Employee was dropping ventilation tubes in the 2 South Mains section when an object bounced off the tube and struck the employee in the right eyeball. The employee was diagnosed with an open globe eye injury.
Employee passed out while looking for tool on floor of transfer tower at crabapple slope. Cause of due to pre-existing medical conditions with BP & heart. Bruised ribs were result of falling over. First aid, no treatment. EE fell over due to light head. Immediately conscious. EE could immediately return. Having diagnostics for pre-existing medical condition.
Employee was checking for leaks on the dozer. EE's left hand was between the door and frame when the wind blew causing the door to shut. It caught EE's left thumb causing a laceration.
Employee was walking in a seal rehab area, stepped into a hole obscured by rock dust, and felt pain in right calf. Injury was not the result of a training or compliance issue.
Employee was grinding rip clips on the bottom of the belt with angle grinder for approximately 30-40 minutes. Employee was wearing safety glasses during operation grinding. Employee and Supervisor both noted that high winds contributed to this accident.
Employee was bolting along the Continuous Miner located in the 2 South Left Mains #2 Entry, when they were walking over a pile of coal and felt a sudden pain in the left calf. Employee was diagnosed with a torn calf muscle.
EE was at the first tire of the rear axle on drivers side. As EE was directly facing the tire, EE bent knees and squatted down to remove the wheel chock. EE reached approximately eight inches directly in front of self with right hand to pick up the wheel chock. EE was unable to stand up. EE felt a sharp pain in the lower back and went down onto EE's knees.
A Bailey Prep Plant Employee was filling a reservoir for the pumps at the load out facility, when Employee stepped down in between two pallets and twisted right knee. Employee became a lost time on 11/22/2019.
A fatality occurred due to natural causes, specifically a cardiac event, while employee was working at a surface transfer building. Confirmation of the cause of death was provided by the Washington County, PA Medical Examiners office.
Employee suffered an medical emergency that resulted in untimely passing. Medical treatment was provide at the surface facility until EMS personnel transported ee to a hospital where ee was pronounced dead.
Employee was kneeling down to splice belt in the E30 section, when EE felt something shift inside the right knee. EE was diagnosed with a Torn Meniscus to the right knee on 09/18/2018 and began missing work.
After helping to secure a ventilation tube to the mine roof in the G1 Headgate section ee turned to walk away and twisted left knee. Resulted in a strained left knee.
Employee was working in the 2 East Mains Section. The fire suppression on a scoop outby in the section inadvertently actuated releasing Foray ABC Multi-Purpose Dry Chemical that is used in the system. The employee had a respiratory reaction (Reactive Airway Disease) that affected breathing.
On 12/30/15 the employee was lifting an I-beam injuring their back and began missing work on 1/11/2016 which was reported on a 7000-1. On 10/1/2017 the employee was in the locker room getting ready for their shift. While retrieving work boots they re-aggravated their back and started missing work again on 10/2/2017. This 7000-1 is being submitted to abate citation (9077226).
On 7/30/2016 the injured employee exited a loading machine ee was operating, turned towards the outby end of the machine and stepped over the cable when ee felt pain in left knee. On 3/14/2018 ee had a partial lateral meniscectomy to repair the knee and began missing work.
While walking on loose gravel in Oak Springs yard, employee felt pain in right knee. LTA 6/7/16.
Was found on 11/08/15 at 12:43 pm by a security officer. She was unconscious and unresponsive.
Employee was walking toward the miner when he stepped off the coal pile and felt pain in his lower back. LTA as of 5/4/15.
Employee was struck on the right ear with a roof strap while attempting to clean off the top of a miner causing a laceration.
Two tram operators were operating trams headed toward 9 Vent bottom when they seen the victim laying next to the track rail. He was unresponsive and had no pulse. CPR started immediately. Cause of death was probably heart attack. The victim had no signs of injury on his body.
Employee was working at a desk in the office at the Horn Mt. Portal of the Buchanan 1 Mine on 06-29-14. At about 8:30 AM she started feeling sick and left at 10:30 AM. She went to the doctor and it was determined that she had a urinary track infection. She returned to work on her next scheduled day to work which was 07-01-14.
On 3/29/14 said his left arm was numb and he was having chest pains. Supervisor reported to site. EE refused an ambulance to be called and said it was not job related. Supervisor observed EE before he departed site by driving. His mother was going to meet him and take him to the ER. He was asked to provide a medical excuse. EE is not schedule to RTW until 4/1/14/. No lost time.
Security officer was sick and hurting on her right side. Her supervisor was concerned she might have appendicitis. Security officer took her to the doctor. This was not a workplace injury. She has a herniated disc in her back that was causing the sharp pain in her right side making it look like an appendix problem according to her dr. Her dr gave her a slip to RTW on 2/6/14.
AT APPROXIMATELY 10:45 AM AN EMPLOYEE BECAME ILL AND COLLAPSED WHILE ASCENDING A SCAFFOLDING AT THE BAILEY PREPARATION PLANT AREA. HE WAS ATTENDED TO BY PERSONNEL ON THE SCENE AND TRANSFERRED TO AN AMBULANCE AND TRANSPORTED TO WASHINGTON PA HOSPITAL WHERE HE WAS PRONOUNCED DEAD. CAUSE OF DEATH IS CURRENTLY UNKNOWN PENDING AUTOPSY RESULTS.
Employee had cement dust blow into eye.
While on duty on the surface of Vent Shaft# 12 of the Consol Buchanan Mine on 07/26/2013 at 2056 hours, EE experienced numbness in his legs while sitting in his vehicle. He requested that an ambulance be called. The ambulance arrived at 2130 hours and he was transported to the Clinch Valley Medical Center in Richlands, VA.
An employee was found unresponsive underground in the J-bleeder area and was subsequently pronounced dead as a result of Natural Causes.

He was walking from the miner to the loader when he felt pain in the back of his right knee. *We dispute the allegations in Citation #8049156 and submit this report solely to terminate the citation.

The mine has experienced a death in the 1 East Bunker Control room. Employee was sitting in a chair in the bunker control room when his lost consciousness and stopped breathing. EMS was notified, transported to the hospital and later pronounced dead by the coroner. Cause of death Cardiac Infarction. This is not a mine related injury. MSHA report no contributing factors.
Bitten by stray dog on mine property, bite to right forearm. Employee was getting lunch out of vehicle when dog approached.
Unknown, accident was not reported by the employee
EE got off the mantrip and started walking,his back started hurting.
EE was performing maintenance on a hyd shovel. Wind was gusting at times. Around 3:00 a piece of grit was felt in the eye. It was washed out with water and felt OK. That night the grit was felt again. All attempts to remove didn't work. Went to hospital and had it taken out. Eye protection being worn.
Employee was walking up a slight grade in the #2 entry, F-19 section and felt a pain in the back of his left knee. He continued to work and began missing on 7/16/09 for surgery.
Employee was exposed to coal dust in his eyes. The employee had a cold in his eyes prior to working.
Employee was walking around the tail end of a conveyor belt. When he bent over to get around the "X" brace that crosses over the catwalk, he felt a sharp pain in the middle of his lower back. "X" brace was about 3 feet above the catwalk.
EE had a heart attack while working. A clot was removed and a stent was put in. Not work related.

While loading a belt drive motor the employee was struck by an unknown flying projectile in the right bicep causing a contusion and a puncture wound. The physician had to open up the wound to determine the extent of the damage which required staples to close.
Underground Inby. Door blew open and pressure caused an Omega block to fly out and hit Employee in back of head.

The injured EE was struck above left eyebrow by a foreign object, while lugging steel ties to track rail. An attempt to remove object was made by physician. EE had 7 stitches to close wound made by the physician.
EE LIFTED LEG AND CROSSED OVER OTHER LEG TO REMOVE HIS WORK BOOT, CAUSING HIS KNEE TO DISLOCATE. EE WAS IN THE BATHHOUSE WHEN THIS OCCURED.
Wind blew door shut on finger.
EE WAS WEARING LEATHER BOOTS, WORKING IN WATER THAT WAS ANKLE DEEP. EE COMPLAINED OF PAIN IN HIS LEFT FOOT; LEFT GREAT TOE BECAME INFECTED. ACCIDENT REPORTED 7/26/05.
VICTIM WAS IN THE MINE WHEN HE SUFFERED A HEART ATTACK AND DIED. DEATH WAS LISTED AS NATURAL AND IMMEDIATE CAUSE WAS ATHEROSCLEROTIC CORONY ARTERY DISEASE. ALL EVIDENCE, WHETHER PHYSICAL, OBTAINED BY INTERVIEWS, OR BY DOCUMENTATION SUGGESTS THAT THE DEATH WAS NOT MINING RELATED.
