Employee was opening a box at the receiving desk with a utility knife. The knife slipped and they cut the top of their right thumb. Employee was taken to the emergency room where they received three stitches on the top of their thumb.

Employee was uncoupling line inside storage room and a piece of metal on the pipe clamp cut their pinky finger while they were lifting the line. Employee was taken to the emergency room and given five(5) stitches.

Miner was loading a round when a chunk came down and struck Miner on the right side of Miner's head and shoulder area. Miner was brought to surface and taken to the hospital with an ambulance and helicopter
Historical inundation of run off water from rain.
EE was cutting insulating tape for termination in oil switch at Mahler white dole stockpile transformer bay. While cutting EE cut right hand with hook knife when removing tape. EE went to the ER and was given 4 stitches in right palm.
10:35am EE was jack legging to install pins and screen in the back of Upper Mahler Incline on LT3435 when the steel broke about 5in from the machine due to a flaw. The Leg fully extended. EE went over handrail at height of around 13ft 5in landing on EE's left side & hitting EE's head. Mine Rescue stabilized and transported from heading to surface on back board and cervical collar.
IE and co-worker were tying in a round in MWD 4163. Working their way right to left, IE was past the cut tying in the last 3 holes when a chunk of ore from between 2 slips fell striking IE and the handrail. Mine Rescue was dispatched and brought IE to surface for the Ambulance. IE suffered a mild concussion, 3 staples in their head and a bruised hip.
A Miner was underground for their shift when they started experiencing chest pain and numbness in their arms. Miner was brought to surface immediately and transported to the local hospital by ambulance.
Miner was riding on man carrier of tractor; the tractor was traveling up a ramp when it stalled and started rolling backwards. The miner then jumped off the moving equipment spraining left ankle.
Miners were removing tank inspection door which is steel plate 28"X40". Top nuts were loosened and left on. The remaining nuts were removed. Miners hit door to relieve pressure. Once removing top nuts the door was still hung on studs. They tugged on door. Material shifted causing door to push employee back against the belt guard causing them to turn, twisting the right ankle.
Was walking looking up at mining area, tripped over rock, put hand out to catch fall and landed on a rock that cut hand next to thumb. Transported to surface.
Miner was operating LR1033 in New Fold escape drift when turbo caught on fire. Miner deployed fire suppression system, it did not put the fire out. Suppression system possibly malfunctioned or did not operate due to wet streams being too concentrated. Miner then used 10lb extinguisher to suppress flames. Miner was taken to local hospital for evaluation and released the same day.
An Exterminator with Orkin was walking through a storage area looking for a trap when EE tripped over an unseen object and fell directly onto left shoulder/upper arm causing a fracture.
After the end of shift an employee slipped and fell in parking lot while walking to EE's vehicle. The fall resulted in a sore shoulder that was a bruise or sprain.
A miner was leaning out over a fall protection railing and in a downward direction and was standing with one foot upon a round scaling bar. The scaling bar rolled causing the miner to lose balance and fall approximately 7 feet to the ground below. As a result of this fall injury was caused to miner's shoulder.
The miner was standing on a ladder and pushing up on a roof hatch door on the new shaft cage in the shop. The hatch door weighs approximately 40-50 lbs. The miner felt the ladder move and EE's hand slipped on the door EE was holding open causing it to come down on EE's left pinky finger tip.
Miner was drilling when the striker bar broke inside the drifter. The miner used a hammer to try and free it, the coupler was struck with the hammer causing a sliver of metal to come off and embedded in miner's left forearm just above the wrist.
Miner got brake cleaner spray in left eye, treated by a doctor and prescription provided.
Miner felt a pop from the right knee as EE was stepping up to another leveled surface. Miner woke up the next day to a swollen knee and pain that required medical treatment.
Miner was helping install long support in the ladder raise, the miner was installing a dag. The Jackleg kicked out and the dag slapped down on the miners right side of the hard hat and then the right side of the face by the right eye.
Miner was dismounting tractor, as miner stepped off the tractor miner stepped on a rock and rolled ankle.
The operator of a haul truck lost control on the main travel way. The haul truck struck the rib with a full load causing the operator to strike their head and ribs off the inside of the cab causing bruised ribs and a concussion.
Mechanics were working on a loader in the shop that was having a hydraulic issue. They removed 2 cartridges and started to remove the 3rd cartridge for the stab cylinder. When the mechanic removed the 3rd cartridge it blew out under pressure causing EE to stumble back and strike chin on the metal table.
Worker was operating the controls of the power wedge with right hand while in a kneeling position in front of the axle and brake pack when the pack suddenly loosened and moved off of the axle. The pack, which weighs approximately 300 pounds, fell approxmately 20 inches before contacting workers left hand, crushing it between the brake pack and a plastic oil pan.
Worker was clearing muck away from collar pipe. EE grabbed a rock and when EE did another rock slide down and pinched EE's left thumb causing a laceration that required surgical glue.
Worker tripped over a rock, right shoulder was injured trying to stop the fall.
While manually adding a 10 foot HQ drill rod to the drill string the miner felt a strain in EE's right forearm.
Employee was inspecting trailer jack for installation. Jack was already extended, employee retracted jack by removing pin, not realizing jack was spring loaded, jack quickly retracted and caught EE's right thumb between foot and jack housing.
Miner was stepping off the jumbo drill with EE's right foot on the sill, when EE put EE's weight on EE's foot it slipped off the sill rolling the ankle. Miner was not maintaining 3 point contact when EE slipped.
Using a jackleg drill when the retainer collar came off causing the jackleg to twist pulling employee's right arm causing a pain in the right elbow. Was reported as first aid but due to on going pain the miner was treated by a doctor on 10/7/2019.
As the miner grabbed the whip-check to pull the jackleg drill down, a small piece of metal went through the glove into the right pointer finger. The next day the puncture wound was red and sore and appeared to be infected prompting the worker to seek medical attention.
Worker was using a jackleg to bolt the back in 2700-400 Mud Pond. A piece of rock about the size of a softball dislodged from the screen on the back that fell approx. 15' striking EE's hard hat and then the bridge of EE's nose. The rock caused a laceration to the bridge of EE's nose. The miner received 5 stitches and was released to work with no restrictions.
Worker was performing a pre-operational inspection of LH410 at the time of the incident. The worker was checking the filters for the air conditioner which required the hood to be raised on the loader. The worker had EE's left hand on the frame of the housing when the hood came down and struck EE's left thumb.
Miner was stripping off the sheathing from a high voltage cable at the transformer, the hook knife that the miner was using to strip the sheathing slipped causing a laceration to the little finger on the left hand requiring 3 stitches.
EE was walking out of B20 drill station after returning tool when ee fainted. EE reports became weak. Transported to surface then to hospital as precaution. EE assessed and treated for dehydration and sent home. See attached reports Ambient air temp 60 deg, Fahrenheit, in fresh air ventilation drift, 5 gal water coolers at set up.
An employee was attempting to clean out a previously blasted vertical drill hole with an air hose. A sudden release of pressure from an unknown source inside of the hole forced material to be ejected from the hole and struck the employee. The employee passed away from the incident.
After mucking employee began setting area up to drill and became light headed. As the area had been blasted on previous shift exposure to blast gases were thought to be a potential issue. However, gas sampling preformed in area and blood gas testing revealed only a moderate level of carbon monoxide was present. 7000-1 is being submitted as a precaution.

Employee was drilling hole to install ground support, when hole intercepted previously installed rock bolt. Upon contact, the machine stalled, causing it to move upwards suddenly, striking employee on left upper lip. 5 stitches received.
