INJURED STATED HIS LOWER RIGHT BACK WAS HURTING ON AND OFF EVER SINCE THE 04-10-95 INJURY.HE STATED WHEN HE GETS OFF THE SHUTTLE CAR APPROX 15 MINUTES,THE PAIN EASES UP BUT STARTS AGAIN WHEN H E OPERATES THE SHUTTLE CAR.HE DID NOT SEEK MEDICAL ATTENTION UNTIL 06-15-95.HE STATED ON 04-10-95 HE WAS TRAMMING UP #3 ENTRY WHEN HE RAN OVER A ROCK ABOUT TWO FEET LONG AND SIX INCHES WIDE A

EE SAID HE WAS TURNING AROUND IN THE SHUTTLE CAR WHEN HE HIT HIS LEFT KNEE ON THE CONVEYOR SWITCH CAUSING PAIN IN HIS LEFT KNEE
EE STATED HE HAD LOADED A CAR OF COAL IN #1 ENTRY AND TRAMMED HIS CAR 5 BREAKS AND HIT A HOLE IN THE ROADWAY 1 BREAK INBY THE FEEDER. HE FELT PAIN IN HIS BACK BUT CONTINUED TO WORK UNTIL THE M INER FINISHED THE CUT (APPROX. 15 MIN.) AND THE PAIN WAS TOO MUCH AND CAME OUTSIDE. THE PAIN WAS LOCATED IN THE THORACIC PART OF THE SPINE. RESULTED IN STRAIN TO THE BACK.
EE STATED THAT HE WAS IN #4 ENTRY HELPING EE HANG THE OFF STANDARD CAR CABLE EE SAID THAT THEY START ED THE CAR UP & THE CABLE TOOK UP PULLING THE HANGERS FROM THE ROOF BOLT PLATES EE SAID THA T THE CABLE CAUGHT HIM ON THE RT SIDE OF HIS NECK & KNOCKED HIM AGAINST THE RIB HURTING HIS LEFT SHOULDER EE SAID THE CABLE DRUG HIM APPROX 3 TO 4 FT
INJURED STATED THAT HE GOT INTO THE BACK END OF THE RAILRUNNER PREPARING TO GO UNDER THE OUTSIDEBELT. HE SAID THAT HE WAS DOWN ON HIS KNEES. WHEN THE RAILRUNNER STARTED UP, THE FORCE CAUSED HI M TO RAISE UP. HIS HEAD CAUGHT THE CROSSOVER GUARD ON THE #1 BELT AND IT DRAGGED HIM HALF OUT OFTHE RAILRUNNER, RESULTING IN LACERATION TO LIP AND REMOVAL OF ONE TOOTH.
INJURED STATED THAT HE WAS TRAMMING TOWARD THE FEEDER WITH A LOAD OF COAL, WHEN THE TRUNION BROKE ON THE DUMP END OF SHUTTLE CAR. THIS BOUNCED HIM IN THE DECK OF THE SHUTTLE CAR AND JARRED HI S LOWER BACK.
KEN STATED THAT HE WAS IN #3 ENTRY. THE HAD REPLACED PART OF SHUTTLE CAR AND THAT THE SLACK HAD BEEN PULLED OFF THE CAABLE REEL. HE HAD HIS RIGHT FOOT ON THE SLACK TO KEEP THE CABLE STRAIGHT . AS THE SLACK TOOK UP, THE CABLE PUSHED HIM UP AND INTO THE COAL RIB. RESULTING IN FRACTURE TO RIGHT LEG IN KNEE JOINT.
EE STATED THAT HE WAS BACKING THE FRONT BRIDGE OUT OF THE FACE OF #3 ENTRY & THAT THE BRIDGE WAS ON A HUMP I THE FLOOR THE TAIL OF THE MINER GOT UNDER THE DUCK BILL ON THE BRIDGE & WHEN THE BR IDGE DROPPED OFF IT BOUNCED HIM CAUSING THE BACKOF HIS HEAD TO HIT THE CANOPY POST
HE STATED THAT HE HAD GOTTEN INTO THE SCOOP BUCKET FOR MANTRIP FROM THE END OF THE TRACK TO THE SECTION. HE WAS ON THE OUTSIDE EDGE OF THE BUCKET. THE SCOOP WAS TRAMMING TOWARD THE SECTION WHE N THE SCOOP WENT THROUGH A SWAG AND THE SCOOP OPERATOR RAISED THE SCOOP BUCKET INTO THE ROOF, DRAGGING HIM ALONG THE ROOF FOR APPROX. 5 FEET BEFORE PULLING HIM OUT OF BUCKET INJURING HIS SHOU
INJURED EE WAS A PASSENGER IN A MANTRIP MOTOR THAT STRUCK A DERAILED MOTOR AND EE WAS THROWN ON HIS SHOULDER.
EMPLOYEE STATED HE WAS IN #3 HEADING CUTTING #3 LEFT BREAK. HE WAS BACKING THE MINER UP WITH THE REMOTE CONTROL & AS HE CAME BY THE MINER CABLE HE STEPPED ON MINER CABLE CAUSING HIM TO SHIFT H IS WEIGHT & HE INJURED HIS LEFT KNEE. [TWISTED]
EE STATED THAT THE SCOOP WAS GOING UP #5 ENTRY CARRYING MANTRIP TO SECTION. HE WAS RIDING IN THE SCOOP BUCKET WHEN SCOOP BECAME STRUCK ON PILE OF LOOSE COAL, CAUSING SCOOP TO JUMP UP AND BOUNC ED HIM CAUSING LOWER BACK PAIN.
WHILE RIDING IN MANTRIP TO SECTION FROM THE SURFACE, THE AUTOMATIC FIRE DELUGE SYSTEM ON THEPERSONNEL CARRIER DISCHARGED AND HE STATED THAT HE INHALED THE CHEMICAL RELEASE.
WHILE RIDING IN MANTRIP FROM SURFACE TO THE SECTION, THE AUTOMATIC FIRE SUPPRESSION SYSTEM ON THE PERSONNEL CARRIER DISCHARGED AND HE INHALED THE CHEMICAL RELEASE. DORAN WORKED 2 HOURS ON 3-32-93, THEN CAME OUTSIDE WITH SAME SYMPTOMS AND RETURNED TO DOCTOR.
INJURED WAS IN A RAILRUNNER TYPE MANTRIP. INJURED WAS POSITIONED TOO HIGH IN THE MANTRIP CAUSING INJURED TO STRIKE AN OVERCAST RAIL WHICH KNOCKED INJURED BACKWARD.
INJURED AND ANOTHER EMPLOYEE WERE RETURNING TO THE SURFACE COMING OUT OF MINE IN A RAILRUNNER.EELECTRIC BRAKES FAILED AND HYDRAULIC BRAKES WERE APPLIED,LOCKING UP WHEELS ON RAILRUNNER CAUSING IT TO JUMP THE TRACK ON SURFACE AREA.THE JOLT OFTHE WRECK CAUSED PAIN TO BACK AND RIGHT KNEE.INJURED WORKED EVERYDAY WITH PAIN UNTIL HE SAW A DOCTOR ON 05-28-92.INJURED HAD SEEN A DOCTOR ON 01
INJURED STATED THAT HE DROVE THE SCOOP TO WHERE OXYGEN AND ACTYLENE TANKS WERE STORED ON A SLED. HE SET THE BRAKE AND DROPPED THE SCOOP BUCKET. HE GOT OUT OF SCOOP AND LOADED HOSES AND GUAGE S FIRST. HE AND ANOTHER EMPLOYE EPULLED THE SLED UP IN THE SCOOP BUCKET WHENT EH SCOOP ROLLED FORWARD APPROXIMATELY 12 INCHES CATCHING HIS FOOT BETWEEN SCOOP BUCKET AND MINE FLOOR.
TRAMMING SHUT CAR IT BOTTOMED OUT CAUSING HIM TO STRIKE HEAD AGAINST TOP OF CANOPY
WHILE TRAMMING SHUTTLE CAR (STANDARD SIDE CAR) FROM FEEDER BACK TO CONTINUOUS MINER. TRAVELING ALONG THE #4 ENTRY ROADWAY, CAR TIRE DROPPED INTO A HOLE IN ROADWAY CAUSING INJURED TO BE BOUNCED UPWARD, THEN BACK DOWN SUDDENLY, RESULTING IN AN ALLEGED INJURY TO THE LOWER BACK.
HE WAS OPERATING STANDARD SHUTTLE CAR IN ENTRY APPROX. 2 BREWKAS IN FEEDER GOING TOMINER AFTER A CAR OF COAL. HE HIT A HOLE IN THE ROADWAY, AND WAS BOUNCED OUT OF HIS SEAT AND WHEN HE CAME BA CK DOWN, HE LANDED ON HIS BACK ON OUTSIDE RAIL OF OPERATORS DECK.

SCOOP HAD BELTHEAD RAISED UP TO PUT IN OIL BATHS SCOOP LET THE BELTHEAD BACK DOWN BUT FRAME OF BELTHEAD GOT CAUGHT ON OIL BATH BELTHEAD WAS RAISED BACK UP AND VICTIM WAS PUTTING OIL BATH IN PL ACE WHEN BELTHEAD FELL CATCHING VICTIMS LEFT AHDND BETWEEN BELTHEAD FRAME AND OIL BATH
INJURED WAS TAK3NG PARTS INSIDE FOR A 265 LEE-NORSE CONTINUOUS MINER IN DECK OF RAILRUNNER. AFTER GOING APPROXIMATELY 420' INBY TRACK ENTRY PORTAL,HE RAISED UP TO LOOK OVER TOP OF RAILRUNNER T O CHECK LOAD,AN8 A LOW HANGING METAL ROOF STRAP CAUGHT HIS COVERALLS,PULLING HIM BACK ON TOP OF BUMPER AND RAILRUNNER,CAUSING ABRASIONS TO TOP OF HEAD,LEFT SHOULDER,BASE OF NECK,STRAIN TO SOD
WHILE RIDING IN3BUCKET OF 488 SCOOP , THE SCOOP ROUNDED A CORNER CAUSING SCOOP BUCKET TO JERK WHICH IN TURN CAUSED HIS BACK TO HIT AGAINST THE SIDE OF SCOOP BUCKET, INJURING HIS BACK
WHILE SCOOPING 9OCK AND DEBRIS FROM MAIN LINE TRACKWAY, APPROX. 2,900' INBY PORTAL, USING A 488 S&S SCOOP THE BUCKET RO DECK OF SCOOP CAUGHT THEEND OF A SECTION OF 4"X20' PLASTIC PIPE, CAUSING PIPE TO "SPRIN1" UP HITTING INJURED IN RIGHT SIDE OF FACE AND HEAD, CAUSING TWO LACERATIONS TO RIGHT SIDE OF FOREHEAD. BOTH LACERATIONS COMBINED REGULARED 20 SUTURES.
