Was driving down pit ramp and got jarred when hit a soft spot. Did not report till after shift. Did not seek medical attention until 06/29/26. was given full release. follow up on 07/06/2026 was put on TTD.

Employee was loading a job box in the back of a service truck, job box slipped off forklift smashing left thumb.

Employee was helping to get a blade unstuck and when it slid in mud and jarred their back.

A haul truck operator was jarred, while being loaded by a loader, when the loader bucket made contact with the bed of the haul truck. On 1/16/2025, the employee was taken off of work due to continuous pain.

A safe rescue line attached to the back of an employee standing on the ground got entangled on a handrail of a Bulk truck while pulling through an ammonium nitrate silo. With the line being tangled, it got tight and jerked the employee off their feet and into the ground causing injuries to left shoulder and right bicep.

Employee was driving a boom crane out of mine traveling on the 8060 area of mine when they lost traction and the crane began to slide/roll back down the hill until it ran into the left-hand berm/hillside and tipped onto it right side. The employee was wearing a seatbelt but sustained a concussion, compression fracture in lower back, and multiple contusions.

Employee received a laceration that required two stitches when a rubber tire dozer and scraper made contact.

A haul truck operator was traveling at an excessive speed down a ramp when the haul truck encountered a containment channel. The truck hit the channel and bounced the operator in their seat, which caused a lumbar fracture in the lower back.

An employee's finger was contacted causing a fracture.

Employee was operating a scraper on the west Vantage dump when the vehicle struck a pothole or rut, jarred the operator's lower back. There was no equipment damage and the back sprain was minor but requiring an unspecified amount of restricted duty days.

Driving haul truck and hit some washboard. Steering wheel jerked injuring left wrist.

Employee felt pain in their hip when their haul truck bed was contacted by a loader bucket while being on loaded on 2/1/2018. Due to a reoccurring pain the employee was seen on 7/23/2020 and it was determined that they needed to work on restricted duty.

Employees shift ended at 7:00 p.m. and at 7:10 p.m. while employee was leaving the facility on personal motorcycle, employee made contact with an ecology block causing an accident. Employee received minor abrasions and a torn calf muscle in employee's left lower leg.

Driver inattention caused Haul Truck to strike berm. In the process operator struck head against inside right window. Sustained cut on head requiring stitches.

Miner was driving a haul truck. After the second load, miner says they hit a bump, causing back pain. The validity of the claim is in dispute.

While backing a haul truck into sample stockpile, operator ran over a large rock. This action resulted in truck bouncing and operator striking back of head on the AC post, which caused a laceration needing 13 staples.

Employee reached behind the seat to retrieve the passenger side seat belt when his finger came into contact with a sharp object causing injury to his right index finger.

The injured employee was driving a service truck down into the pit to work on a loader. While driving slowly (reported to be approx. 5MPH) due to wet and rough road conditions, the employee was jarred around and said truck's seat bottomed-out, hurting his back. A Licensed Health Care Professional diagnosed the injury as: #847.2 - Sprain, Lumbar Region.

An employee reported lime in the cab of a haul truck. The employee was driving the truck to the shop and began feeling burning in their lungs. The employee was transported to the RMGC Clinic and was released to full duty.

An employee inhaled lime dust in the cab of a haul truck. The employee was transported to Nye Regional Medical Center and was released to full duty.

The driver was backing down an access road to the stage 4 heap leach. While backing the truck lost power causing the air brakes to weaken/fail. Near the bottom of the slope the truck began to turn side hill and rolled. The driver was belted in but received minor laceration to the top of his head from striking it in the cab.

Per statements IEE who is an experienced haul truck op. was training another EE on a haul truck. EE was on 2nd day of training as he was backing up to a loader operator had the bucket raised so the truck could be loaded, as the truck got into position apparently the bucket was dropped quickly which jarred the truck and hurt his lower back. EE is on light duty.

EE was task training on a 777 F series with another EE as the passenger.He was backing haul truck under the Cat 993 loader when the haul truck was rocked violently. He states he was catapulted by the air seat into the headliner of the cab. Other EE was in the training seat and immediately his back started hurting.

An employee in a haul truck was being loaded by a loader. It is believed that the loader tire contacted the haul truck tire causing the employee to be jolted in the cab. This caused the employee to chip their tooth. The employee was transported to the dentist, where it was determined that the chipped tooth would require repair.

Worker pinched her right thumb in the door jam of a CAT 938G loader while opening the door to the cab. Worker was climbing into the machine when the injury occurred.

An employee was working the C-5 conveyor, replacing a return idler. A rock fell from the belt and contacted the employee on the mouth, causing one tooth to be chipped and one tooth to be cracked.

Employees were moving an eyewash station building with a fork lift. As the fork lift operator began lifting the building, an employee bent down to pick up an electrical cord. The building tipped over and pinned the employee against a concrete barrier. The employee was transported to the hospital, where it was determined that they had 2 pelvic bone fractures.

Operator drove front left tire of a ten wheeled gravel truck into a hole covered with ice causing the truck to roll on its left side.

A haul truck driver was beiginning an uphill climb in third gear. The excessive speed caused his CAT 785 seat to bottom out after cresting a hump. This resulted in a lower back strain to the driver.

A 950B Cat Loader moving from the upper portal to the lower portal lost it's brakes on grade and struck a parked pickup truck. The pickup truck was pushed over a miner causing injuries to her left arm and left leg. Please see attached report for details.

A haul truck operator experienced numbing pain in his neck after a large rock, along with smaller material, was unloaded from a shovel into his truck. The large rock caused jarring which contributed to this injury.

An operator of a CAT 789C haul truck backed onto a berm which gave way at a waste dump. The haul truck and operator traveled down the slope of the dump and came to rest upside down on a lower dump. The operator was wearing the seat belt at the time of the incident. The emloyee was treated for lacerations to the scalp and released the same evening from the hospital.

Employee working on belt turned to walk away and was struck with a small particle. Debris grazed cheek and went behind safety glasses. Employee became reportable when he received prescriptions for his eye.

Improper use of equipment. Personnel riding on equipment when they should not have been. Lack of proper guarding.

The employee alleges that he was sitting in a fuel truck. As he turned to engage the Power Take Off he strained his back. Management became aware that the alleged injury was a restricted work activity case on 2/16/05.

EE WAS BEGINNIGN TO TROUBLESHOOT A REPORTED STEERING PROBLEM WITH TRUCK, EE WAS SITTING IN SEATM RESTING LEFT ARM ON STEERING WHEEL WHILE REMOVING SHROUD SCREWS UNDER CONSOLE, WHEEL INDEPENDENTLY TURNED THROUGH2-3 REVS, LEFT ARM BECAME CAUGHT IN WHEEL AND WEDGED AGAINST RETARDER LEVER, ARM BROKE BETWEEN ELBOW AND SHOULDER.

EE DROVE LOADED SCRAPER OFF AN APPROX. 20' - HIGH STOCKPILE. EE MISSED THE TURN AND DROVE THE SCRAPER OVER A 3' - HIGH BERM. (SCRAPER DID NOT ROLL; EE WAS WEARING SEAT BELT - NOT EJECTED.)

Dumping material from a 631 scraper on top of berm, can slid to the side causing scraper to turn onto side.

An employee was operating LR9 (992 G) on the 1180 Central floor, loading 74B along the highwall. He was loading trucks and cleaning up the highwall and strained his lower back on the right side.

THE EMPLOYEE WAS OPERATING A Z6T HAUL TRUCK WHILE CLIMBING A 12% GRADE AND NEGOTIATING A LEFT-HAND RADIUS CURVE. THE EMPLOYEE STATED THAT HE LOOKED BEHIND BECAUSE HE THOUGHT HE SAW A LIGHT. HE ALSO HAD HIS LEFT ARM OUTSIDE OF THE OPERATOR'S COMPARTMENT. THE TRUCK CRUSHED HIS LOWER ARM AGAINST THE RIB. HIS ARM WAS AMPUTATED AT THE HOSPITAL, BELOW HIS ELBOW.

EE HIT HEAD ON ROOF OF TRUCK WHEN TRUCK WAS JARRED WHILE LOADING.

WHILE OPERATING A HAUL TRUCK. HIT BUMP CAUSING PAIN IN LOWER BACK.

JARRED LOWER BACK WHILE DRIVING 785 B HAUL TRUCK ON A ROUGH ROAD.

WHILE DRIVING ACROSS FISH CREEK, DUMP HAUL TRUCK HIT A BUMP AND DRIVER FELT PAIN IN NECK.

DRIVING TRUCK FROM PIT, TRIED TO ADJUST MIRROR, TRUCK HIT BUMP, CAUSED LACERATION TO LEFT HAND.

A RUBBER BAND WAS USED TO HOLD THE THROTTLE SLIGHTLY OPEN ON A SNOWMOBILE WHILE THE MACHINE WAS BEING PULL STARTED. WHEN EE PULLED THE ROPE SHE SLIPPED ON SOME ICE, FELL FORWARD ONTO THE HANDL E BARS. THE THROTTLE STUCK WIDE OPEN AND THE SNOWMOBIL WENT OUT OF CONTROL THROWING HER OFF. THE SNOWMOBILE CONTINUED UP AN EMBANKMENT AND CAME TO REST AGAINST A 988 LOADER.

WHILE RIDING IN A PICKUP IN THE PIT, THE TRUCK HIT A HOLE. THIS RESULTED IN AN INJURY TO EE'S BACK. HE RETURNED TO WORK WITH RESTRICTIONS. THE RESTRICTIONS HAVE NOT ALLOWED EE TO PERFORM HIS N ORMAL WORK DUTIES.

DRIVER DROVE ONTO WRON SECTION OF TAILINGS DAM(CLAY SECTION), REALIZED HE WAS ON WRONG SECTION. HE THEN PROCEEDED TO BACK UP. SECTION WAS ELEVATED APPROX. 4' ABOVE ROADWAY. HIS BACK DUAL GOT O FF THE SIDE OF SECTION, WHEN THIS HAPPENED THE TRUCK TIPPED ON ITS SIDE.

EE STATES HE WAS GOING TO THE LOADING PIT TO BE LOADED, WHEN THE CAB DOOR OF A B-70 BELLY DUMP JUMPED OUT OF ITS HINGES AND STARTED TO FALL. THE EE GRABBED THE DOOR HANDLE TO KEEP IT FROM FALL ING UNDER THE WHEELS AND JERKED HIS LEFT ARM.
