Employee cut left thumb while cutting a piece of steel.

EE was assisting the plant welder, with fabricating brackets for the bin gate clam shell. While using a drill press and grinder small metal particles collected on the sleeves of the EE shirt. EE wiped EE's face with EE's sleeve contaminating EE's eye with the small metal particles. EE did not seek medical treatment until 5/12/2026.

EE was using a battery powered drill when the drill bit got caught on the steel causing the drill to quickly spin around. This caused the bottom side of the drill to flip and impact EE at the top of their safety glasses, resulting in a laceration above their right eye.

The employee pinched the end of their right thumb between a bolt head and the recessed hole it was in. This occurred when the bolt spun when an impact wrench was applied to the nut on the other end unexpectedly.

Employee was pushing upward on a bolt head to attempt to lock it into position so the nut could be removed with an impact wrench. The employee unintentionally energized the tool causing the bolt head to spin in palm and causing a laceration.

Employee was welding on a slide wrench. Employee had welding shield down and didn't notice the slide starting to move. Slide wrench fell on employee's left pinkie finger splitting open top of finger requiring 5 stiches. Employee was wearing thick welding gloves and proper PPE at time of incident.

TECHNCIAN WAS ATTEMPTING TO REMOVE/REPLACE A WORN BUCKET CUTTING EDGE. USING A WRENCH TO HOLD THE RETAINING BOLT, THE TECH ATTEMPTED TO LOOSEN THE NUT WITH A PNUEMATIC SOCKET DRIVER. THE BOLT AND WRENCH TURNED IN TECH'S HAND. TECH WAS WEARING GLOVES AT THE TIME OF THE INJURY

While cleaning a torch tip, an employee was burned when the torch inadvertently ignited.

Driller was attempting to remove the hammer from the end of the drill section when the outer collar of the drill steel released from the hold position and pinch the driller's left ring finger between the collar and the hammer wrench he was trying to install.

While the employee was cleaning the plant structure with a hot water cleaning system (steam genie), the wand of the system was broken and the hot water burned the employee on the left forearm.

EMPLOYEE STRAINED HIS BACK WHILE USING A CHAINSAW AND LIFTING LOGS. THE IRRITATION OF THE BACK INJURY WAS REPORTED THE FOLLOWING DAY. THE EMPLOYEE WAS PLACED ON RESTRICTED DUTY ON 3-20-01. HE WAS NOT CERTAIN HOW OR WHEN HE HURT HIMSELF.

WHILE TRYING TO STRAIGHTEN AN UPSIDE-DOWN V-BELTON THE CRUSHER SHEAVE AND V-BELT ASSEMBLY, EMPLOYEE'S LOCKING PLIERS SLIPPED AND HIS FINGER WAS CAUGHT BETWEEN THE BELT AND THE SHEAVE, CUTTING HIS RIGHT LITTLE FINGER.

WHILE EE WAS WAITING TO SERVICE MOBILE DRILL AND DRILL WAS BEING LOWERED FROM SUPPORTS TO TRACKS, EE'S LEFT FOOT WAS CAUGHT UNDER TRACKS AS THEY CONTACTED THE GROUND. THE SECOND TOE ON THE LEF T FOOT WAS PINCHED BY THE CONTACT.

EE WAS RELOCATING CRANE. HE HAD HIS FOOT RESTINGON THE HOIST BRAKE PEDAL. THE PEDAL PUSHED BACK VIOLENTLY BENDING HIS LEG. THIS CAUSED A FRACTURE OF FIBULA BONE IN RIGHT LEG

EE HAD REPLACED A GRINDING WHEEL ON HIS BIT GRINDER AND STEPPED TO THE SIDE TO TURN IT ON. WHEN HE TURNED IT ON THE WHEEL BROKE INTO 3 PIECES, ONE OF THE PIECES HIT THE EE ON THE RIGHT FOOT BREAKING THE LITTLE TOE.

THE EE STATED THAT THE PNEUMATIC HAMMER SLIPPED DOWN WHILE HE HAD HIS FOOT ON THE GENTRALIZEN ARM TO HELP HOLD THE DRILL STEEL AS HE WAS BREAKING IT LOOSE. THIS RESULTED IN A FRACTURE TO THE B IG TOE ON THE LEFT FOOT.

EMP STATED THAT HE WAS USING A TORCH TO CUT A STEEL PIPE. BEFORE THE PIPE WAS COMPLETELY CUT INTO, IT BROKE. THIS CAUSED HAND TO BE MASHED BETWEEN PIPE BEING CUT AND ANOTHER PIPE NEAR BY.

EMPLOYEE WAS CLEANING BOLT HOLE THREADS ON THE INSIDE OF A CONE CRUSHER. HE HAD SPRAYED WD-40 SOLVENT INTO THE THREADS TO HELP LOOSEN THE RUST AND SCALE. LATER HE PICKED UP THE TORCH, SINCE IT WAS HANDY, TO BLOW THE LOOSENED MATERIAL OUT OF THE HOLE. THE OXYGEN AND WD-40 CAUSED A FLASH WHICH RESULTED IN A BURN TO THE RIGHT ARM AND FACE OF THE EMPLOYEE.

EMPL TIGHTENING LUG NUT ON TRUCK WHEEL WITH IMPACT WRENCH.SOCKET BROKE IN HAND-LACERATION & FRACTURE TO LEFT INDEX FINGER.

EMPLOYEE WAS RETRIEVING A PIECE OF STEEL. AS THE HAMMER CAME DOWN, IT MISALIGNED WITHT HE COMPLING. THIS CAUSED THE HAMMER TO HIT THE TOP OF HIS LEFT FOOT.

WALKING DOWN SLIPE TRACKS OF DOZER DROPPED DOWN OVER END OF SLOPE IN FAST MOTION CAUSING HIM TO SLUNG BACK AGAINST DECK OF SEAT.

EMPLOYEE WAS SHARPENING A DRILL BIT, WHENT HE GRINDING SHEEL BROKE. A PIECE OF THE WHEEL STRUCK HIM ON THE LEFT THIGH. THIS RESEULTED IN FAINBY DEEP WARNED WHICH REQUIRED SERVE SUTUMES. D WHICH REQUIRED SEVEN SUTURES.

EMPLOYEE STATED THAT HE WAS SHARPENING A BIT WHEN THE BIT SLIPPED OFF THE GRINDING WHEEL. THIS CAUSED HIS FINGER TO COME IN CONTACT WHITH THE WHEEL THUS CUTTING THE CORNER OF HIS LEFT INDEX FINGER.
