While treating mine water at Outfall 2, hydrated lime in the hopper escaped through a gap at the flow adjustment plate and hit the employee in the eyes. Employee immediately began flushing eyes with bottled water. Due to severe irritation, the employee was taken in for medical evaluation.

Employee was driving a club car in the quarry when they reached for their radio releasing the steering wheel, which caused the wheel to spin hard to the left. When the employee attempted to correct the steering wheel, they also pressed the brakes. This caused the mobile equipment to ascend the berm and tip over on the passenger side.
The Dryer operator was using a mini skid to perform general cleanup of the Dryer Building. While traveling in reverse, they pinched their little finger between the mini skid and an I-beam cross member.
The operator said the injury came from the jerking motion of the loader when in operation.
2 Haul trucks were traveling in opposite directions down and up a ramp in the dump. As they met on the ramp the driver immediately pulled off to the side to avoid contact. One truck started to slide when the brakes were applied causing a collision. Employees were wearing seat belts, however, driver of truck #2 received a laceration above the eye requiring three stitches.
The loader operator stated their back was hurting from operating the loader. The soreness and pain was located in the lower back.
The left front tire blew out on ME-02 loader. The worker jarred EE's back when the tire blew. The incident occurred 2/10/22 did not seek medical attention until 02/21/22
While driving a Komatsu WA500 Loader, an employee drove over a rough road and bounced in EE's seat, injuring EE's shoulder. When EE bounced, EE caught self on the way down with EE's R arm on the arm rest, which ended up hurting EE's R shoulder. The employee said speed was not a factor.
The production operator transported a hot cooler breach hopper. When the production operator let their gloved left hand down. The sand flowed into the glove causing second degree burns on the left hand.
Employee was loading a 773 haul truck with the 988K Cat. loader. While loading ore into the truck, a rock slid over the back of the bucket down the arms and into the cab crushing the dash into the operator's ankle.
