EE was driving a rock truck out of the pit area when the load shifted while traveling uphill. The front wheels lifted off the ground and abruptly came back down once the load was dumped out of the bed, causing the employee pain in EE's back. The injury occurred on August 11, but the employee continued working until August 19, when EE visited the doctor and was written off work.
The employee's left foot was positioned near the edge of the scoop bucket as it was being lowered to the ground, resulting in the bucket making contact with their foot.
Employee was maneuvering a rock jostling the 992G loader, at this time the employee struck their head inside the operator's compartment causing lacerations to the left side of head and left ear. While operating a haul truck, the employee steered quickly and jerked their body abruptly causing a strain.
Employee was using scoop to assist with moving shuttle car, the bucket of the scoop slid upon the shuttle car. The shuttle car rolled forward causing the scoop bucket to drop causing the scoop to bounce and the employee then struck head in the canopy.

Employee was traveling as a passenger on a rail personal carrier mantrip to the Main section. As the mantrip traveled up the slope incline, it lost traction and slid down the incline hitting a parked rail mantrip at the bottom of the slope.

Employee was operating a haul truck and the right front suspension cylinder failed causing employee to strike left arm and shoulder on the inside of the cab.
Employee was descending the slope on a supply motor, The motor derailed bouncing the employee around in the operators compartment, causing contusions.

Employee ran into diversion ditch while operating a coal truck and the truck turned on it's side. The employee slid from the drivers side to the passenger side of the truck.
Employee was backing up to dump an Artic truck and the bed rolled to the side. The employee stated that when the bed rolled to the side, the seat belt tightened up creating pressure on EE's left hip. The employee was transported to LRMC and was written off work because of a possible pinched nerve.
While employee was riding in the battery mantrip. Had knee bent and bounced and felt pain in left knee. The same knee was injured also on 5/20/2018.
Employee was operating shuttle car and hit corner of rib with canopy and material struck them in the face causing laceration above right eye.
