Contractor was working approximately three feet off of the ground, standing on a four foot step ladder. Contractor shifted weight causing him to lose balance and fall to the ground. Ligament was torn in right knee.

Dozer operator smelled diesel fuel, shut engine off. When employee stepped out of the dozer, flames began coming out of the engine compartment. Employee discharged one fire extinguisher, the fire did not appear to diminish. Superintendent advised the employee to move away. Upon arrival the fire department could not reach the dozer. An excavator was used to put dirt on the fire.
While employee was performing the pre operational check on the Kenworth Tandem Coal truck he was going to operate, employee attempted to raise the hood by himself. As employee pulled the hood he felt a pain in his right arm. This resulted in a strain to his arm.
While employee was pulling a piece of 6" plastic pipe approx 50' in length, by hand, down hill, the pipe struck a rock causing it to bounce into the air and the metal flange to strike the employee's left ankle. This resulted in a fracture to the ankle.
While using a track hoe to pull pipe out away from a pump three employees were on the ground in order to connect and disconnect the strap to the machine. During the pulling process a metal flange became lodged causing the pipe to pull tight. When the pipe dislodged it flexed in the direction of the employees, striking and causing two of the employees to fall.

While walking up a ramp during wet conditions the employee slipped resulting in his left knee being twisted. The employee had walked down the ramp in order to be able to assist in fueling a pump.
Employee was climbing onto a 980C Caterpillar Front End Loader when the bottom step pulled out of the bolts. Employee grabbed the handrail with his right hand to restrain himself. The force caused his rotator cuff to be torn in his right shoulder.

The dozer employee operates was being welded on. Employee stood in the area while welding was being performed without the proper protective equipment on causing eye irritation.
It was close to getting dark outside, time was a factor, employee wasn't paying attention to what he was doing. Damage is contusion to 3 fingers, stitches required in 1 finger.
EE dayshift foreman,was welding using a welding hood that belonged to someone else. The hood did not provide adequate protection for ee eyes. EE eyes were irritated due to the welding. EE was written off work for 10/18/2010 and returned to work on 10/19/2010.
While traveling down the ramp toward the PC2000 trackhoe, the brakes on the Mack service truck that injured EE was riding in failed. After being told twice by the driver, other ee jumped from the truck. He received minor scratches on his left side. He was x-rayed and showed no injurys. The doctor stated that he had mild bruises.
While traveling down the steps of the PC2000 with a hydraulic pump hose in his hand the hose became hung. Injury pulled on the hose. When the hose was free it caused the reel to release and pull the hose causing him to loose his balance. His right leg lodged between the track and step. He has a bruised knee and sprained ankle.
Operator was getting on 375 Komatsu Dozer, he put his foot on the step on the side of the push arm and grabbed the handle on the tilt cylinger, his hand slipped off the handle and he fell backwards onto the ground on his backside.

Employee was replacing an air compressor on a Mack water truck. The compressor mounts in close quarters, and slipped, mashing his little finger against the truck frame. The little finger was cut, requiring 3 stitches.
Employee was walking down the side of a spoil pile to move a piece of 8 inch plastic water pipe. Before he got to the pipe, his feet slipped on loose rock. He fell to the ground, breaking three bones in his right foot. He was wearing safety toe, slip on boots with smooth soles.
The rock truck driver backed up to the trackhoe to be loaded. The spoil rock on the left side of the rock truck slid down against the truck wheels. The action jarred the truck. The driver claimed his back was hurting, and was taken to the emergency room for examination.
Employee was opening day-box when a gust of wind slammed door causing contusion to right hand middle finger.
The victim (mechanic) removed fuel injector from engine and then instructed his helper to turn the engine over. Diesel fuel and/or air came out of the 1/4 inch injector hole and struck him in the left eye and entered his brain, causing fatal injury.

While pushing overburden in the east end of the west pit, a bulldozer hit and detonated a primer. The only damage sustained was a broken lense on left corner blade light.

A direct lightning strike on a seven foot deep bore hole loaded with one foot of explosives, caused a premature detonation of the borehole. Flyrock from the detonation shattered the rear window on a Komatsu D475 bulldozer, no injuries involved.
Employee had finished shift, parked drill and was climbing down from machine. When he stepped on ground and released grip on ladder, he twisted right leg. Employee did not insure proper footing before releasing grip on ladder.
EE stated he lowered stands on coal crusher and then attempted to pull the fifth wheel release lever. He stated the release lever was hard to pull and he felt something tear in his neck.

Employee claims he was walking across hood of rock truck to clean right side mirror. Because of wet weather the hood was wet and slick. The employee slipped and fell onto the hood. THE EMPLOYEE DID NOT REPORT THE UNTIL 09-25-2006, AND DID NOT BEGIN LOSING TIME UNTIL 10-05-2006.
Employee stepped off back of service truck, slipped and fell on his hand.
EE was running a front end loader on a coalyard, feeding a coal crusher. When the coal crusher stopped-up, EE parked his loader and jumped to the ground, a distance of 6ft. He did not begin losing time until 7-27-06 and did not notify management that the time loss was related to the accident until August 1, 2006.
