Prior to the beginning of Team Members regular shift, they experienced a non-work related acute on-set medical event that resulted in their death. This is an immediately reportable accident not a work related injury or illness.

Employee was shoveling off the under belt to do some repair work. Lead man went to help and check on employee. Employee was acting funny, confused, did not know location and was sweating. It was 62 Deg F. Lead man called an ambulance and the safety manager.

Miner was experiencing shortness of breath and chest pains while operating loader. After notifying tower operator of symptoms, miner drove themselves to hospital where they were admitted and treated for a heart attack resulting from 95% blockage in one of the main arteries related to personal medical condition.

During welding activities on a catwalk, the employee strained knee when repositioning self while kneeling.

Driver was climbing down parked haul truck at end of shift when EE was stung on the left pinky finger by unknown insect that became symptomatic the following shift and resulted in an infection.

Employee believes he was bit by a spider on 06/18/15 while turning on water pump. Employee did not inform supervisor of bite until 06/19/15 employee did not want to receive medical treatment. On 6/23/15 the location had become infected employee was sent to receive medical treatment. Employee has been and remains on Full Duties.

An individual (Non-Miner, Non Employee) died of natural causes.

Sometime between the hours of 1200 am and 6:00 am an unknown male between the age of 25 and 35 committed suicide. The victim used a self-purchased rope and entered onto the mine site, where he secured the rope to a railing on the screen and walked below and committed the act of suicide.

EE said he bent over to go under the sill leg at the S-60 when he heard a "pop" in his back, he sat down, said he'd be alright, minutes later he said he was going home to rest his back, left the Quarry at 10:47am. He called the Quarry at 11:20 in extreme pain he was at the C.I.M.C., a nurse took him into the Hospital, failed to file report before leaving on his own.

EE had the inspection door raised over the hydraulic tank in the upright position. A gust of wind blew the door down and struck him on the tip of his middle finger on left hand crushing and breaking the bone in the tip of his finger.

No specific working condition caused employee to have a seizure, he has an existing condition that causes seizures. He was traveling within the Quarry when he became confused, a co-worker was with him at the time, and helped him until he became alert again.

A bee came in through the window and stung employee on right calf. A couple of days later sting became infected.

BENDING KNEE TO GET IN POSITION TO WELD ON PLANT.

EE WAS HELPING A LOADER OPERATOR MOVE A CONVEYOR IN THE PIT. EE WAS THE GROUND MAN AND SPOTTER. EE SAID HE BECAME VERY DIZZY AND THEN VOMITED. EE HAD TO LIE DOWN FOR SEVERAL MINUTES.

EE was removing his overalls at the end of work shift. He noticed his right middle finger was hanging at the first joint. Does not know how it occurred.

This day there were high winds at the pit. sand and other material were plowing around the area. The operator got out of his rock truck and got dirt and rock in his eye. He did not have safety glasses on but he did have them around his neck.

INSECT BITES WHILE OPERATING LOADER IN STOCKPILE AREA.

WHILE MAKING REPAIRS ON VERTICAL SHAFT CRUSHER, A STRONG WIND BLEW DIRT INTO EE'S EYE (RIGHT).

A SILVERFISH INSECT CRAWLED INTO EMPLOYEE'S EAR AT THE TIME HIS EAR PLUGS WERE OUT.

LEANING DOWN LOOKING AT PIECE OF FEEDER HAD A SHARP PAIN GO THROUGH MIDSECTION AND TRIED TO STAND UP AND COULD NOT.

THIS PARTY WAS TRANSPORTED FROM HIS WORK SITE (COAL TRUCK) TO THE NEAREST MEDICAL FACILITY BY A FOREMAN. HE COMPLAINED OF CHEST PAINS AND BREATHING DIFFICULTY. MEDICAL EXAMINATION AND TESTING DETERMINED HE HAD SUFFERED A HEART ATTACK.

STUNG BY A BEE. LEFT SIDE OF FACE SWELLED.

WHILE OPENING THE ELEVATOR DOOR, CEMENT FLUSHED OUT. EMPLOYEE WAS KNOCKED OFF A PLATFORM AND INHALED CEMENT.

EMPLOYEE REINJURED HIMSELF. CO. TRIED TO CONNECT IT TO PRIOR INJURY.

SECOND INJURY, AGGRAVATED PREVIOUS INJURY. WHILE COMING DOWN FROM LADDER, MISSED STEP & TWISTED BACK. STANDING ON LADDER INSIDE MAGAZINE WORKING ON CEILING.

EMPLOYEE WAS CRAWLING OUT OF THE 657 SCRAPER WHEN THE WIND CAUGHT THE DOOR AND JERKED HIS ARM OUT OF PLACE.

DUST AND LOOSE MATERIAL WITH SWEAT & RAIN (CLINKER DUST SKIN IRRITATION).

INJURED WAS CLIMBING INTO A FIAT ALLIS 745 FRON END LOADER WHEN STRONG WIND BLEW THE DOOR SHUT WHILE HIS HAND WAS IN THE DOOT JAM AREA.THE DOOR STRUCK AND PINCHED THE LEFT INDEX FINGER BEHIND THE FINGER NAIL AREA,CUTTING THE END OF THE FINGER OFF.

INDIVIDUAL WAS 9EMOVING RIVETS ON A DRAKE SHOE AND BENT OVER TO PICK UP A HAMMER WHICH HAD FALLEN TO THE FLOOR AND INJURED HIS LOWER BACK

STANDING ON DITCH SIDE SLOPE EMPLOYEE BENT OVER AND HIS RIGHT KNEE GAVE OUT DAMAGE CONSISTED OF STRAINING A LIGAMENT IN HIS RIGHT KNEE
