Employee slipped and fell hitting is hip, shoulder and head on concrete floor. The employee was seen by doctor the next day and recieved first aid care. The employee continued to do full job duties but had gradual worsening of shoulder pain. An MRI was conducted and revealed a small rotator cuff tear on 9/9/30 and the employee was givne prescription medication.

Employee tripped over a raised concrete area, lost his balance, reached out to break his fall, and fell on his hand fracturing the left index finger. EE also sustained a hematoma to distal end of same index finger.

On March 16, an EHS Specialist suffered a back strain while ascending a ladder during a MSHA inspection. This incident has been classified as a recordable injury based on the medical determination that his work activity aggravated a pre-existing condition, incident was deemed work related on 4/23/10. Injury-RDI. Injury-Lost workday incident.

Employee alleges right wrist strain while tying big bags. He denies performing job differently or outside the norm.

Employee (Mechanic) got his middle finger (right hand) caught between the cable and load block pulley on a mobile crane. Emp. was attempting to push cable back into pulley groove when injury occurred. Emp. was not wearing gloves.

Excavator was shutting down at end of shift. Excavator was top loading trucks, the operator cut a ramp to walk excavator off the level it had been operating from. While walking down ramp, excavator started bogging in dirt and suddendly stopped, caused cab to tilt over. EE released seat belt, hit forehead on windshield, deep cut. 2 witnesses called foreman(called 911).

employee was on a temporary cross tie platform, laid across the tracks that was not secure, to fill a truck. It rolled causing the employee to fall on his back and elbow.

EE was assigned to assist BASF maintenance in rebuilding pumps. The ee was carrying part of the pump housing, lost traction and slipped down into a seated position; the pump housing fell across the ee left leg causing a laceration that requited sutures. EE returned to light duty work the next day.

Employee was working with a shovel in a tank when underside of left forearm contacted blade of agitator. Agitator's blade was sharpened on edge, causing laceration to forearm.

EE was tightening a bolt from the 2nd step of a ladder. His wrench slipped off bolt causing him to lose his balance and fall. He landed on his right hip and cracked his right pelvis.

EE was assisting drilling crew in connecting cables & pipe, stepping up & down from the drill rig platform (repetitive motion). He felt a slight pain in lower back. EE has a previous back injury dating back to 2004 with surgery & restricted duty. He remained on restricted duty & had reoccurence in 2008 & now another & is scheduled for Dr. appt. 1/29/10.

Employee was descending stairway carrying 4 empty 5 gal water bottles. He was not using the handrails provided. He missed the last step and fell forward onto both knees. He was eventually diagnosed having a "patellar dislocation quad tendon rupture" requiring surgery. Diagnosis did not occur until July 24, 2009. Surgery scheduled for July 28, 2009.

While performing rounds, an operator went to climb two steps. The operator stepped on the first step and felt his Achilles tendon pop. He was transported to the emergency room, where his was given medication, crutches and the ankle was immobilized. The operator injured this same tendon 3 years ago in a non-work related event.

Operator was disconnecting a hose from a tank. The hose was depressurized and believed to be empty, but apparently had residual recovered sodium silicate. As the operator was disconnecting the hose, the chemical splashed onto his face and under his safety glasses, resulting in irritation to the eyes.

Employee was walking down the stairs to control panel at belt filter when his foot slipped and he twisted his left knee

A Mechanic suffered a strain to his left shoulder while he, along with another employee, supported a gas valve assembly that was being removed for replacement during a Calciner outage. The physican diagnosed a muscle strain. The Mechanic continued to have pain in his shoulder. The employee had a MRI which revealed a small tear in the rotator cuff.

Employee was spraying weed killer around the sides of the shower house. The employee stepped from the parking lot to the ground and twisted his left ankle on some uneven concrete. There is a 16-inch drop at the edge on the parking lot to the ground.

EE was using a drill press in the shop to drill holes to a larger size in a piece of aluminum stock 4"X4"X4". The vice had been removed from the drill press. He attempted to hold the piece in his left hand while drilling, his hand slipped & the drill bit cut his hand between his thumb and index finger.

EE was in the shop working on a spray bar. He was installing a spray nozzle(approx. 3/4" x 11/2" long) into a 2" pipe. The wrench he was using to thread the nozzle into the pipe slipped off, causing him to twist his back. At the emergency room, he received an injection for the pain.

Employee was cleaning out a containment pit of wet sediment with shovel. In the process he strained his back.

The employee suffered a hairline fracture to his left foot while descending a three foot fixed ladder. He was examined by an orthopedic specialist where an x-ray revealed the fracture. The Operator returned to work with a removable cast and is scheduled for a follow-up appointment in three weeks. This incident is currently classified as a Recordable Injury- RDC.

Employee exited a control room to a platform and while descending steps caught the back of his boot heel on the previous step and fell backwards injuring his left knee.

Original incident Knee hurting on 5/08 by truck driver. Second 12/02 complained about a knee hurting. Dr.Visit and examination the Dr. found that the employee has a pulled/strained hamstring and the Dr. referred him to have physical therapy.

EE was looking for a bucket to collect a sample from a stockpile. He lifted an equipment rim to pull the bucket out from under it. His hand slipped off the rim and it fell on his finger crushing it. He went to the emergency room in Gallup New Mexico where he was treated. His injury required two stitches and a splint to protect his finger.

Mechanic was performing maintenance work on a bauer mill. A bolt was stuck in the bottom and when he tried to clear it, the rotator assembly moved and piched the distal end of his left little finger between the assembly and the bauer wall. Employee sustained a laceration to the distal end of his left pinky finger.

I-Tech was helping electricians pull new wire in MCC 1. The rope that was attached to the wire came loose and the I-Tech fell to the floor and felt a sharp pain in his right knee.

While lifting a screen deck into place employee stepped on a bolt, slipped slightly (did not fall)and felt a pain in his lower back area. Employee has a sore muscle in his lower left back.

#4 Silicate Concentrator area sump pump not pumping material. While investigating, pump discharged liquid & struck ee in face; knocked off safety glasses, liquid went into eyes. Flushed eyes at eye wash station. Examined by site nurse before being transported to see local opthamologist who prescribed antibiotic ointment, pain medication & released employee.

EE was cutting the end of a Robroy condulet to remove excess in order to insert plug. He had his left hand stabilizing the pipe, about one foot up from the end. He was cuttig the end of the condulet with a razor knife in his right hand and pulling up. The blade slipped and cut his left thumb, resulting in two stitches.

Worker was stooped over showing another worker where some conduit needed to be touched up with paint. When he stood up, he bumped the back of his head directly below his hard hat on a piece of angle iron.

Two operators were changing out a roller on a filter belt. One operator was supporting the roller. The other operator was unbolting the bracket that holds the roller in place. The operator supporting the roller, stopped supporting it. The bolt broke, the roller fell and struck the right middle finger of the operator who was unbolting the roller.

Employee was walking down a slipped concrete surface to pick up a safe work permit that the wind had blown down. He slipped in some mud on the concrete surface. He turned to catch himself, his hard hat fell off and he fell on the hard hat liner and cut his forehead above his left eye and 1" long. 5 stitches.

Process Specialist suffered from heat stress while working in the calciner area. Employee was in the maintenance area observing maintenance activities for approximately fifteen minutes before exiting the area and became ill upon returning to his office.

Installing conveyor tail guard section, it fell and pinched left middle finger.

Employee was using grinder to cut metal pipe. Grinder contacted left index finger causing laceration requiring 4 stitches. **Addendum to previous report submitted on 6/17/08: Employee continued to have complaints and as a result he was referred to an orthopedic doctor that resulted in his return to modified duty effective 7/10/08.

Employee was performing normal activities in the logisitics area. He stood up after hooking up a hose to a railcar and felt a pain in his right side. The employee was diagnosed with a hernia. The employee did not report this to management properly.

A mechanic worked on a dust collector bag change on 5/22. The next Tues,he called in with complaints of back pain, but did not indicate a work related injury. While receiving medical attention, he reasoned that the bag change must be the cause of injury. On 6/13, he met with supervision to make a W/C claim. An investigation is in process. Awaiting assessment by physicians.

A Mechanic was lifting a pipe from the ground floor to intermediate level of Building. As the lift began, he began to move on the ground level to access stairs to the intermediate level. He tripped over a 3" drain pipe running at floor level and fell, striking his right knee. He was sent to an orthopedic specialist who diagnosed a fractured patella.

EE was on a ladder adjusting the water flow to the dry scrubber, this valve is not within arms reach. On the 3rd step, EE fell backwards and her right leg wedged between the thrid and foeth steps causing her to receive a open compound fracture.

Worker complains of low back pain and leg pain. Also complaining of leg numbness. No incident has occurred, but worker has 2004 work injury and back surgery.

Employee arrived at nurse's clinic at 7:30 am to report complaints of increasing pain in both hands.

Empl has history of hypertenstion. Med?s not controlling. Empl had headache early am, began getting dizzy with a tight feeling in chest. Left area where he was cleaning chutes & went to control room reporting this to another empl. Initial treatment was for heart attack, which was ruled out by normal EKG. Underwent further testing for what has been deemed as a personal condition.

Employee was pulling a bottle cart when the wheels on the cart got stuck between two motor bases. As he pulled the cart, the cart came loose causing him to lose his balance and fall backwards. As he fell, he grabbed onto a structure to catch himself. When he did this, he felt a pain in his left bicep.

Employee experienced low back pain while bending to retrieve extension unit used with bulk loading.

Employee was dropping railcars when it bumped into another railcar. The impact caused him to be thrown against the side of the railcar, which caused multiple bruises to his right side.

Mechanic walked around the Roving Crew truck to get something out of the tool box when he struck the bumper-mounted vise with his right knee. EE sought medical treatment on 10/8/07.

EE was operating the water truck on the haul road. He started up the ramp entrance to the pit area, as he got to the top of the hill, his truck started spinning on the wet road. The truck slip ped backwards, down the ramp and hit the berm, which caused the truck to turn over. There was cab damage to the truck.

Employee was assigned to install a manway cover and valve on Tank #22 and was instructed that a co-worker would assist him. However, EE lifted the manway cover alone and states hurting his lower back. EE sought medical attention by MD on 9/4/07.

Employee working with gasoline powered sludge pump used to transfer water. Employee noted pump operating but not producing flow (lost suction). Employee shut off gasoline engine driving pump and bled pressure. He slowly loosened plug at top of pump to further reduce pressure. Plug sprung loose and released hot water under pressure. His right forearm sprayed by hot water.

Employee was adjusting an Aseptrol packaging machine guide. Approximately 1/4" of the tip of the middle finger was amputated by the knife.
