Employee stepped down from 773B water truck access ladder and stated when their foot hit the ground it felt like they had strained their right knee. Ground was level and smooth in the area. Employee was taken for medical evaluation and placed on restricted duty on 9/3, released after follow up visit 9/14

EE was walking from shop to roll press and strained calf.
Employee was fueling an air compressor when employee slipped off of the second step of a folding ladder, landing on left hip. Rainy conditions may have contributed to the incident. Employee was taken to physician diagnosed with a bruised hip and possible lumbar strain placed on restricted duty.
Employee tripped and fell while measuring a silo. When EE lifted the cover, the silo was blowing dust out. EE turned head quickly to get out of the dust and tripped. EE fell forward onto the silo lid that is a 3'X3' box 18" off of the ground. EE's shoulder hit the lid first then the right side of face.
Employee was exiting the finish mill building went through dust curtain, started walking downstairs lost footing and landed on ankle spraining it.
Employee was traveling down an interior stairwell, when EE missed a step and rolled ankle. Employee was taken to Physician for medical evaluation, diagnosed w/ a sprained ankle placed on restricted duty. EE was released to full duty 7 days later. Stairs were clean, dry, well lit, and in good condition. Not a contributing factor. Employee stated EE just missed a step.
EE slipped on ice from storm the previous night resulting in a concussion. Hardhat was being worn.
Employee was exiting passenger elevator while holding a 6 ft stepladder, when EE stumbled and fell, landing on left knee and side. Employee reported incident to supervisor, was taken to OMP for medical evaluation, was placed on restricted duty (no lift push, pull,15lbs, no squatting or kneeling until re-evaluated, returned to full duty 2/29
EE was returning cardox tubes to proper location when EE slipped on floor. This strained EE's left leg.
Employee was walking down a flight of stairs wearing cleated over boots when EE lost their footing. The stairs were made of a perforated grating for grip. The cleat caught in the grating and EE's momentum caused EE to lose footing. EE hit the bottom but said was ok. EE was sent to the clinic as a precaution. EE was given a 25 lifting restriction until 2/2/2015.
During a clinker breaker choke, employee climbed fixed ladder to check load on cooler. When EE stepped down, he stumbled & fell against railing, hyperextending his little finger. Was checked by physician-possibly flexor tendon rupture. Regular duty initially, expect restricted duty after surgery-2 days restricted prior to surgery, 8 days lost after surgery, RTW w/restrictions.
Employee was in change house at the start of the shift, tripped over a bench, stumbled & fell. Didn't report incident to supv. Later that day, he mentioned it in passing, but no incident report taken. He was off the next day, worked the next day & then went to ER -Employee was placed on restricted duty 9/16(no OH work, climbing or twisting until recheck)Rtrn to reg duty 9/29
Employee was walking up a set of stairs and in his words, "tripped over his own feet", falling forward getting a finger caught in stair grating
EE was walking along the 603 feeder landing while performing an inspection. He was looking at the skirting and not the walkway. There was an open hole (15"x 15") in the grating that he stepped through. EE hit his knee on the side of the grating as he fell as well as his elbow.
Employee left control room and went down the stairs. At the bottom of the stairs, he stumbled and turned his ankle. Steps were clear, no trip hazards present. Employee reported incident, was taken to dr. placed on restricted duty (limited standing and walking) 6/24.
The employee was working on the stretch wrapper machine. He had to bring down the tower, do the Lock out and go into the area where the dispenser is. At that point he hit the top sheet dispenser structure with his body and lost his balance getting injured on the left side his eye by hitting the base of the photocell.

The employee was walking up the stairs outside the Roll Press building and apparently caught the toe of his boot under one of the steps causing him to fall forward. The employee fell forward landing on his RT elbow.
Employee was leaving the Change House at Plant 3 at the start of his shift. As he stepped outside the double doors of the Change House lunch room, he slipped on a patch of ice. He cut is left hand and suffered a hairline fracture of his RT hand.
The employee was standing on top of a cover to the outside incline drag conveyor in order to remove the top section of the head pulley guard to #2 drag conveyor. The employee received a call on the plant radio, and when he reached for his microphone, he lost his balance, falling to the ground. He was standing 3' 10" off the ground when he fell.
Employee was working inside of a dust collector hopper repairing a conveyor screw. When he went to turn around, his foot was caught in the flights of the screw. As a result, he twisted his right knee. He was taken to a Dr. for evaluation, and placed on restricted duty (on 2/5)(no repetitive kneeling, squatting or bending) until follow up appt. (2/13)
Employee was working above the old Finish Mill#1 roof in a manlift trying to remove a purlin, He step out of the man lift cage & walked on the roof, the roof collapsed & fell aprx.4 feet hanging from his arms between a structure channel and a cross bar;& rescued by another manlift;he suffers scratches under his arms & hands; he was wearing the PPE including harness fall protection.
Employee reported that the Doosan forklift used in the warehouse had a defective horn. They drove the forklift to the tuck-shop and tag it out of service. While walking back to the warehouse, the employee reported that they slipped on the ice near the iron ore ramp.
Employee was moving the godwin water pump at quarry #3. As he was walking up the gravel road, he slipped on the wet gravel falling to the ground. He suffered a cut on his LT wrist which required some stitches.

Employee was walking down the ramp passing underneath the marginal belt conveyor and slipped loosing his step. He tried to maintain his balance to avoid the fall, he twist his body with force and felt a sudden pain in his lower back. The employee reported immediately to his supervisor and notify discomfort he was feeling in his lower back.

Injured slipped on ice when walking from parking lot to office and sprained his ankle.
Employee was operating forklift, dumping a skidbox. When he exited forklift, he stepped down on a crack in the concrete pavement, spraining his right ankle. He was seen by physician, diagnosed with a sprain with a fibular fracture, and will likely be off work for 10 working days, with some restricted duty after that.
EE was leaving the warehouse 1 location going to lunch. She was walking pass the big hanger bay door and tripped over the forks on the Grade-All which were pointed towards the hangar bay door. EE could not get up immediately after she fell so she crawled to the side walk and set there for approximately a minute and then got up and proceeded to walk back to her office.
Employee was sent to the cooler electrical room for cleaning and maintenance purpose. When finished the job he passes in front of the cooler hydraulic room that had tree leaves and mud in the floor he slipped but not fell and the force made to avoid the fall cause the right leg muscle to strain.
Lubricator had just checked oil on vent fan bearings. He climbed down the vertical access ladder and was standing on an 11" high concrete pad at the base of the ladder. He turned to leave, his foot rolled off the corner of the concrete, causing him to fall, landing on his elbow. He received a cut which required 3 stitches, was placed on restricted duty for 6 days
The injured slipped on some clinker on a concrete floor and fell against a corner of a partially crushed 55 gallon drum and suffered contusions of his ribs and side.
Employee was at water filter plant, priming the water pump. The bleed valve is about 7' overhead. Employee chose to stand on motor to access valve. He lost his footing, fell, striking his arm on the motor, cutting his forearm. Employee was taken to OMP & received stitches on his arm & placed on restricted duty. Employee traveling on foot, stumbled over 1/4" checker plate covering opening in floor. When he stumbled, he pulled or strained muscle in lower back. Plate was flat, but had thin layer of dust on it making it harder to see. Employee sent to physician, placed on restricted duty for 5 days, released to full duty.
Employee tripped on a low mound of material on the floor and jammed his toe against the inside of his steel toe boot.
Injured stepped on material in the walkway and felt a pain in his hip when he slipped and regained his balance. Sprain of right hip.
EE using 16' extension ladder(extended out two rungs-10'overall)to access a cover plate on mill feed pipe. EE was on 2 or 3rd rung of ladder (approx 2'-3' high) Rung lock wasn't fully engaged, ladder slipped, EE fell to ground. Sent employee to phys. for evaluation-was placed on restricted duty-no ladder climbing, heavy gripping with index finger. will re-evaluate 3-23
Injured got out of his personal vehicle in the parking lot, lost his footing on ice, fell on his back, and suffered thoracic contusion.
Employee was struck by a guard that he and a coworker were removing from the kiln drive and fell to the catwalk on which he was standing striking his right hip, back, and shoulder.
Employee tripped over a pallet of material in the Brick Shed and fell backwards to the ground, striking his left elbow on the ground, resulting in a laceration to his elbow requiring 4 stitches.
Office employee was walking from the parking area to the offices administration building, she slip and fall (same level) in the sidewalk at the entrances of the administration building during heavy rains. Employee was returning from her lunch break. As result of the fall the employee suffers a contusion on her left hand and minor scratches on both knees.
Climbing into mill from ground, employee reached the top few ladder rungs when the ladder slipped out from underneath him. Employee caught himself from falling through the hole by having his arms out to the side, bracing himself. He felt pain in his left shoulder as a result of the sudden impact.
Employee was exiting the hopper door on the #2FM, #1 DC screw when the stepladder he was using tipped over and he fell approx 5ft to the floor. He landed on his back and struck the back of his head on the floor. Employee was evaluated at Company Physician. Evaluation determined head contusion, no other injuries were noted. Employee was placed on a work restriction of no climbing.
Employee was completing a job using the vacuum truck. As she was returning to the truck she caught her foot on the vacuum truck hose and safety valve resulting in her falling to the ground, striking her right hand and right knee and landing on her right side.
