Employee was climbing down the mounting ladder of EE's yard loader when EE missed the last step and fell to the ground. EE attempted to catch self by extending EE's left arm which resulted in a fracture to EE's left wrist.

The employee climbed onto assigned loader to initiate the pre-shift inspection. While climbing down they thought they were on the bottom step when they were actually on the second step. When they released to touch the ground they fell further than anticipated causing them to fall to the ground. They suffered a fracture to the left wrist.

The employee stepped on a metal bracket to climb on to a conveyor frame EE was disassembling. The weld holding the bracket on failed causing the employee to fall approximately 3.5 feet. While falling EE attempted to grab the conveyor frame and lacerated left palm. The laceration required stitches.

Employee was climbing back onto EE's haul truck when EE slipped from the first step back to the ground. Its now known that this caused a small tear in EE's right achilles tendon. The diagnosis and treatment plan occurred on 1/7/2020.

While dismounting the shovel the employee slipped from the last step (18" high) causing EE to roll EE's left ankle.

Employee was refueling his loader and stepped on the edge of the concrete base, approximately 6 inches above ground level, severely twisting his ankle.

Employee fainted in the employee parking lot next to his car after his shift was completed.

Employee slipped off of a step while cleaning loose material and sprained his right ankle.

Employee slipped on loose ground while crossing a ditch.

Employee strained his right shoulder by grabbing the handrail to avoid falling from a slip on steps.

While standing on the ladder and reaching to close the cab door of a loader, the operator lost his balance and fell to the ground.

Employee lost his footing while walking down a shallow slope in the pit. In trying to break his fall, he sprained his left wrist.

Employee stepped down from the ladder of the water truck and turned his ankle, resulting in a severe sprain of the right ankle.

Employee was climbing into the cab of a Hitachi shovel when his left foot slipped and his left hand gave way, causing his entire weight to pull against his right shoulder.

EE WAS STEPPING INTO A FEEDER BOX WHEN HE SLIPPED FROM THE EDGE OF THE FEEDER BOX SIDE PANEL. WHEN HIS 3-POINT CONTACT WAS LOST HE EXTENDED HIS RIGHT ARM FOR BALANCE AS HE STEPPED DOWN TO THE BOTTOM OF THE FEEDER 2' BELOW. HE STRAINED HIS RIGHT UPPER ARM.

AN ENCLOSED CAB DRILLER STEPPED OUT OF HIS CAB TO THE ADJUSTING WORK PLATFORM TO LOOSEN THE PIPE JOINT ON HIS DRILL STEEL. WHEN HE STEPPED BACK TOWARD THE CAB CONTROLS HE STEPPED IN THE OPENIN G BETWEEN THE CAB AND THE WORK PLATFORM. HE BRUISED HIS RIGHT WRIST AFTER SLIPPED DOWN.

EMPLOYEE WAS STANDING IN FRONT OF 2 FT HIGH CONCRETE BLOCKS. WHEN HE TURNED TO WLAK AWAY FROM JOB, HE TRIPPED OVER THE CONCRETE BLOCK, FALLING ON HIS RIGHT ARM. HE BROKE HIS ARM ABOVE THE ELBO W, BUT RETURNED TO WORK WITH RESTRICTED DUTY.

EE LOST HIS FOOTING STEPPING DOWN FROM REAR LOADING DOOR OF A FREIGHT TRUCK. HE FELL TO THE GROUND LANDING ON HIS LEFT ELBOW AND CAUSING A CONTUSION TO HIS LEFT SHOULDER. RESTRICTED LIFTING WI TH HIS LEFT ARM.

EE TWISTED HIS RIGHT ANKLE GETTING DOWN FROM CRANE HOIST DRUM TO CAB FLOOR 3-4 FEET BELOW.

EMPLOYEE WAS WASHING DOWN CONCRETE PAD WHEN HE STEPPED ON THE EDGE OF THE PAD. THE PRESSURE FROM THE WATER HOSE PUSHED HIM OFF BALANCE CAUSING HIM TO TRIP BACKWARDS STRIKING HIS HEAD ON A ROC K WHEN HE FELL. THE CONCRETE PAD WAS APPROX 12" ABOVE THE GROUND. THE INJURY RQUIRED 9 STITCHES.

EE WAS CLIMBING DOWN FROM FREIGHT DELIVERY TRUCK WHEN HE SLIPPED AND FELL INJURING HIS RT ARM AND SHOULDER. THE EE HAS HAD NO LOST TIME FROM THE INJURY UNTIL 3-16-98, WHEN SURGERY WAS REQUIRED . THE EE HAS NOT RETURNED TO WORK AS OF THE DATE OF THIS REPORT.

THE EMPLOYEE STATED THAT HE TRIPPED OVER A PIECE OF REBAR. AS HE FELL HIS RIGHT HAND BETWEEN THUMB AND PALM WAS CUT BY A ROCK.

EE STATED THAT HE WAS STANDING ON THE RIM OF A CONCRETE FORM AND STEPPED DOWN TO THE GROUND BELOW, WHICH WAS APPROX. 12"-15"/ HE TWISTED HIS ANKLE CAUSING A HAIR LINE FRACTURE.

WATER TRUCK OPERATOR LOST HIS BALANCE WHILE WETTING THE MUCK PILE HE FELL TO THE GROUND & CUT HIS HAND ON A ROCK REQUIRED 4 STITCHES

EMPLOYEE WAS SITTING ON TOP OF A CAGE GUARD. TIGHTENING THE CHEEK PLATE BOLTS, WHEN HE SLIPPED AND FELL. HE LANDED ON HIS FEET, CAUSING HIM TO SPRAIN HIS RIGHT KNEE.

WALKED OVER TO PIT WALL AFTER BLAST, GROUND BENEATH HIM GAVE WAY CAUSED HIM TO FALL ABOUT 12 FT TO ROCK PILE

EMP STATED THAT3HE STEPPED OUT OF THE TRUCK ONTO THE FENDER TO GET SOME AIR, WHEN HE STEPPED AROUND THE MIRROR HE LOST HIS FOOTING AND FELL TO THE GROUND, RESULTING IN A FRACTURRE TO RT HI HIP 3
