Employee was helping load belt into the scoop bucket. As they were lapping belt, the scoop rolled forward and employee's foot accidentally got caught between the scoop bucket and the mine floor causing injury.

Employee was holding an airlock door open when the scoop came through and accidentally slid off the rail causing the scoop bucket to hit the door. The door swung while the employee was holding the handle of the door causing injury to EE's hand.

The cable came across the intersection where EE was standing causing EE to fall. EE struck EE's face which resulted in a laceration to the lip and nose.
EE was operating a rubber tire ride and was in the process of traveling to the No.2 section to help work on a C/M. EE turned up in the No.6 entry when the front tire of the ride hit a rock and spun the steering wheel around striking the EE in the hand cause a fracture to the top of the EE's hand.
EE was loading belt structure when EE's foot slid under the bucket of a scoop, the bucket was being lowered at the time of the incident.
EE was pulling a combination head drive with a battery powered scoop. As EE pulled the belt head into the intersection the head drive got fouled on the corner of the rib and the 3/8" chain that was being used broke and struck the EE in the front tooth breaking the EE's tooth.
EE was operating a scoop in the No.8 heading on the No.3 section. EE hit a hole bouncing the employee around in the deck jarring their back.
EE was watering haul roads and claimed they felt pain in their back and leg. The EE was given 3 days off work which happened to be the same time of a paid holiday weekend with no work scheduled. An MSHA E03 inspection was conducted on 9/20/2022. A citation was issued. This 7000-1 is being submitted in order to abate the citation. The company intends to contest.
EE was traveling up to pit. During this process the shifter in the truck that the EE was operating broke and the truck came out of gear. The truck rolled into a dozer in close proximity.
Injured backed the scoop up to the charger hole to put scoop on charge. The jumper cables that was hung from the roof fell and struck the injured on the middle finger on the left hand cutting the finger
EE was backing powder truck up from in between rows of holes to set over to the next row. there was a small side bench that was 1-2' high. EE's back tandem got too close to the edge and the small bank gave way and the truck rolled over on its side.
EE was tramming a scoop thru a set of double doors @ 81 Brk Intake toward the belt line to rock dust. After pulling thru 1st set of doors, EE arrived at the 2nd set of doors and started to open them when scoop started rolling back toward 1st set of doors which the scoop hit causing door to go across EE's body in the deck of the scoop scraping back and catching arm against scoop.
Employee's hand was caught between airlock doors. EE never missed work until June 27, 2016 shift.
Employee was riding down roadway in rubber tire ride and there was an incline and a bump in the roadway which caused a 10 ton jack to dislodge and fall on his hand (right) smashing middle and ring fingers.
EE was assisting putting a rail car back on the track when the chain connected from the scoop to the rail car broke. The chain struck EE on his side and he fell and hit his head on the mine floor, temporarily losing consciousness.
EE alleges that as he was hauling refuse down the ramp the truck began to slide and the truck started bouncing heavily in some mud when he hit his head on the inside of truck cab. EE was kneeling down beside side dump of feeder while electrician was backing Mac-12 rubber tire personnel carrier towards him. EE did not get out of the way in time and the bumper of the Mac-12 pinned his lower left leg against the feeder. EE sustained fractured tibia in left leg.
