We were notified on 1/28/2016 that EE had been ruled compensable for a black lung claim.
Victim walked over a bank & picked up water hose. As EE was coming up the embankment another man opened the valve. The pressure caused the hose to jerk EE around and fell over the bank. Tore left bicep.
Victim was making repairs on a 992-G loader. As he was dismounting a tool caught on the rail and made contact with his hand. Resulted in a laceration that required 10 stitches in his right index finger.
Victim states he was loading shot rock into truck when a rock slipped and rolled back onto the loader bucket causing his back to be injured.
Victim was loading shot spoil into trucks when he struck a large rock in the spoil. The sudden stop jerked him forward into the seatbelt causing back pain and also jammed his right knee into the control box injuring his knee. Further tests are scheduled to determine extent of injury to his knee.
Employee was attempting to dislodge a stuck conveyor on the highwall miner with a 5 foot pry bar when the bar slipped and struck him in the left jaw, leaving two lacerations. Employee chose not to receive medical attention at time of accident and finished the shift. Employee later decided to get medical attention, at witch time he did receive stitches to close the lacerations.
Employee was stepping up on the 777 truck #4012 when he slipped on step. Employee claims to have strained lower back and right leg. He reported both are sore.
5:50 pm security Officer noticed black smoke coming down valley, went to check it out. Found 992G Loader on fire. 6:06 pm called person from Raven Crest. 6:08 pm called 911 at time of arrival on scene fire was coming out around both back tires and up over the top of them. Didn't see any one around, Security Officer went back and waited at post 4 on fire trucks.
Fueler/greaser was greasing a Cat 980 loader from his service truck when the pressurized grease supply line ruptured discharging a stream of grease at pressure high enough to puncture the employee's skin and inject grease into his right thumb. The grease had to be removed by a surgeon.
A piece of slag being chipped from a recently welded piece of metal went into employee's left eye. Medical treatment was received requiring prescription eye drops there is no lost time with this accident.
Hit hand with hammer between the thumb and index finger on left hand while hand was placed on pipe. Requiring stitches between left thumb and index finger on left hand. Employee returned to work next scheduled work day. No lost time
Employee was welding and fell. Employee fell on 6/24/13, he didn't think he was injured and chose not to seek medical aide. On 6/26/13 he reported to his supervisor that he felt that he had damaged his ribs in the fall. There is no lost days - employee is on light duty.
Moving steel and rolled a T brace on left foot resulting in contusion. X-rays negative. Seven days restricted work. No lost time. Restricted work provided by employer.
Employee was transferring chemical from a tote when one drop of chemical splashed over the top of the employee's safety glasses and into his right eye.
The Equipment Serviceman was removing the drain/fill plug from the final drive of a dozer and while turning around, he felt pain in his back.
The loader operator was digging a rock off of the edge of the pit when the rock slipped off of the loader bucket, rolled into the pit causing the back end of the loader to bounce up and down in the pit and later he felt pain in his back and neck. EE started missing on 10/5/2012.
The truck operator had backed the truck to the toe of the berm, set the dump brake & was dumping the load when the spoil material hung against the tail gate causing the front end of the truck to be lifted into the air causing the cab to turn onto it's side. He was taken to the hospital, was examined, released & returned to work & finished working the remainder of the shift.
The truck operator was observed by another truck operator pulled over to the side of the roadway and parked. His head was bowed, the other operator stopped, climbed onto the truck and shook the victim and called out the victim's name but the only response was the victim's eyes would flutter when his name was called.
When descending ladder from loader employee sprained his right ankle when shifting weight to right foot as left foot left the last rung of the ladder.
The Dozer Operator was working the slope when he trammed over a rock which resulted in the operator being twisted causing a back sprain.
The dozer operator was tramming off of the dump, backed over a large rock and felt pain in the lower left side of his back.
The back dump operator was descending the ladder of the 777C Cat truck, missed the bottom step and that shifted his weight to his left arm and he heard a popping sound in his chest.
Employee dumping D300E articulating truck. Load stuck, raising cab off ground, allowing cab to turn onto its side.
The operator ran the loader off the right side of the road onto the bank and turned over in middle of the road.
After fueling grader, climbing down, right foot slipped striking right knee against machinery.
Employee was performing clean-up at the portable crusher.
Employee was attending annual safety retraining. During lunch break, two employees were "horse playing". EE attempted to pick up another employee and throw him over his shoulder. Both employees fell to the ground, causing EE to dislocate his hip.
Employee was repairing a coupling attached to the feeder with a hammer. A piece of metal lodged in his finger causing a laceration. Employee didn't seek medical attention until 3/01 when finger became infected.
Employee's hand and wrist began to swell and became sore. He states that climbing into and out of the cab of the articulating truck caused the swelling. He did not fall.
Location: office area. He slipped and fell due to the ice and snow in the office area going out to get in his truck.
employee was operating a dozer trying to remove a large rock from the pit. while trying to perform this task, he backed the dozer over a large rock and the dozer hit the ground on the other side of the rock. Employee complained of back pain.
Spontanious combustion occured in coal being stored at the No. 1 upper stockpile/blending area. Dirt was used to cover and extinguish the flames. This action was performed within approx. 15 to 20 minutes of discovery.
employee was entering the drill cab and slammed the door on his right hand thumb, causing bruising and contusions.
Employee was putting his foot down off of the drill and lost balance, spraining his ankle when he hit the ground.
EE was cleaning out rear differential of a loader he removed all the bolts and the support fell on his fingers.
Employee was walking from the miner to the water valve to turn it off when he slipped and fell. As he fell, he tried to catch himself with his left hand and arm. His hand was bent inward causing a slight sprain of the hand and wrist.
Employee was assisting a co-worker while making a splice. Employee attempted to move the knocker prior to the coworker finishing hitting the knocker. Employee's hand was struck by the hammer.
Location: Auto Coal Sampler at the crusher plant. Checking secondary sweep on sampler. Had his hand in the secondary, secondary made a sweep and pinned arm and hand in the belt.
Employee came into contact with electrical cable and received shock.
Employee was bolting and let the head down catching his finger. The tip of his middle finger was cut off of his left hand.
Employee was mounting D-10 Dozier to check oil, felt pain in right leg. Unable to walk normally. Feels like muscle cramp continually. Pain in back side of right leg between knee and hip.
Employee was climbing down the ladder of her Rock Truck when she lost her footing and fell to the ground. She was taken by ambulance to the Pinveville Community Hospital where she was admitted and still remains.
The mantrip buggy got stuck. The men were pushing the buggy out of the hole. The injured employee had his left foot between the buggy and rib. As the buggy came out of the hole, it got traction and the wheel turned and caught his ankle between buggy and the rib. The caused impairment to his left ankle, chipping the bone and contusion on the surface.
The employee was installing roof bolts with straps and the straps slipped, swinging around striking him across the nose and right arm causing lacerations to both.
Employee was servicing an escavator with hydraulic oil. The employee was standing on the tracks of the escavator when he lost his footing and fell to the gound. The employee landed on his shoulder. The bone in the shoulder was cracked, but not broken.
The employee was replacing the sieve screen holder on the cyclone refuse bin. His foot slipped and employee hit his chin on the sieve holder. The cut required 11 stitches.
Employee was using knife to cut plastic zip tie. Knife slipped & cut approx 2" in length to left thumb/wrist. First-aid; Hospital ER; sutures to close wound & Tetanus injection.
Cutting pvc pipe with skill saw, possible shavings buildup prevented guard from functioning properly, after cut on pipe made employee was placing saw down and cut leg when crossed upper thigh.
Employee stated he was descending stairs, stepped from stair to floor, lost his balance and he fell causing him to sprain his ankle.
Lowering of tools (chain hoist) from a raised location to floor, when employee lost footing on platform and fell approx 2 ft. to floor. Sustained dislocation of left ankle and hairline fracture of bone in left foot.
