EE was moving a mantrip from the main line to put on charge at the charging hole. He was facing the wrong way and had his right leg hanging outside the mantrip when he rounded a corner en route to charging hole where his right leg got caught between the frame of the mantrip and the rib causing injury.

Pinched finger between motors

Approx 10X12' section of haul road collapsed into an abandoned underground miner entry. The right rear tandem of EE's 730 E Dresser fell into void created by this collapse. Truck driver thinks that he cut his finger while dismounting truck afer incident occurred. Note: 3 sutures were placed in the small bliter like flap of skin.

While dumping load from truck bed, the bed fell off when raised to full extension of the first jack.

Employee was in the process of dumping his 16th load of rock at dump area, when the truck bed fell approx 2'. The bed did not hit the truck frame but bounced back up to a fully extended position. Employee stated that the jarring motion of the truck caused a "whiplash" type injury to his neck.

Employee was in the process of hooking up a chain from the section pressure pump to the scoop ram when his right foot was caught under the scoop bucket. Employee was diagnosed with 2 fractured metatarsals on the right foot.

EE stated, while his truck was being loaded, a rock rolled & struck the inside of the bed of his truck & caused a severe jolt to his neck & then when he went to dump the load, felt pain in neck. When he returned to the loader, he contacted his foreman & requested an injury report be filled out; he completed his shift & drove himself home.

EE's were in process of cleaning sumps and ponds along haul road with A35C Volvo articulating trucks. Stopped for lunch. After lunch injured EE backed truck from the area and traveled approx 110' backwards, striking a road berm that caused truck to roll over the embankment approx 24'. Transported to hospital. Diiagnosed with fractured ribs, clavicle, vertebra, punc lung.

End loader lunged backwards, throwing employee forward against the seatbelt. Employee was diagnosed with a strained abdominal muscle. NOTE: EMPLOYEE DID NOT BEGIN MISSING WORK UNTIL 8/21/07.

A large truck carrying a big drill came around the curve on employee's side and he had to go in ditch to avoid a collision.

Employee stated he was traveling uphill when his truck powered out, causing him to drift backwards. He cut toward the bank which caused the trailer to flip onto its side and the tractor vertical.

Employee was a passenger in a pickup truck being driven by a fellow employee. They were returning from the loadout to the plant after loading a train. The pickup struck a berm and rolled on its right side. Employee received lacerations of the right arm from the accident.

Employee attempting to shift gears going up grade with truck. Got it out of gear. The truck rolled backwards into a ditch and turned on its side. Employee got out, noticed a fire starting in engine compartment. Used fire exinguisher but did not get fire undercontrol.

Employee was drivng truck down mine road. Lost control and jumped from truck. Vehicle rolled over onto side. Employee did not see Dr. until 5-28-07. 3 days restricted duty. Returned to full duty on 6-1-07.

Employee was riding on top of battery compartment on scoop & was thrown against mine roof.

Employee jumped off mantrip before it collided with another mantrip.

Employee was getting out of mantrip when the operator started tramming forward. The movement of the matrip caused EE to fall to mine floor.

A rock slid & struck the inside of the bed. EE had on his seat belt & the belt pulled on the stitches from non-work related hernia repair some 7 weeks earlier. He was taken to hospital for evaluation. Dr saw no significant damage to repair but suggested follow-up with initial Dr. Only time lost was due to wait to see Dr. The back complaint is new. Citation will be contested.

Employee stated that roads were ice and snow covered. He stated that as he was hauling from pit to cleaning plant. He approached a grader as he was moving through a curve. He stated that as he de-accelerating, the truck started sliding and hit the grader.

Employee struck the rib while operating a scoop. He claims to have injured his back.

EE was driving back to pit to get another load and ran into the ditch.

Employee stated that while driving up a hill, the truck stalled. The truck then drifted back into a ditch, turning over on its side.

EE jammed his head into a scoop canopy while hauling block. He had run over a track rail when this occurred.

Miner drum slid out of scoop bucket and hit employee on his foot(R).

While shoveling a belt, employee was struck by a belt scraper. He then fell against the rib & his rescuer.

Employee was driving truck up hill. Truck started rolling backward and turned over onto it's side. Employee treated at ER - prescribed light duty.

Employee stated that he fell asleep, ran truck in ditch; truck tipped over on side.

Employee claims to have injured his back while operating a Cat 773 Rock Truck.

Employee claims to have injured his back when the scoop that he was operating trammed over a crib block causing him to bounce into the canopy & back into the deck.

EE stated that truck was climbing the hill. The truck stalled and then the truck rolled backward into the berm and turned over on its side.

Driver entered right hand turn on haulage road. Driver claimed the load shifted in his trailer and overturned the tractor & trailer. *Amendment: Per call, did not happen on Remington Prep Plant property. Not reportable to MSHA because it did not happen on any mine property.

Employee struck his head on the shuttle car canopy when he hit a hole in the roadway.

Employee was working on a drive shaft on a mantrip. The mantrip moved & his hand was injured.

EE was attempting to remove a broken belt roller. The belt roller came into contact with the belt & then struck the employee in the face.
