The Admiral Security Guard tripped in a dip in the road while moving a cone for a truck driver to exit the plant.

Employee went to investigate a dusting situation at the silo discharge. EE climbed the ladder to the upper platform and found spillage on the platform. Upon trying to descend the ladder EE lost EE's footing and fell to the lower level.

Walking in rough terrain to access a settling pond

Employee fell off of a skid steer at bucket height on ground while getting the hours of operation from the meter.

While unrigging bag EE fell off the trailer and injured arm.

The employee was walking up a set of open ended steps when EE's left boot got caught on the step causing EE to fall forward. When EE fell forward, EE put all of EE's weight on the left shin. A large contusion formed on EE's left shin.

Plant electrician stepped into the roller mill on the grinding table. EE slipped and fell landing on left shoulder and leg. EE thought EE might be ok at first but later the next morning EE's shoulder was still hurting. EE went to the doctor the next and was put on restricted duty.

Employee was climbing a staircase in the finish mill building when they missed the last step and fell onto left knee. Employee was taken to a local clinic for further evaluation. Clinic provided employee a knee brace and EE was told to not climb stairs or kneel. Clinic also advised employee not to stand for prolonged periods of time and to rest knee every two (2) hours.

Employee tripped over metal plate, and then slipped in a mud puddle, causing EE to fall directly onto EE's left knee.

Employee was descending a short, stationary ladder. As EE reached the ground, EE lost footing and fell. Employee used EE's arms / hands to brace the fall and injured the left wrist when EE hit the ground. Employee was taken for x-rays and they showed that EE broke the wrist.

An employee was descending a staircase in the shipping area when they felt a pop in their knee. Pain was severe so employee was taken for medical attention.

On 2/5/19 a few minutes to 8am, the employee walked towards the pack house. EE stopped to talk to a contractor. When the conversation was done, EE went to exit the area onto the sidewalk. EE did not lift EE's foot high enough to clear the rail causing EE to lose EE's balance and fall. The employee immediately reported the incident to EE's supervisor.

The employee slipped on wet road.

Employee was walking near urea dike and tripped on the ground. The employee say no to receive medical attention. Few days after, EE decided go to to receive medical attention.

A maintenance intern was observing mechanics replace pans on a conveyor when a co-worker pinched the intern in jest, startling the intern to the point where the intern's left knee buckled and the intern fell to the ground. The fall aggravated a previous knee injury to the point where medical attention was required.

Employee walked downs stairs slipped on step. No fall. EE has three points of support.

Employee walked down stairs, at the end of the stairs they slipped and fell to the ground.

Cement transport driver walking down a flight of stairs after tank wagon was loaded and claims to have slipped on the 3rd step from the bottom of the stairs causing driver to fall. Driver indicated left knee was in pain & outside medical services were contacted. Driver was transported to Trident Hospital. After this, very little information provided to mine operator until 08-20-18.

A maintenance mechanic was utilizing a 4-foot ladder to remove siding from a hopper. Employee fell approximately 3 feet and felt pain in lower back. Employee was immediately taken to a nearby urgent care facility to be evaluated and was released on light duty.

The employee carrying out the replacement and mounting of the bearing at the central screw conveyor of the Bag house, climbed to a platform with approx. height 3'7". When employee tries to get off it, slips and falls to the ground hitting back and shoulder blade on the right side.

While checking fire extinguishers at Argos Plant, employee slipped down on a flat, muddy area covered by gravel. Employee's left leg slipped forward and turned out. Knee slipped back and ee fell on right shoulder. EE has a bruised shoulder and a swollen knee.

Employee was walking in front Technical offices when employee slip & fall on the road. The road had the present of some liquid in a small perimeter

A mobile maintenance mechanic was coming down the ladder of a haul truck. As foot contacted the ground, ee right knee buckled.

A contractor was sweeping the floor when EE stepped back on a loose floor board causing EE's leg to fall through the floor. The board was cut to run ducting for a new air conditioning, and the a/c contractor did not properly secure the loose board.

Argos disputes that a Lube Tech suffered an occupational injury at the facility and further disputes that ee suffered an occupational injury that was reportable under Part 50.20. Since ee claims ee slipped and fell injuring shoulder and back, and since ee has been temporarily assigned to light duty, Argos submits this 7000-1 form to avoid the potential issuance of a citation.

Employee was crossing over a stationary railcar. When he put his foot on the step to get down, his foot slipped off the ladder. He immediately grabbed the closest handle with his left hand and felt a sharp pain in his left arm.

Employee was welding beams on a structure when he lost his footing and grabbed a near by beam for support. The sudden movement caused a muscle contusion.

Employee was descending the ladder of a front end loader when he misjudged the last step. The employee missed the last step and foot went all the way to the ground unexpectedly. The employee strained his shoulder while trying to hold onto the ladder.

Employee was loading a truck out of silo 8. The gate was stuck open and material did not stop flowing. Employee went out of the office, thru the center bay door and up the ladder to close the gate. As he was coming back down the ladder he pulled/strained his right calf muscle.

employee slipped on a scaffold and fell approximately 10 feet landing on a horizontal scaffold pole in the sitting position.

EMPLOYEE WAS CLIMBING THE STAIRS WITH AN AIR HOSE IN HIS HAND AND MISSED A STEP AND FELL.

While getting off the motor grader, the operator climbed down the left side steps. As he stepped off the bottom step onto the ground, his right foot slipped out from under him, he lost his grip on the handrails, and fell backwards to the ground. He hit his left elbow on the ground.

Employee was undercutting material in a silo when the material came down on him causing him to fall on the floor and hit his right knee.

A maintenance millwright foot slipped while dismounting a lull. The maintenance employee reached out with his hand to prevent himself from falling from the lull. Weeks later, he sought treatment for the pain experienced in his shoulder resulting from the incident.

While moving with cross ties, a maintenance employee fell backwards hurting his lower back.

A production employee lost his balance while entering the cooler. The employee fell and broke the fall with his knee which suffered a cut over the knee.

Employee was walking on a 8" wide concrete curb by 18" high. He tripped over a piece of rebar that was covered by weeds and fell. He bruised his knee and wrist. Following a visit to the doctor a couple days later, an x-ray revealed a fracture in the wrist. The employee did not lose work hours as a result of the incident.

A production employee was walking through the plant. He injured his shoulder while catching his fall when he tripped over a curb. This incident was initially treated as a first aid but has been elevated to a medical aid when he recently received medical treatment.
