A miner tweaked EE's knee while accessing the boat to work on the water pump. The injury occurred on 5/15/2026 and was placed on restricted duty on 5/18/2026. The miner has returned to full duty on 5/22/2026 with no lost work days.

Mostrando 50 de 52EE was walking from the Shop to the ready line to get in their haul truck when they slipped on a section of ice approximately 18 inches wide by 24 inches long. Roadway and walkway was cleared but miner did not see this small section when walking. Miner was prescribed a muscle relaxer and returned to work.
EE was carrying a piece of metal less than 50lbs to service truck when EE lost EE's balance and fell to the ground landing awkwardly on right hand resulting in a broken finger. Area was free of debris and level.
Hurt ankle on 2/12/25. Did not report injury till after EE went to ortho on 2/17/25. Claimed to step off haul truck and foot slipped and twisted ankle a bit 5 days later. Ortho found that there was a fracture in the foot. After our investigation EE was put on workmen comp for 24 days until EE was released back to full duty.
Loader operator was exiting loader. EE had three point contact on the hand rail. EE's right hand slide off the rail causing EE to loose balance. EE swung to the left and couldn't hold self and fell. EE landed on EE's right side.
A miner was exiting the skid steer EE was operating when EE slipped and contacted a sharp angle on the manufacturer built fork attachment causing a laceration that needed stitches.
The EE was climbing down the ladder of the Haul Truck they were operating to check out the equipment for a possible fluid leak and missed a step as they were descending down the ladder and fell to the ground. The employee injured their knee when they hit the ground.
A miner was conducting EE's morning pre-shift on the Cat Loader EE was operating, while walking around the equipment the operator stated EE stepped wrong twisting EE's ankle and then stepped again twisting it again. The miner was taken to the medical clinic and issued a walking boot to wear for 1 week or less if pain goes away. Miner returned to work next day.
A miner was walking out to the dumpster from the Shop and attempted to kick a rock out by the dumpster and the rock was froze to the ground causing miner to fall down. When miner fell the miner landed on face and knocked a crown off of a tooth. The crown had to be glued back on by a dentist.
A miner was unplugging the Block Heater from the Cat 775 Haul Truck when EE bumped the back of EE's head on the truck fender causing a laceration. Injury resulted in the miner receiving staples to close the wound on the back of the head.
A miner stepped off the haul truck and felt EE's knee pop when EE twisted to walk away from the truck. A MRI was conducted and a torn ligament was found requiring surgery.
Employee was going down to work on a shaker, slipped on wet chute slime and caught self with right arm. From the date of incident 5/5/21 to 7/21/21 employee worked normal duty and normal job. On 7/21/21 employee had out patient surgery to fix rotator cuff.
EE cut the old cable and let it fall to the ground. EE then somehow got it wrapped around their leg and stepped on the other end. When EE started to walk, EE tripped self and fell to the ground/strain/sprain R shoulder. Employee was operating haul truck in pit when EE pulled over on level ground to take a quick stretch and walk around the machine to do a mid-day equipment check. When EE reached the ground level from the ladder, EE turned to the right to begin walking but EE's right foot was against material on the ground. When EE rotated laterally, EE felt/heard a pop followed by knee pain.
EE was walking to a conveyor belt to check the condition of the belt splice, when EE slipped on a patch of ice and fell. Initial diagnosis was shoulder pain, later diagnosed with rotator cuff tear.
Walking to work area, Ee attempted to step over caution tape. The tape hooked on the heel of boot, causing EE to fall on leg.

He instead struck a crusher wear liner laying on the ground lacerating the palm of his left hand. He was wearing all required ppe including leather gloves. He received 5 inner and 14 outer sutures.

Employee was heating up catch-alls with propane tank & wood burner under Conveyor #25. As employee ducked to go under the conveyor his right foot slipped and he fell. As he fell his right leg twisted - felt pain at inside of right knee. Employee was carrying a propane tank & weed burner so he couldn't catch himself from falling. Went to clinic on 3-11. His right knee was sprained.
This person is NOT an employee of Crawfordsville.He is a truck driver for A&C Trucking.The driveway was paved over weekend,the stairs to the scalehouse had been removed. Trk Driver had stacked 2 blocks in front of the door. When trk driver went out the door, he missed the step, fell on his left elbow. Had swelling-pain. Received a sling,was restricted, returned to work sameday

EE alleges he fell off a front end loader on 10/27/10 sustaining multiple injuries including a closed head injury during his course of employment. EE did not report this on date of alleged injury. There were no witnesses. Supervisor denies there was an injury. Employee filed application for adjustment of claim on 1/5/11. Employee was terminated on 11/1/10 for attendance issue.
Employee was climbing down the ladder on the 998 Cat Loader and lost his grip and fell approximately 5-6 feet to the ground and landed on right side of his body. Employee twisted his right knee and received a sprain to right knee. Employee was off work one day.

EE WAS GETTING OFF LADDER WHEN HIS FEET GOT ON THE GROUND HE SLIPPED ON THE SNOW & ICE & FELL BACKWARDS INTO THE FUEL TANK STRIKING HIS HAND AND CAUSING A LACERATION TO HIS HAND. EE RECEIVED STITCHES.
EE WAS INSTALLING A LIGHT. EE WAS CLIMBING OF THE ELECTRIC TRAILER ONTO A LADDER. EE FELL OFF THE LADDER AND HIT HIS RIBS ON THE STAIWAY. EE WAS RESTRICTED AND RECEIVED A PRESCRIPTION.
EE WAS WALKING TO GET A CROWBAR FOR ANOTHER EMPLOYEE AND SLIPPED ON ICE. EMPLOYEE'S LEFT LEG WENT FORWARD CAUSING EE TO FALL BACK ON HIS RIGHT ANKLE. HIS ANKLE WAS BROKEN.
