THE FORKLIFT OPERATOR WAS PLACING A PALLET OF PRODUCT IN A STORAGE BAY & DUE TO AN OBSTRUCTED VIEW & NOT ST AYING ALERT TO HIS SURRONDINGS,HE STRUCK THE WALKING EMPLOYEE. THE EMPLOYEE STRUCK SUFFERED A SPIRAL FRACTURE OF THE LEFT ANKLE.

UPON TIPPING OVER THE ROLL OVER PROTECTION ON THE LIFT TRUCK LANDED ON HIS RIGHT LEG BREAKING THE LEG.
INJURED HAD PARKED LOADER #1532 BEHIND SHOP AND WAS IN PROCESS OF ADDING OIL TO THE ENGINE. ALANADAMS HAD PULLED TRUCK #1252 DOWN TO THE SHOP TOSEEK REPAIR OF THE DEFROSTER. ALAN STARTED BACKI NG UP NEXT TO THE LOADER AND THEN CUT BACK OF THTRUCK INTO THE LOADER THUS PINCHING INJURED BETWEEN THE TRUCK AND LOADER FRAME. THE TRUCKS BACK UP ALARM WAS WORKING PROPERLY.
THE FRONT WHEELS OF HIS TRUCK DROPPED INTO A 6" WASH. RECOIL OF SEAT MADE DRIVER'S HEAD HIT THE ROOF OF THE TRUCK & JAMMED HIS NECK.
EMPLOYEE WAS GACKING AROUND A PALLET OF SLIPSHEETS WHEN THE FORKTRUCK STEERING END HIT STACK OF PALLETS HE SWERVED TO GET OFF PALLETS & BACKED OFF OF LOADING DOCK THE FORKTRUCK LANDED ON COUNT ER WEIGHT END & FLIPPED OVER ON ITS SIDE EMPLOYEE BACK WAS INJURED BUT HE WAS WEARING HIS SEATBELT
OPERATOR WAS DRIVING THE LOADED WATER TRUCK OUT OF PIT #1 ON THE HAUL ROAD (9 DEGREE SLOPE, 600'), APPROX. 3/4 OF THE WAY UP THE ROAD, HE HEARD A "LOAD POP" IN THE REAR OF THE TRUCK. THE TRUCK CHOKED DOWN AND BEGAN ROLLING BACKWARDS DOWN THE ROAD. THE EMPLOYEE PUSHED ON THE FOOT BRAKE AND ENGAGED THE EMERGENCY BRAKE. THE TRUCK CONTINUED TO ROLL BACKWARDS. THE EMPLOYEE WAS ABLE TO C
EMPLOYEE WAS ON LOADER, CROSSING RR TRACKS & SAW SHOW TRUCK COMING, HE CLAIMED HE TRIED TO STOP BUT ALLEGEDLY HAD NO BRAKES. HE DROPPED THE BUCKET WHICH THREW HIM INTO THE WINDSHIELD. HE STATE D BRAKES WORKED BEFORE THE ACCIDENT, IT WAS CHECJED AFTER ACCIDENT, FULL OF FLUID & BRAKES WORKED, SEAT BELT NOT BEING WORN PROPERLY. HE WAS TAKEN TO ER, RECEIVED 1ST AID ONLY, RELEASED.
THEY WERE MOVING AN IRON BEAM WITH A FORKLIFT BEAM WAS BEING TURNED OVER TO WELD BOTTOM SIDE THE BEAM HUNG ON THE FORKLIFT & EE TRIED TO FREE IT WHEN THE BEAM CAME FREE IT CAUGHT HIS RT MIDDLE & RING FINGER BETWEEN THE BEAM & A WOOD BLOCK ON THE FLOOR IT MASHED OFF THE FINGER TIPS ON THOSE TWO FINGERS SURGERY & SKIN GRAFTING WAS REQUIRED
EMPLOYEE WAS GOING TO BREAKROOM. ANOTHER EMPLOYEE WAS DRIVING A PICKUP TRUCK TRUCK AND DECIDED TO SCARE (HORSEPLAY) I.NAME. HE WAS CLOSER TO HIM THAN HE THOUGHT AND HIT HIM WITH THE TRUCK CAUS ING HIM TO HIT THE BUILDING AND FALL. EMPLOYEE DID NOT SEE DOCTOR UNTIL DOCTOR UNTIL MONDAY, NOV 2 (DAY MISSED WORK).
66923IFT WAS HOLDING PORTABLE HOPPER WITH BULK BAG ATTACHED. EMPL STANDING ON FORKLIFT MAST PUNCHING WET CLAY THROUGH HOPPER INTO BULK BAG. AFTER EMPTYING HOPPER, EMPL REQUESTED IT BE LOWERED 26071YLOADER COULD RE-FILL. AS HOPPER ASSEMBLY WAS LOWERED EMPL FOOT WAS PINCHED BETWEEN MAST ANS FORKPLATE.SEE ATTACHED SKETCH.
HE WAS DRIVING A TRI-AXLE DUMP TRUCK HAULING LOADS OF WASTE CLAY AT THE TRENTHAM MINE. HE DOESN'T KNOW FOR SURE WHAT HAPPENED, ONLY KNOWS WHEN IT STARTED HURTING. HE WASN'T LIFTING, PUSHING OR PUSHING ANYTHING. HE THINKS IT COULD HAVE BEEEN CAUSED FROM TURNING HIS UPPER BODY AROUND TO LOOK BACK. THIS WAS NOT REPORTED UNTIL 8-17-92.
HE WAS DRIVING AN EMPTY DUMP TRUCK, GOING TO THE DENTON MINE TO GET A LOAD OF CLAY. THE ROAD WAS A LITTLE ROUGH COMING OFF THE HIGHWAY INTO THE MINE AND IT JARRED HIS BACK. BACK STARTED HURTIN G BUT HE FINISHED THE DAY.
HE WAS TRIPPING A SELF DUMPING HOPPER WHILE STANDING ON THE TAILGATE OF A DUMP TRUCK AND PARTLY ON THE MAST OF #1623 FORKLIFT.HE REACHED OUT WITH RIGHT HAND TO TRIP LEVER ON HOPPER,HAD LEFT HA ND ON BOTTOM BEAM OF HOPPER.WHEN HOPPER DUMPED THE BEAM CAME UP AND CAUGHT LEFT AHND BETWEEN BEAM AND FORKLIFT TINES MASHING LITTLE FINGER AT LAST JOINT.
EMPLOYEE WAS DRIVING FORKLIFT BETWEEN 2 STACKS OF PALLETS WITH HIS FOOT STICKING OUT SIDE OF FORKLIFT. HIS FOOT STRUCK ONE OF THE PALLETS, CAUSING HIS KNEE TO BE TWISTED. IT WAS NOT REPORTED UNTIL THE NEXT DAY, WHEN HE CALLED IN, SAYING HIS KNEE HAD SWOLLEN, AND HE HAD GONE TO THE DOCTOR.
EMPLOYEE WAS OPERATING D25C ARTICULATED DUMP TRUCK AND WAS WEARING SEAT BELT RATHER LOOSELY.TRUCK HIT A BUMP IN RAOD CAUSING JOLT RESULTING IN BRUISING.
EE WAS BACKING HIS TRUCK UP TO BACKHOE TO BLOADED. HIS TRUCK WENT THRU A SOFT SPOT CAUSING THE TRUCK TO ROCK AND BOUNCE THE TRUCK STRUCK THE BACKHOE CAUSING EE TO HIT HIS HEAD ON LEFT SIDE WIN DOW FRAME OF TRUCK.

PUTTING OIL IN ENGINE TH SERVICE TRUCK BEGAN TO ROLL FORWARD PULLING HOSE AND CATCHING L HNAD WHICH WAS BETWEEN RADI ATOR SHROUD AND OIL NOZZLE CUTTING OR TEARING L.HAND WHICH WAS BETWEEN RADIATOR SHROUD AND NOZZLE CUTTING OR TEARING L HAND BETWEEN L THUMB AND FORE FINGER.
