Mostrando los 46Employee was driving through the plant in the plant pickup. A sharp pain hit him in the back between the shoulders. Employee reported the incident to the supervisor. Employee was then taken to the Dr. for examination. The Dr. stated that Employee had a muscle spasm and placed him on light duty and physical therapy.
After eating dinner under a shade tree, ee's were headed back to washing plant to work. EE's gave the truck a lot of gas and injured slid out the back of the truck landing on his head.
Injured was eating lunch with remainder of crew in shade of railcar, when locomotive hooked to cars & started moving. Injured was knocked down and in falling his hand was run over by wheel of railcar.
Employee needed to open the top hatch of the cherry picker to see his signal man. The hatch wouldn't stay open and kept falling back down. He propped the hatch open with a mirror he found laying behind the seat. When he applied the swing brake, the mirror fell, struck his hand, broke, and cut his left index and middle fingers requiring stitches.
Employee was cleaning out empty rail cars when it was struck by additonal empty rail cars causing a strain to the back secondary to the jaring.
Employee states that while he was runing the clean-up loader his back started to get stiff and sore. He also stated that he was sore the next morning. After seeing the doctor the doctor stated he had a straind muscle in his lower back. EE states he can't remember doing anything other than operating the loader that could have injured him.
He was operating a bobcat and he stated that it locked up jaring him around causing him strain his right shoulder
Employee was sitting in haul truck being loaded by a loader he states that the rock shook the truck and caused him to strain his lower back
THE OPERATOR WAS ENTERING OUR STOCKPILE AREA AND CROSSED A DRAINAGE DITCH. HE HAD BEEN THERE EARLIER IN THE DAY TO DRAIN WATER FROM THE STOCKPILE AREA. HE WAS TRAVELING APPROXIMATELY 15 MPH WH EN HE ENTERED THE DITCH. AT THAT TIME THE WEIGHT OF THE LOADER SHIFTED FROM LEFT TO RIGHT WHICH FORCED HIS BACK TO SHIFT LEFT TO RIGHT THEREFORE CAUSING A MUSCLE STRAIN IN THE LOWER BACK.
EE ALLEGED THAT WHILE BEING LOADED WITH BOULDERS THE LOADER OPERATOR DUMPED THE ROCKS TO HARD CAUSING THE TRUCK TO SHAKE AND INJURING THE EE'S BACK. THIS INCIDENT HAPPENED ON 10-23-03 SHIFT BU T HAPPENED ON THE 10-24-03. THE EE DID NOT REPORT THIS INCIDENT TIL THE NEXT DAY. EE WAS TAKEN TO LOCAL HOSPITAL, EE WAS BUT ON TWO DAYS OF RESTRICTED DUTY.
EE WAS CRAWLING OVER THE FRAME OF THE PLANT CONVEYOR. WHILE HE WAS CROSSING OVER A ROCK FROM AN OPENING BETWEEN THE CENTER HOPPER AND THE WING WALL FELL HITTING HIM ON THE LEFT HAND. THE ROCK WAS ABOUT 2 1/2"ROUND AND FELL ABOUT 10'. NO REASON WHY THE ROCK FELL. HIS HAND WAS ON THE FRAME WHEN THE ROCK STRUCK HIM. THE DR. PUT A WRAP ON THE WRIST & HAND.
EE WAS ALIGNING 2 PIECES OF FLANGED PIPE WHICH WERE FLOATING ON PONTOONS ON THE DREDGE POND. THEY WERE USING AN OUTBOARD BOAT TO LINE UP THE PIPE BECAUSE OF HIGH WIND. INJURED WAS ON THE PIPEL INE. THE BOAT WAS AT HIGH THROTTLE TO COMPENSATEFOR THE WIND.INJURED STEPPED ONTO THE BOAT & THEFRONT OF BOAT SLIPPED UNDER PIPELINE. THE BOAT SHOT FORWARD CATCHING LEG BETWEEN BOAT & PIPELIN
CONTRACT HAULER LOADED TRUCK AND WAS LEAVING THE STREETMAN PLANT. THE TRUCK DRIVER PULLED TO THE RIGHT AND TRAILER GOT OFF IN GRASS. THE DRIVER TRIED TO PULL IT BACK ON THE ROAD AND THE TRUCK ROLLED OVER. HE TRUCK DRIVER RECEIVED A LACERATION TO HIS LEG.
EE ALLEGES THAT WHILE PUSHING A BAG (CONVEYOR WAS CHOKED UP) HIS RT. FOOT GOT CAUGHT IN CONVEYOR PULLING HIS BODY INTO IT UP TO HIS CHEST.
EMPLOYEE ALLEGES THAT WHILE WALKING DOWN ROAD, A FORKLIFT HIT HIM ON THE RIGHT SIDE OF HIS BODY.
EE WAS OPERATING MAN-LIFT CHANGING LIGHTS THROUGHOUT THE PLANT & TERRAIN WAS ROUGH. EE WAS EXPERIENCING PROBLEMS AROUND 11:00 AM, DID NOT REPORT IT UNTIL 3:00 PM THAT DAY. EE WAS RELEASED TO F ULL DUTY THE SAME DAY. EE HAD FOLLOW UP ON 10/1/02. EE PUT ON LIMITED DUTY UNTIL MORE TESTS COULD BE PERFORMED TO DETERMINE WHAT IS CAUSING DISCOMFORT IN HIS ANKLE.
EMPLOYEE ALLEGES THAT WHILE OPERATING A FRONT END LOADER, HA AND ANOTHER EMPLOYEE OPERATING A EUC BUMPED INTO EACH OTHER CAUSING HIM TO BE THROWN INTO THE CAB OF HIS LOADER. **NOTE** EMPLOYEE DID NOT SEEK MEDICAL TREATMENT UNTIL 7/18/02.
EE COMPLAINED OF BACK PAIN WHEN OPERATING 980G LOADER ON WED., MAY1, 2002. COMPANY DOCTOR DIAGNOSED EE'S CONDITION AS ACUTE INFLAMMATION OF LUMBAR DISC. EE DID NO KNOW WHEN IT REALLY HAPPENED; THEREFORE, WE DO NOT HAVE A DATE OR TIME.
EE WAS HAULING BOULDERS TO THE LEVEE IN THE EASTPIT, THEY HAD BEEN WORKING ON FOR A FEW MONTHS. THE LOADER OPERATOR HAD PUT ONE BUCKET IN THE BED OF THE HAUL UNIT FOR CUSHION WHEN THE OPERAT OR WAS ATTEMPTING TO PUT THE SECOND BUCKET ON THE TRUCK A BOULDER WEDGED IN THE BUCKET. THE BOULDER CAME FREE AND BOUNCED THE OPERATOR INSIDE THE CAB. EE WENT TO DR.4-8-02 & RELEASED TO WORK.
EE ALLEGES WHILE DRIVING HE HIT DUCT WORK AND TIPPED THE TRUCK OVER CAUSING CONTUSION TO HIS THIGH.

EE WAS EXITING A CATWALK WHEN LEAKING MATERIAL FROM BELT WAS DISCOVERED. EE TURNED AROUND ON CATWALK TO INVESTIGATE AND HIS GLOVE WAS CAUGHT IN BETWEEN BELT AND ROLL. EE WAS ABLE TO PULL HIS HAND FREE.
EMPLOYEE ALLEGES THAT WHILE MOVING A MANLIFT, HEHIT A BUMP IN THE ROAD CAUSING HIM TO BOUNCE UP HITTING HIS RIGHT ELBOW.
RAIN, WET ROADS. DRIVING TOO FAST, DRIVING ON WRONG SIDE OF ROAD HAUL UNIT TAKEN OUT OF SERVICE DUE TO DAMAGE TO THE ROPS. DRIVER SUSTAINED BRUISING AND SCRATCHES.
A ROOT (CHUNK) WAS CAUGHT IN THE SUCTION OPERATOR LIFTED SUCTION AND PADDLED TO SUCTION/JET. SUCTION RAISED 6-8" ABOVE WATER SURFACE. OPERATOR STUCK HAND IN SUCTION TO PULL CHUNK OUT. HAND SLI PPED AND HIT ELBOW ON PADDLE BOAT.
EMPLOYEE WAS OPERATING A JOHN DEERE FRONT END LOADER, ALLEDGES THAT THE SEAT DID NOT ADJUST FOR HIM, THUS CAUSING HIM BACK PAIN.
EMPLOYEE WAS RUNNING MATERIAL THRU THE LOADING TIPPLE. EE WAS HAVING PROBLEMS GETTING CLODS IN FEEDER CAUSING IT TO STOP UP. EE WENT DOWN THE THE TUNNEL TO SEE WHAT PROBLEM WAS. EE STUCK RIGHT HAND UP IN FEEDER TO REMOVE CLODS HE HIT THE KILL CABLE KILLING THE BELT CAUSING FEEDER GATE TO CLOSE ON HIS WRIST. EMPLOYEE FAILED TO LOCK-OUT BELT BEFORE WORKING ON FEEDER.
EMPLOYEE IN VEHICLE #1 GOING SOUTH BOUND OUT OF DRIVE WAY. COLLIDED WITH CONTRACT CARRIER ENTERING DRIVE GOING NORTH IN LEFT HAND CURVE.
EE WAS OPERATING A FRONT END LOADER. HE COMPLAINED OF NECK PAIN FROM THE ROUGH RIDE. HE WENT TO A CHIROPRACTOR FOR TREATMENT AND RETURNED TO WORK WITH NO LOST TIME OR RESTRICTED DUTY.
EE WAS LOADING RAIL CAR IN A 980G LOADER HE PULLED OFF OF THE PAVED CROSSING ONTO THE LOADING PAD. THE UNEVEN AREA CAUSED THE LOADER TO JERK IT JARRED EE CAUSING BACK PAIN
DRIVING HAUL TRUCK HE HIT POT HOLE & STRAINED UPPER BACK.
THREE EMPLOYEES WERE DEWINTERIZING THE PLANT USING THE JON BOAT TO DO SO. THE BOAT WAS CRANKED; IT WAS IN GEAR WITH THE THROTTLE WIDE OPEN. THE BOAT LUNGED FORWARD, EJECTING TWO EMPLOYEES INTO THE WATER. THE BOAT STRUCK THE STRIPPING BARGE &SMASHED INJURED'S THUMB OFF. THE THIRD EMPLOYEE PULLED BOTH VICTIMS OUT OF THE WATER. INJURED ALSO HAD A HAIRLINE FRACTURE TO THE LEFT WRIST.
EE ALLEGES THAT HIS RIGHT ARM WAS PULLE INTO CONVEYOR BELT & PINNED HIM FOR APPROXIMATELY 25 MINUTES.
STEERING WHEEL JERKED LEFT WHILE VEHICLE WAS BEING TOWED.
LOWER BACK PAIN GOING TO CHIROPRACTOR WAS ON LIGHT DUTY
DRIVING TOO FAST DOWN HAUL ROAD & JACKNIFED THE TRACTOR & TRAILER
EMPLOYEE FILLED WATER TRK AT SHALE PIT WATER SPOUT, HE THEN PROCEEDED NORTH AROUND WEST DRAINAGE POND. TRUCK THEN HIT THE BEAM AND WENT OVER THE EDGE INTO WATER. EMPLOYEE BROKE WIDOW AND EXITE D TRUCK. HE THEN WAS TAKEN TO HOSPITAL, WAS X-RAYED AND STITCHES PUT IN ARM.
MAINTENANCE DEPT. HAD JUST COMPLETED INSTALLING A 6" CAST IRON GATE VALVE INTO THE 6" STEEL OVERHEAD WATERLINE THAT IS USED TO FILL THE WATER TRUCK. THE WATER TURCK WAS BACKED UNDER THE SPOUT TO BE FILLED. AFTER FILLING EE CLOSED THE 6" VALVE AND COWORKER STARTED TO DRIVE OFF WHEN EE NOTICED THAT METAL SCREEN COVERING MANHOLE HAD HUNG ON FILLER SPOUT CAUSING THE 6" DOWN SPOUT TO BR
WHILE OPERATING A CRANE IN DREDGE AREA, THE OPERATOR WAS MAKING A PICK WITH THE BOOM FULLY EXTENDED, WHEN HE STARTED THE SWING, HE CAUSED CRANE TO LAY OVER ON ITS SIDE.
INJURED EMPLOYEE NORMALLY WORKS IN SCALEHOUSE. HE WAS ASKED TO RUN A LOADER IN PLANT OT HELP LOAD RIAL CARS. HE DID THIS FOR TWO HOURS AND ALLEGED HIS BACK WAS SORE 7-14-00. HE SAID HE FELL IN HIS YARD 7-14-00, WENT TO ER DR 7-14. HE WAS REFERRED TO ANTOHER DOCTOR ON 7-25-00.
EE WAS PULLING PIPELINE INTO SLEEVE WITH THE WORK THE BOAT MOTOR WAS NOT LOCKED DOWN AND THE COVER WAS WHEN THIS MOTOR WAS ACCULTED THE MOTOR RECKLED AND STRUCK THE EE ON THE LEFT .
EE WAS RIDING THE BUS TO WORK AREA WHEN THE BACK AXLE FELL INTO A SOFT SPOT IN THE ROAD, CAUSING A CONTUSION TO THE ELBOW.
EE WAS DRIVING A FLATBED TRUCK ON A DIRT ROAD WHEN A DUST CLOUD BLEW INTO THE CAB, CAUSING AN IRRITATION TO THE EYE.
