employee cleaning tools & materials off bed of maint truck. Emp bent over to pick up choker off ground, had hand resting on edge of storage box and lid fell on his left hand, causing a fracture of the 3rd metacarpal of left hand.
Employee was installing a section of 20 inch steel pipe to a 90 degree fitting. Two employees utilized hand tools to attempt to free a frozen nut. Employees hand was rotated toward the pipe causing his right ring finger to be caught in between the open end wrench and the pipe. Employee received a laceration requiring sutures to correct.
While Mechanic's Helper was swinging sledge hammer, he missed intended target; causing a metal fragment to chip off.
while removing bolts from the belly pan it fell; causing blunt trauma to pinky and ring fingers.
Employee was exiting the float truck bed area, utilizing manufactured steps for egress. Instead of stepping straight down, the employee stepped to the left corner of the bumper step. The employee lost his balance striking his left shin on the left corner of the step bumper, resulting in a laceration that required stitches to correct.
WHILE BACKING UP; OPERATOR HIT ROUGH AREA CAUSING HIM TO JOLT STRAINING HIS BACK.

employee was injured replacing the liner in a small trash can in the Electrical Dept's lunchroom. As he was tearing a new bag from the roll, his right index finger contacted a large piece of glass sitting on an adjacent desktop. The employee was transported to our clinic where 8 stitches were provided to treat the laceration
Small space that mechanic was working in limited visibility and was only able to rely on touch/feel.
WHILE HAULING FILL; OPERATOR HIT HOLE IN ROAD CAUSING SHARP PAIN IN HIS BACK.
While hauling fill; IW hit rock on haul road causing pan to jolt.
employee working was lubricating a 3/4 inch air ratchet with oil. While clearing excess oil from ratchet, oil mist sprayed into eyes. Employee treated and evaluated with chemical conjunctivitis in both eyes. He was prescribed anti-inflammatory and antibiotic medications and released to return to work with no restrictions.
While dismounting machine, IW stepped on uneven ground conditions, causing foot to slip.
EARLY MORNING AND THE SUN WASN'T COMPLETELY UP HOWEVER IT WAS LIGHT ENOUGH FOR GOOD VISIBILITY WITH OR WITHOUT LIGHTS BEING ON. BACK UP ALARM WAS FOUND TO BE IN GOOD WORKING CONDITION. The victim was struck by a road grader when it backed up. The accident occurred in the staging area where the victim and other coworkers were checking their equipment for the shift.
MS EMPLOYEES WERE MOVING CRIBBING AT THE CF HARDEE BUCKET YARD. EE'S WERE WORKING TOGETHER, STACKING CRIBBING ON A FORKLIFT. WHILE STACKING THE CRIBBING, INJURED EE GOT THE PINKY AND RING FINGER ON HIS LEFT HAND CAUGHT IN A PINCH POINT.
EE WAS USING A 4' PRY BAR TO SLIDE A PIECE OF DECK PLATE THAT HE WAS WORKING ON IN THE CF MAINTENANCE SHOP. THE PRY BAR SLIPPED AND THEN SMASHED HIS LEFT INDEX FINGER (THROUGH HIS GLOVE) BETWEEN THE PRY BAR AND A PIECE OF ANGLE.
Miner was hammering a keeper pin into place on a dragline bucket tooth when he missed the pin, striking the tooth itself with the hammer. A sliver of metal chipped off of the tooth and struck the miner between the first and second finger of his left hand. It required three stitches to close the laceration and the employee was released back to work with no restrictions.
While trying to loosen bolts on plate; IW felt sharp pain in right shoulder
While operating pan on dusty haul road; dust entered IW eye.
EE was attempting to insert a spike puller to remove a spike, when the action of hitting said spike puller dislodged a piece of an old weld that had adhered itself to the spike. When this piece became dislodged, it flew up & hit him in the neck causing a laceration that required two (2) stitches to close. EE was transported to ER for treatment. Released, no duty restrictions.
Mechanic was using a lathe to fab part for a track torch. Metal shavings spun around & caught his left index finger, causing a laceration. He was treated by the facility Nurse & sent to local clinic where he received 11 sutures and was returned to work with restrictions.
A mechanic was using a cutting torch to split a 20 pipe for a wear band. He cut thru the pipe & stored energy in the pipe caused it to spring open striking his left ring finger was struck by pipe. He was evaluated by facility Nurse & sent to local clinic for x-ray, which confirmed a fracture. His finger was set & splinted. He was released to return to work with restrictions.
While attempting to turn disconnect switch off; plastic casing came off release handle causing hand to slip.
A Maintenance employee was working in the Washer structure. Employee was standing on the ground directing a crane into place when he was struck in the mouth by a falling piece of rock. The employee was taken to Occupational Health Partners where he received 13 sutures. The employee was released to return to work with no restrictions.
EE was dismounting machine when his foot slipped causing his leg to hit track pad growser.
While attempting to check final drive on D8 Dozer #1702; IW was using a ratchet to pull up causing pain to in middle of back.
While using chipper hammer to clean rails off; hammer hit metal causing a piece of metal to come off corner of hammer and lodging into middle finger of right hand.
Employee was stepping over a matrix pipe line carrying a small aluminum ladder. Employee stated that he felt a pain in his lower back. A subsequent examination and evaluation by a physcian resulted in the diagnosis of a muscle strain. Employee was allowed to return to work on modified duty with work limitations pending full recovery.
A contractor employee was in the process of removing electrical zip ties from bundled electrical cords using a pocket knife. The knife slipped and the employee was cut on his left thumb through his cotton work glove. The laceration required three sutures to close. The miner was released back to work with no restrictions.
Miner was tightening bolts on a piece of equipment when a bolt snapped. The sudden decrease in resistance caused the Miner's hand to fly back, mashing his right ring finger between the wrench handle and a nearby pipe. This resulted in the Miner receiving 5 sutures on the affected finger.
Employee was engaged in cutting, grinding, and welding activities when he noted discomfort in his right eye. A subsequent examination later the same day by an Optometric Physician revealed the presence of a paint chip. It was removed and the employee was allowed to return to work with no restrictions.
Operator hit bump in haul road causing him to lose control of truck. Dump truck came to rest in ditch after hitting guard rail.
Employee did not have proper foot placing and twisted left ankle. In an attempt to break fall, employee injured left hand. Employee suffered sprain/strain to ankle and two fingers. No fractures.
Employee was driving a vehicle alongside of a materials pipe line, when a stud on the right front tire struck a pipe flange, causing the wheel to suddenly jerk. This in turn caused the steering wheel of the vehicle to unexpectedly spin, yanking the emploee's left thumb downward and causing a "Bennett Fractue".
Employee was attempting to empty a trash drum when he heard a popping sound in his right elbow. The initial doctor evaluation reccomended immobalization of arm until further testing. A subsequent examination by orthopedic specialist revealed a damaged right bicep muscle.
Laborer was moving tools & reports tweaking back muscle while pulling on rope. Laborer was immediately removed from jobsite to office to report injury and taken for medical evaluation. Local MSHA office and mine company personnel notified immediately.
Employee was operating an open cab tractor when he was struck on the right temple by a piece of PVC pipe that was lying in the back of a parked truck. Employee was briefly knocked unconscious and was taken to the hospital for treatment and evaluation. Employee was released and allowed to return to work the evening of the incident.
Employee was driving wooden wedges under the tub of a 1370 dragline to tighten the cribbing under the tub. On his backswing, he hit some adjacent cribbing, altering the path of the sledge hammer and hit his left foot.
An employee was removing lube piping while another employee about 10 feet behind him was operating a grinder. A piece of metal from the grinding hit the inside of his safety glasses and bounced into his eye.
Employee was lining up a trunion pin on a dragline bucket and his right pinky finger was caught in a pinch point.
While installing a mechanical gearbox in the Benification Plant, employee's left index finger was pinched between the gearbox and an adjacent handrail.
While replacing rubber coated material screens, employee struck his left index finger. The subsequent x-ray revealed a fracture.

Employee was working on the back end of a moving trailer when he lost his footing and fell to the ground. A Straw Blower machine attached to the back of the trailer ran over the employee's pelvic area.
Employee was climbing on to an excavator when his foot slipped off the track. He used his right hand to catch him self and this is when his right hand struck the track and caused a small laceration to his index finger. He recieved two stiches in his right index finger and no medications. Returned to work the same day.
Employee broke out in a rash on various parts of his body.
