Left area where he was cleaning chutes & went to control room reporting this to another empl. Initial treatment was for heart attack, which was ruled out by normal EKG. Underwent further testing for what has been deemed as a personal condition.

OPERATOR WAS CUTTING OPEN OXIDE BAGS CREATING SOME DUST, CAUSING HIM TO SNEEZE. WHEN SNEEZED, HE FELT SOMETHING (PAIN) ON HIS LEFT SIDE OF LOWER BACK.
WHILE DESCENDING A FIXED LADDER, WASPS FROM A NEARBY NEST FLEW AT EMPLOYEE. HE QUICKENED HIS PACE AND POSSIBLY MISSED A RUNG OF THE LADDER, THENFELL TO THE GROUND.
EE EXPERIENCED A MASSIVE HEART ATTACK CAUSING IMMEDIATE DEATH.
EE WAS INSPECTING COOLER COLLECTORS. A LARGE GUST OF WIND BLEW THE DOOR OF THE COLLECTOR CLOSED THEY HAD BEEN HELD OPEN BY DOOR STOPS. THE DOOR STRUCK EE LEFT ARM AS IT SWUNG CLOSED. INITIALLY DIAGNOSED AS BRUISED WRIST FOREARM BUT LATER X RAY SHOWED FRACTURE OF ULNA. EE PLACED ON LIGHT DUTY.
