Employee was driving service truck thru pit on haul road. EE drove into a large water formed pothole and jarred back.
Trespassing vehicle traveling at an apparent high rate of speed ran into the barricades at pole line 4160 volt. Damage to vehicle and 3 locations along pole line. (Pole, 2 cross arms, and cross arm).
Emp. was backing up ramp in truck to dump load. Emp. got to close to Rt side of ramp. bank gave way under weight of truck & slide sideways down bank & then rolled onto roof aprox 30 feet.
EE was washing undercarriage of 2006 Ford F250 pick up truck when cable slipped off the JCB forklift.
Employee was operating bucket truck to weld on boom of Demag. When finished welding and lowering bucket, employee somehow caught himself between bucket and boom of Demag.
EE leaned out window to adjust direction of water cannon and felt pain in abdomen.
Employee drove through pot hole. Steering wheel spun and struck finger.
Several miners were assigned to clean the #6 belt line. When the conveyor was started one miner continued to clean around the head pulley with a 4' bar. The bar was pulled into the pulley & the end of the bar struck his right hand causing injury.
WHILE OPERATING TRUCK EMP EXPERIENCED BACK & LT LEG PAIN.
EMPLOYEE PARKED WITH ENGINE RUNNING AND DOOR OPEN. WHILE WALKING FROM TRUCK IT DRIFTED AND HE CHASED IT SLIPPING ON SNOW. TRUCK RAN OVER LEFT ANKLE AND FOOT WITH LEFT FRONT TIRE.
MAN WAS GOING DOWN RAMP INTO PIT WITH WATER TRUCK, HE SAID TRUCK ENGINE SHUT OFF AND TRIED TO TURN TRUCK INTO HIGH SIDE OF ROAD TO STOP, BUT TRUCK RAN UP HIGHWALL AND FLIPPED INTO ITS RIGHT SIDE, MAN HAD SEATBELT ON.
EMPLOYEE FELT SOMETHING IN EYE. AS HE RUBBED EYEIT BECAME IRRITATED. GOT EYE DROPS.
OPERATOR NOTICED SMOKE BEHIND CAB. WENT TO GET FIRE EXTINGUISHER AND ENTIRE CAB CAUGHT FIRE. EE JUMPED FROM CAB TO THE GROUND.
EE WAS DRIVING COMPANY TRUCK INTO PIT AND HIT A BOUNCE CAUSING HIM TO HIT HIS WATER JUG AND BRUISE HIS RIBS.
