INJURED WAS MOVING HIS CABLE ANCHOR FOR SHUTTLE CAR UP. THE CABLE REEL TURNED PULLING ANCHOR INTO THE BATWING WHICH CAUSED THE CHAIN ON THE ANCHOR TO STRIKE INJURED ACROSS FACE.

EE was going to clean No.2 Entry when he turned the corner he hit an oncoming production scoop bucket to bucket. The impact of the collision caused the EE to be thrown into the top.
Employee stated that he was hauling a load of belt structure with a scoop when he ran over a large rock and hurt his neck and back. He continued to work until 4-9-13 when his Dr. removed him from work and scheduled surgery for 5-1-13.

The employee got off a fork and got on another forklift and backed into the forks of the other machine, the fork struck his left leg. He went to the doctor and had 12 sutures placed in the cut he received. He returned to work the next scheduled shift. Full duty.
