT THUMB, BREAKING THE BONE AN D CAUSING A LACERATION REQUIRING SIX STITCHES.
EE WAS UNLOADING A TRUCKLOAD OF STEEL, GOT THUMB MASHED BETWEEN THE RIGGING HOOK AND THE IBEAM. EE WAS WEARING GLOVES AT TIME OF ACCIDENT. MASHED THUMB REQUIRED STITCHES.
MOVING MANLIFT OVER ROUGH TERRIAN, INCLUDING RAILROAD TRACKS. MANLIFT BASKET BOUNCED APPROX. 3 FEET ADN JARRED EE BACK. ROUGHT TERRIAN CENTUIBUTED TO ACCIDENT.
EE WAS HYDRO BLASTING A 2" PRODUCT MATERIAL LINE WITH A 40K LANCEHOSE. OTHER OPERATED THE FOOT PETAL. THE PRODUCT LINE WAS APPROX. 20 FEET IN THE AIR ADJACENT TO A CATWALK, WHICH WAS USED FOR ACCESS TO THE LINE. EE WORE A SAFETY HARNESS ANDWAS TIED TO TO AN I BEAM, WHILE STANDING ON THE PIPE RACK.
EE WAS WASHING BEACH HORSE FLOOR WITH PRESSURE WATER HOSE. THE HOSE LINE CAUGHT ONT HE DUMP TRACK AND WAS PULLED OUT OF EE HAND. THE END OF THE HOSE HIT EE IN THE LEFT TEMPLE AND KNOCKED HIM T O THE GROUND. EE WAS TAKEN TO THE BAINBRIDGE MEMEROILA HOSPITAL AND TREATED FOR SMALL ABRASION ON TEMPLE WITH SUPER GLUE AND ANTIBOTIC OINTMENT. EE WAS RELEASE AND RETURN TO HIS SHIFT ONE.
THE PREMIER MILL #4 WAS RUNNING WITH NO FLOW. THE OPERATOR WAS CLEARING THE DISCHARGE SCREEN. THE SCREEN BLEW OUT OF THE MILL SPRAYING THE OPERATOR WITH HOT LIQUID.
THE PREMIER SLURRY MILL WENT OUT ON HIGH PRESSURE. OPERATOR WAS CLEARING SCREEN. SCREEN BLEW OUT OF MILL SPRAYNG OPERATOR WITH HOT LIQUIOR OPERATOR SUSTAINED 2ND BDEGREE BURNS ON 60% OF LEFT A RM.
HAND CAUGHT IN PINCH POINT OF AGITATOR WHILE TAKING VIBTRATION READINGS.
EMPLOYEE WAS WORKING ON AN AUTOMATIC BAGGER ATTEMPTING TO CORRECT A BAG THAT WAS OUT OF POSITION WHEN HIS RIGHT WAS CAUGHT AND PINNED BY THE BAG PUSHER.
INJURED EMPLOYEE WAS ATTMPTING TO MACHINE A HUB IN THE METAL LATHE IN THE MAINTENANCE SHOP. THE HUB HAD A LIP THAT PREVENTED THE JAW OF THE CHUCK TO HAVE FULL CONTACT WITH THE PIECE. AS HE S TARTGED THE LATHE, THE CUTTING TOOL CONTACTED THE HUB AND EJECTED IT INTO HIM, RESULTING IN A LACERATION ON HIS CHIN THAT REQUIRED FOUR STITCHES.
