EE was assisting the plant welder, with fabricating brackets for the bin gate clam shell. While using a drill press and grinder small metal particles collected on the sleeves of the EE shirt. EE wiped EE's face with EE's sleeve contaminating EE's eye with the small metal particles. EE did not seek medical treatment until 5/12/2026.

IW was dismounting the adjustment ring with EE's right hand on the top locking post when the feed cone swung and pinched EE's right middle finger between the locking post and feed cone, resulting in a fracture and laceration to EE's right middle finger.
Employee was climbing down the mounting ladder of EE's yard loader when EE missed the last step and fell to the ground. EE attempted to catch self by extending EE's left arm which resulted in a fracture to EE's left wrist.
Employee was working in a 5-man team to screw bowl of crusher in place. While exerting force with EE's arms EE's right shoulder was strained.
EE struck a ceramic impact panel with a hammer in an effort to dislodge it when a piece of the ceramic went under EE's glasses and entered EE's left eye.
EE was using a battery powered drill when the drill bit got caught on the steel causing the drill to quickly spin around. This caused the bottom side of the drill to flip and impact EE at the top of their safety glasses, resulting in a laceration above their right eye.
Employee was helping remove a panel from a screen when the screen panel jerked their arm down and over extending upper arm causing pain in shoulder. The incident occurred on 6/11/2025 but the employee's shoulder became worse over time and they had to receive surgery on 6/23/2025.
An employee sustained a small fracture and laceration to the tip of EE's right pinky finger. EE was lifting a return roller onto the top of a conveyor belt to stage it until needed. EE pinched the finger between the roller and the belt.
The employee climbed onto assigned loader to initiate the pre-shift inspection. While climbing down they thought they were on the bottom step when they were actually on the second step. When they released to touch the ground they fell further than anticipated causing them to fall to the ground. They suffered a fracture to the left wrist.
While working on a de-watering pump the employee grabbed the v-belt of the pump and pulled on it to see if the pump was free of the obstruction they were clearing. The belt rotated and pinched the employee's left pinky between the belt and the pulley.
The employee pinched the end of their right thumb between a bolt head and the recessed hole it was in. This occurred when the bolt spun when an impact wrench was applied to the nut on the other end unexpectedly.
Employee was attempting to cut a rubber curtain with a razor knife when employee slipped and cut right forearm.
Employee was pushing upward on a bolt head to attempt to lock it into position so the nut could be removed with an impact wrench. The employee unintentionally energized the tool causing the bolt head to spin in palm and causing a laceration.
The employee was adjusting a cone crusher using controls contained in a metal control box. The metal lid was dislodged by the wind causing it to fall and strike the employees finger. The contact resulted in a fracture of middle finger.
The employee stepped on a metal bracket to climb on to a conveyor frame EE was disassembling. The weld holding the bracket on failed causing the employee to fall approximately 3.5 feet. While falling EE attempted to grab the conveyor frame and lacerated left palm. The laceration required stitches.
Employee was removing a tooth from an excavator bucket and was using a hammer to break it loose. A piece of metal struck EE on the chin causing a laceration injury.
While attempting to remove a drill bit from the bit basket employee's left index finger was lacerated.
Employee was using a utility knife to cut a zip-tie from a cable. When the zip-tie was cut the momentum of the knife caused it to strike EE's opposite arm causing a laceration to EE's forearm.
Employee was using a knife to pry open a dust collection cartridge when it slipped and cut left thumb.
Employee was climbing back onto EE's haul truck when EE slipped from the first step back to the ground. Its now known that this caused a small tear in EE's right achilles tendon. The diagnosis and treatment plan occurred on 1/7/2020.
The employee used the back edge of pocket knife blade to remove the cap from a bottle of chalk for a chalk line. When the cap released, the front of the blade cut right thumb. The laceration required 3 stitches.
The injured employee was working as part of a team installing an I-beam on a construction project. While attempting to move the beam in place with a pry bar, the beam shifted downward and out striking the employees left thumb. It resulted in a fracture and laceration.
Employee was welding on a slide wrench. Employee had welding shield down and didn't notice the slide starting to move. Slide wrench fell on employee's left pinkie finger splitting open top of finger requiring 5 stiches. Employee was wearing thick welding gloves and proper PPE at time of incident.
The mechanic was removing the driver side platform from a haul truck. The platform was secured to the shop overhead crane. When the last bolt was removed the platform rotated and the edge of the platform cut the top of the mechanic's head.
While shoveling rock as part of stemming for a shot the employee strained back.
The employee was attempting to remove a hose with a wrench when ee strained left hand.
While dismounting the shovel the employee slipped from the last step (18" high) causing EE to roll EE's left ankle.
While the employee was moving a 55 gallon drum of oil, the contents shifted and caused the drum to stand up unexpectedly and slammed against an adjacent barrel. The employees left pinky finger was pinched between the lids of the two barrels. The finger flesh was removed from the nail to the tip.
While lifting a worn screen panel the employee strained the lower right side of back.
After completing work with an excavator, the employee was dismounting the parked machine. As employee stepped from the step to the ground employee felt a pain in right shoulder with which employee was holding the hand rail. The employee did not see a doctor until 02/18. Employee submitted to an MRI and results were read on 02/19/18. Employee has surgery planned for 02/22/2018.
The employee opened a gate for a contractor to leave. The contractors trailer became entangled with one side of the gate causing the trailer to jump sideways striking the employee who was holding the other side of the gate.
The Pit Foreman, was discovered in the restroom unresponsive and without a pulse.
While sliding a screen panel into place the employee strained lower back.
One employee was tossing down a 100' tape measure to another employee who lost it in the sun and was struck in the face. The employee fractured a small bone in the nose. No work time has been missed. The diagnoses of a need for medical treatment was made on 06/26/17. Repair procedure has not yet been scheduled.
TECHNCIAN WAS ATTEMPTING TO REMOVE/REPLACE A WORN BUCKET CUTTING EDGE. USING A WRENCH TO HOLD THE RETAINING BOLT, THE TECH ATTEMPTED TO LOOSEN THE NUT WITH A PNUEMATIC SOCKET DRIVER. THE BOLT AND WRENCH TURNED IN TECH'S HAND. TECH WAS WEARING GLOVES AT THE TIME OF THE INJURY
An employee was standing on a belt conveyor that was locked and tagged out. Another employee removed the tag and lock and started the belt. The employee jumped from the belt and onto the catwalk. EE landed on hands and knees and sprained right wrist. Employee's first doctors visit was on 7/26/2016.
While lifting a roller associated with a conveyor the employee injured EE's back.
While loader operator was digging into muck pile to load haul trucks, the loader struck hard ground in the form of a high wall toe, jolting the loader and rocking the operator. The operator sustained a pulled muscle in his neck, shoulder, and right arm area.
Employee cutting wire mesh with shears when shears slipped and employee cut his finger on exposed wire mesh.
When turned on, the air bubble in the washdown hose caused the hose to twist in the hand of the employee and the nozzle attachment band cut the employee's right hand at the base of his thumb.
While lifting primary crusher toggle for repair, lifting device broke and employee fell approx. 3-4 feet on top of falling hydraulic toggle.
