The injured employee was helping to install electrical cable through conduit when a spool of cable unexpectedly shifted and fell off the stand striking lower left leg injuring knee, calf and ankle. On 6/11/19 the employee was given work restrictions which limited their work capabilities.

An employee strained their right knee on 10/31/2018. They did not have any restrictions placed on them on 10/31/2018. The employee went for a follow up recently and at this point was given restrictions and reported them to the supervisor.
An employee was working with a Southwire M6K Maxis to pull wire. The individual got the left little finger caught in the rope; the employee couldn't get finger out or shut the tool off in time, amputating the top of the employees' fifth digit.
Miner was cleaning/shoveling excess feed from the around the mobile rinser, loader operator placed material into hopper when a compacted segment slid from grizzly grid striking miner. Miner lost EE's balance / footing falling to the ground. During chain of events after being struck and coming to rest on the ground, miner sustained a fractured femur on EE's left leg.
Employee was cleaning under a conveyor with a ride behind skid steer. Employee pinched fingers between controls and conveyor. Employee suffered lacerations and broken finger.
Employee was aligning two pipe flanges using a Spud wrench. After spiking the bolt hole of one the flanges attempted to align the two flanges by pulling on the spud wrench. The spud wrench came out of the bolt hole striking the employee on the bridge of the nose.
Employee was walking to raw feed area to collect a sample for quality control. While walking, employee tripped in mud and fell down. First aid was performed. Employee went to an occupational clinic on July 16, 2018, where employee was treated for a sprained right knee.
Plant operator stepped onto grating. The grating shifted, causing the operator to trip and fall. First aid was provided. Employee went to an occupational clinic on July 18, 2018, where ee was treated for a sprained right knee.
Crane mechanic was working on the Link Belt Crane pulling the air cleaner. EE stepped back thinking EE was stepping onto the main deck from the catwalk. EE miss-stepped, missing the main deck and fell to the ground level. EE was not wearing fall protection.
Diesel Exhaust Fluid splashed into miner's eye when removing the fueling nozzle. A medical review was performed on 8/22/18 that diagnosed the miner with chemical conjunctivitis and traumatic cataract of the left eye.
Miner was removing cover on a bucket elevator, cover swung and struck miners arm. Miner received a laceration to miner's left elbow.
Dozer operator felt pain in EE's shoulder after driving EE's dozer into excavation.
Employee was closing hatches on top of a semi-trailer. Employee was wearing fall protection. The driver released EE's air brakes and began pulling off the scale. The sudden movement caused the employee to stumble backwards and EE fell off the side of the trailer. The employee struck the side of the trailer.
Employee was walking down stairs when EE missed the last step and fell.
Employee was using a 4 lb hammer and hammer wrench to loosen bolts on a Rotex. While swinging the hammer it hit a junction box in close proximity and caused the hammer to impact their ring finger on the left hand. Impact caused open fracture with blow out laceration at the tip of the finger.
Employee was leaving flocculant building when employee slipped.
Technician was on on-call weekend and was unloading and placing barrels. The technician had to cross over a concrete transformer pad. There was a hole that was not covered and the area was poorly lit. As the technician turned around the technician missed footing and tripping on the edge of the hole and foot going into hole.
Wet Plant belt #2 employee laying on cat walk on conveyor trying to brush material off belt and pulley. Wire brush got caught and pulled into belt, employee let go of wire brush & hit fore arm on 2.5 inch angle iron.
Employee was removing a steel plate (16"x 16") with the assistance of another employee, one was holding while one was cutting, when the steel plate was freed up the employee was in an awkward position and tried to handle the plate by himself. Resulting in a strain to his right arm and was given work restrictions returning to work.
Miner smashed right middle finger while changing tail pulley on belt Conveyor.
Team Member WAS CLOSING A RAILCAR LID AND WHEN I DID I FELT A POP IN HIS LOWER BACK AND INSTANTLY HAD PAIN. IT WAS A CLEAR NIGHT, NO RAIN. HE WAS DOING HIS NORMAL JOB AND JUST MOVED WRONG
Employee was working on removing ice as he stepped off the last step on the ground level set of stairs with my left foot and slipped on a flat sheet of ice. Investigation Findings: At the bottom of the stairs on the line 1 diester there was a layer of packed snow and ice covered by water as the ice was melting. There were no signs of ice or snow on the stairs or hand rails.
Employee was getting air filters off the warehouse rack. He was using the 5 foot mobile stairs instead of the 8 foot stairs. when he reached for the filters on the top shelf at a height of 12 feet he lost his balance and fell backwards off the stairs, landed on his right heel. Employee has a broken heel
Employee driving a Volvo 180 loader hit 3 bumps in travel way and strained his back. Employee brought to ER and diagnosed with strain back and put on light duty.
Employee grinding weld using 4.5" grinder, he stated it kicked-back he stopped to inspect disc, noticed a small "nick" & stated he thought he should change it but he decided to try to finish the 1" of weld left to grind. Restarted it & it kicked out again & he lost control of the tool. Cut to R-arm that required several stitches.
Employee was walking to the stacker belt after a heavy rain storm, there was an accumulation of water he tripped fell forward and hit his head on the metal frame, leaving a red mark, there was no excessive swelling, no loss of conscience.
Employee was taking a hydraulic line off Volvo 220G loader.
After getting out of loader, operator turned and twisted right knee.

Employee was preparing to clean computers in control room when he set the air compressor down his back had a spasm.

Employee was working in a dozer when he decided to exit the vehicle. At this time he caught his left foot on the foot rest and twisted his left knee. Employee hurt his knee on the 5th and sought medical attention on the 6th of November.
Cutting outboard bearing on a 2 ton fan on south side of building. Bearing would not slide off and had to cut off with grinder. Outside cuts were made. When working on last inside section, the grinder blade shredded causing pieces of blade to hit top of the employee's left hand. PPE worn: hearing protection, face shield, mechanic gloves, safety shoes, and safety glasses.
While placing a pipe onto a pump, the pipe rolled and pinched left middle finger between two flanges, broken finger
Employee did not report until 8/19/13 after his supervisor asked why he was wearing a brace on his wrist. He stated he injured it on 8/14/13 not really sure how it was just sore. At this time he did not seek medical treatment. On 8/21/13 the employee went to the Urgent care they diagnosed him with tendonitis and told him to wear a brace at all times and gave him restrictions.
Employee was trying to locate a bolt on a CAT D6 Dozer and tweaked neck while looking for bolt in awkward position.
Employee was shoveling Sand off of a concrete Pad turned wrong and pulled a muscle in their lateral side of back.
Employee was walking on locomotive walkway and slipped on oil causing him to fall onto his RT knee.
Employee was bending over to change out screens. He felt sore afterward but did not report until 12/10/2012. His doctor placed him on restricted duty.
The employee was checking the oil on a CAT 980K and while running a rag down oil dip stick was impaled by a cable spur into his left index finger. He came immediately and reported the incident to Safety Coordinator. After looking at his finger they transported him via ambulance to RMCH in Gallup.
EE and another employee were trying to flip the undercarriage, when it slipped and fell on his arm. Employee cut wrist, needed sutures.
Employee bypassed guard on belt conveyor to clear sand built up around the tail pulley. Employee failed to de-energize equipment and perform Lockout/Tagout procedures. The shovel made contact with the belt, it pulled the shovel into the tail pulley. When the shovel was pulled into the tail pulley it began to spin striking the employees left arm with the shovel handle.
The haul truck operator was operating the haul truck when he hit a bump in the road causing the fire extinguisher to momentarily discharge. The operator finished the rest of his shift for the day without complaint to his condition. The employee reported to work for his next scheduled shift and worked 6 hours before complaining of shortness of breath.
The employee was removing rocks from the rock crusher. One of the rocks shifted and smashed his finger.
Over a two day period the mine received a significant amount of rain creating erosion around a temporary impoundment.
