Mostrando los 41EMPLOYEE STARTED MISSING TIME 9-12-03 FROM CHRONIC BACK AND NECK PAINS SINCE AN ACCIDENT ON JAN 24, 2003. ON THAT DAY EE WAS LOADING ROOF BOLTING SUPPLIES FROM A SCOOP WHEN HE FELT PAIN IN THE LOW BACK AND NECK.
EMPLOYEE WAS HAULING COAL FROM THE MINER WITH A COAL HAULER JUST AS HE LEFT THE MINER HE RAN OVER A LEDGE WHICH CAUSED HIM TO BOUNCE UP INTO THE CANOPY. HE WORKED UNTIL 8-19-02. THE DR TOLD HI M TO START MISSING WORK ATT THAT TIME. THE ACCIDENT BECAME REPORTABLE THEN.
2 MECHANICS & SCOOP OPERATOR REPARING A BROKEN CONVEYOR CHAIN ON MINER. USING A SCOOP AND STEEL CABLE TO PULL BROKEN CHAIN THROUGH BOOM OF MINER. THERE WAS A CATCH IN THE CHAIN AND WHEN IT REL EASED, THE CABLE FLEW UP AND STRUCK THE EMPLOYEEON BOTTOM OF HAND. AN EXAMINATION WAS RECOMMENDED AND THE X-RAY SHOWED A SMALL FRACTURE IN LITTLE FINGER(LEFT HAND). NORTH MAINS SECTION
THE EMPLOYEE WAS OPERATING SC AND HIT A HOLE IN THE HAUL ROAD, WHEN THE SC HIT THE HOLE, THE OPERATOR BOUNCED FROM HIS SEAT CAUSING HIS HEAD TO HIT THE CANOPY. THE X-RAY SHOWED A BRUISE AND CHIP TO THE 6TH VERTEBRA. THE FOLLOWING DAY HE WAS RELEASED WITH A NECK BRACE.
EE WAS RIDING A MOTOR OUTSIDE. HE RAISED UP TO SEE THE TRACK AHEAD. WHEN HE RAISED UP HIS HEAD CAME IN CONTACT WITH THE TOP. DR.'S REPORT STATES THIS IS A NEW INJURY.
EMPLOYEE WAS TRAVELING FORWARD IN A SCOOP FROM #3 ENTRY TO #2 ENTRY TO PUSH OUT A LOAD OF SUPPLIES WHILE TRAVELING FORWARD. HE WAS TRAVELING BACK TO THE #3 ENTRY IN REVERSE IN LOW COAL CONDITI ONS WHEN HIS HEAD CAME IN CONTACT WITH THE MINE ROOF AND THE SEAT BACK REST. IT APPEARS THE EDGEOF HIS HARD HAT CAUSED A LACERATION TO HIS HEAD.
A SUPPLY CAR WAS LIFTED BY THE SCOOP BUCKET TO REMOVE CEMENT BLOCKS THAT WERE UNDER THE CAR. WHEN THE EE REACHED FOR A BLOCK, THE CAR SLID OFF THE SCOOP BUCKET. THE EE'S LEFT THUMB WAS CAUGHT BETWEEN THE CAR & A CEMENT BLOCK RESULTING IN A FRACTURE.
EE WAS HANGING "SLACK" CABLE BEHIND THE SHUTTLECAR ANCHOR- THE SHUTTLE CAR OPERATOR ENERGIZED THE CABLE REEL- THIS CAUSED THE CABLE TO SLAP THE EE IN THE HEAD.
EE WAS GETTING BLOCKING FROM UNDER SCOOP BUCKET WHEN OPERATOR ACCIDENTLY DROPPED THE BUCKET ON HIS LEFT HAND CUTTING THE END OF HIS MIDDLE FINGER OFF.
WHILE TRAVELING IN THE SHUTTLE CAR IT STARTED TO SLIDE TOWARD THE RIB. EMPLOYEE PUT HIS ARM OUT TO HELP PREVENT HIMSELF FROM FALLING OUT OF THE CAR WHEN HIS ARM BECAME CAUGHT BETWEEN THE SHUTT LE CAR AND RIB CAUSING A FRACTURE.
WHILE TRAVELING THROUGH THE MINE TOWARD THE SECTION EMPLOYEE'S HARD HAT BUMPED THE MINE ROOF CAUSING HIM TO STRAIN HIS NECK AND RIGHT ARM.
WHILE OPERATING SHUTTLE CAR EMPLOYEE RAN INTO A HOLD IN THE ROADWAY JARRING HIM AND STRAINING HIS LOWER BACK.
THE EMPLOYEE WAS OPERATING A SHUTTLE CAR. THE SHUTTLE CAR HIT A HOLE IN THE ROADWAY AND THE EMPLOYEE JAMMED HIS HEAD AGAINST THE CANOPY.
EMPLOYEE WAS TAKING HEADERS AND WEDGES TO FINISH STOPPING. HE COULD NOT MAKE A TURN SO HE START-ED TO BACK UP WHEN HE ROLLED FORWARD CAUSING THESTEERING WHEEL TO SPIN AROUND AND HITTING HIS LEFT THUMB.
EMPLOYEE WAS OPERATING SHUTTLE CAR TRAVELING THROUGH THE MINE WHEN HE ENCOUNTERED A DIP IN THE MINE FLOOR. HE BOUNCED UP INTO THE MINE ROOF, CAME BACK DOWN, HITTING HIS BACK ON THE SHUTTLE CAR SEAT.
INJURED EMPLOYEE WAS OPERATING DOZER PUSHING COAL INTO #3 SECONDARY FEEDER WHEN A ROCK FLEW OUT OF THE SECONDARY STACKING TUBE DOOR THAT FEEDS DIRECTLY INTO #3 FEEDER. ROCK WENT THROUGH THE D OOR GLASS AND STRUCK OPERATOR ON THE RIGHT SHIN JUST ABOVE THE EMPLOYEE'S BOOT CUTTING HIS RIGHT LEG REQUIRING 3 SUTURES.
EMPLOYEE WAS LOADING GLUE ONTO HIS SCOOP. AS HE TRAMMED ACROSS THE SECTION HE HIT A HOLE IN THE ROAD CAUSING HIM TO HIT THE RIB. HIS LEFTSIDE STRUCK THE SCOOP DECK. HE CONTINUED TO WORK UNTIL THE 23RD THEN WENT TO THE DR.
EE WAS MOVING THE MANTRIP TO PUT IT ON CHARGE. THE WHEEL HIT A ROCK CAUSING THE WHEEL TO JERK. THE STEERING WHEEL SPINNED LETTING THE STEERING KNOB HIT THE LEFT HAND.
EMPLOYEE WAS MOVING A MANTRIP THROUGH A CURTAIN. HE USED HIS LEFT FOOT TO RAISE THE CURTAIN. HIS FOOT GOT HUNG ON THE ROOF CAUSING FRACTURE TO HIS LEFT LEG.
7-13-01, EMPLOYEE REPORTED HE WAS INJURED 7-12-01. HE CONTINUED TO WORK. 7-16-01 WENT TO DR.. HE REPORTED THAT HE HIT THE RIB WITH THE SCOOP, BOUNCED AGAINST A LIGHT AND INJURED HIS RIGHT SHOU LDER.
WHILE TRAVELING THROUGH MINE IN SHUTTLE CAR, EE ENCOUNTERED A LARGE HOLE IN THE MINE FLOOR, BUMPING HIM AGAINST THE SEAT OF THE SHUTTLE CAR CAUSING PAIN TO HIS LOWER BACK WITH PAIN MOVING TO H IS RIGHT SIDE. LOST TIME BEGAN 6/4/01.
HE WAS RIDING IN A 2 PASSENGER PERSONNEL CARRIER, WHEN HE SLIPPED OR FELL OFF CAUSING THE PERSONNEL CARRIER TO RUN OVER HIS RIGHT FOOT.
HE ENTERED RAIL MANTRIP & SAT DOWN. OPERATOR ASKED IF EVERYONE WAS IN THE BUS. ANSWER WAS YES. HE STUCK HIS RIGHT FOOT OUT TO PUSH HIMSELF FURTHER INTO THE MANTRIP. AT THIS TIME BUS WAS IN MO TION. RIGHT FOOT WAS CAUGHT BETWEEN RAIL BUS ANDPIECE OF COAL. LACERATION (4 STITCHES) & BRUISE.
EMPLOYEE WAS DISLODGING MUD AND ICE FROM MOVING BELT CONVEYER WITH 5' PIECE OF REBAR WHEN THE REBAR BECAME CAUGHT BETWEEN THE IDLE ROLLER AND MOVING BELT CONVEYER DRAGGING INJURED THROUGH MOVI NG BELT CONVEYER AND IDLE ROLLER.
DRIVING A TRUCK - HIT A POTHOLE. BACK STARTED TOHURT REAL BAD.
THE ROAD WAS SNOW COVERED AND ICY. IT HAD BEEN ASHED RECENTLY. THE DRIVER STARTED DOWN THE HILL TO THE PLANT WHEN IT JUST TOOK OFF AND OVERTURNED. HE WAS CUT ON THE EAR AND HIS SHOULDER WAS BR UISED.
DUST FROM HAUL ROAD GOT INTO EE'S EYES 12/12/00 12/13/00 WHEN HE CAME BACK TO WORK HIS EYES WERERED & SWOLLEN 12/27/00 HE SAW DR. HE WILL RTW 1/2/01
EE STOPPED 1ST SC SO HE COULD GET BY. 2ND SC HEARD MINER RUNNING & THOUGHT 1ST SC THAT WAS STOPPED LEFT TO DUMP COAL. 2ND SC CAME AROUND CORNER & BUMPED STOPPED SC CAUSING IT TO TWIST STRIKING EE ON LEFT LEG EE'S LEG WAS CAUGHT BETWEEN SC & RIB
EE WAS OPERATING A SHUTTLE CAR. THE CAR HIT A HOLE WHILE ON TYHE WAY LOADED TO THE FEEDER. THE CAR BOUNCED CAUSING EE TO HIT THE CANOPY AND INJURE HIS NECK.
EE WAS INJURED WHILE PULLING LO-LO BACK INTO PLACE. HIS MBC JERRED CAUSING HIM TO BOUNCE AGAINST THE MBC.
EE WAS OPERATING A MANTRIP. THE FRONT WHEEL HIT A ROCK IN THE ROAD. THIS CAUSED THE STEERING WHEEL TO SPIN. THE STEERING KNOB STRUCK EE ON THE BACK OF RIGHT HAND.
EMPLOYEE WAS MOVING MANTRIP OUT OF BREAK TO GET OIL. FRONT WHEEL HIT RIB, CAUSING STEERING WHEELTO SPIN. STEERING WHEEL KNOB STRUCK LEFT HAND. X-RAY POSITIVE.
FLATBED SUPPLY CAR WENT THE WRONG DIRECTION AND INJURED COULD NOT GET OUT OF ITS PATH AND SUPPLY CAR STRUCK INJURED'S LEFT FOOT CAUSING IT TO BE LODGED BETWEEN CAR AND THE MINE BOTTOM.
EE WAS HOLDING CRIB BLOCK AGAINST FEEDER FLIGHT WAITING FOR CAR TO PULL UP AGAINST IT. EE OPERATING FEEDER (JOGGING CHAIN) WAS TOLD TO HOLD IT. FEEDER OPERATOR REVERSED FEEDER CHAIN CAUSING CR IB BLOCK TO GO UNDER BUMPER OF SC & CAUGHT INJUREDS HAND BETWEEN CRIB AND FEEDER FRAME.
EMPLOYEE WAS TRAVELING FROM 002 SECTION TO 001 SECTION. HE STOPPED HIS RIDE TO BACK UP. THE RIDE ROLLED INTO THE RIB. WHEN THE WHEEL HIT IT CAUSED THE STEERING WHEEL TO SPIN. THE STEERING KNOB STRUCK THE BACK OF HIS LEFT HAND. X-RAY WAS POSITIVE.
WHILE PULLING MINER WITH HYDRAULIC MINER PULLER, A SHACKLE PIN BROKE AND HIT MAN IN LOWER LEFT STOMACH AREA. (MAN WAS APPROX. 50' AWAY). NO VISIBLE EXTERNAL INJURIES/TRANSPORTED TO HOSPITAL.
ABOUT 4:30 PM EE WAS ON HIS WAY OUTSIDE TO GET SUPPLIES. HIS GLOVE FELL FROM THE SCOOP. HE STOPPED THE SCOOP AND LEANED OUT TO GET IT. HIS FOOT SLIPPED FROM THE BRAKE AND THE SCOOP ROLLED ONTO HIS RIGHT HAND. HE MASHED HIS MIDDLE AND RING FINGERS.
EE WAS IN BUCKET OF SCOOP. SCOOP OPERATOR BACKED OVER A ROCK. EE BOUNCED ONTO SCSR. X-RAY SHOWED ONE BROKEN RIB.
EMPLOYEE WAS OPERATING MANTRIP. HE HIT A ROCK WHICH CAUSED THE STEERING WHEEL TO SPIN. THE STEERING KNOB STRUCK AND BROKE HIS LEFT LITTLE FINGER.
EE WAS RIDING OUT OF THE MINE ON A MANTRIP WHEN A BUMP ON THE ROAD WAS ENCOUNDTERED CAUSING HIM TO BOUNCE UP AND WHEN HE CAME BACK DOWN HE LANDED ON A PIECE OF COAL IN HIS SEAT CAUSING A BRUIS E TO HIS LEFT HIP AND LOWER BACK PAIN.
